超低出生体重児(ELBW)児46例につき、母体および胎児の臨床背景、tocolysisの有無と妊娠分娩経過、その後の児の経過を4群に分けて検討した。1)母体因子によるELBW児10例:母体の入院理由は母体妊娠高血圧症候群(HDP)悪化に伴うものが8例、CAOS(慢性常位胎盤早期剥離)疑いを含む常位胎盤早期剥離が2例であった。terminationの理由は母体HDP悪化が6例、胎児の状態悪化が4例であった。新生児死亡は3例(30%)に認めた。2)胎児因子によるELBW児6例:母体の入院理由は子宮内胎児発育不全に伴う胎児機能不全疑いが5例、双胎間輸血症候群による胎児機能不全疑いが1例であった。terminationの理由は胎児の状態悪化であった。新生児死亡は0例(0%)であった。3)10日以内のtocolysis施行14例:母体の入院理由は前期破水12例、胎胞脱出4例、子宮頸管短縮1例、陣痛様の腹緊増強、疼痛4例であった。terminationの理由は陣痛発来、発熱など感染徴候によると考えられるもの19例、トラケレクトミー後で子宮頸部腫瘍再発1例、高度胎胞脱出1例であった。新生児死亡は3例(14.3%)であった。4)10日間以上tocolysisを行えた9例:母体の入院理由は頸管長高度短縮5例、胎胞脱出2例、高位破水2例であり、入院時に明らかに臨床的羊膜絨毛膜炎と診断された症例は認めなかった。tocolysisは塩酸リトドリン点滴7例、塩酸リトドリン+硫酸マグネシウム点滴2例であった。terminationの理由は陣痛発来6例、高度の胎胞脱出2例、完全破水1例であった。新生児死亡は2例(22%)であった。

癒着胎盤の帝王切開術に際し、両側総腸骨動脈バルーン閉鎖術(BO)を併用した7例(BO群)と、併用しなかった7例(非BO群)の成績を比較検討した。その結果、術中出血量は2群間で有意差を認めなかった。だが、BO群では出血量減少に寄与したと考えられる症例が認められ、合併症も少なかった。以上、癒着胎盤における帝王切開術時のBO併用は有用な止血法である可能性が示唆された。

Lynch syndrome is caused by germline mutations in the mismatch-repair genes， which increase the risk of certain cancers. The MSH2， MLH1， MSH6， and PMS2 genes are known as causative genes， and deletions of the epithelial cell adhesion molecule (EPCAM) gene located upstream from the MSH2 gene can also cause this syndrome. We report a client who was highly suspected of having Lynch syndrome from her family history and received a presymptomatic genetic testing without any proband genetic information because of no survivors with cancer in her family.

切迫早産治療中に塩酸リトドリンによる薬剤性無顆粒球症および薬剤性肝障害を発症し周産期管理に苦慮した一例

症例は35歳女性で、0経妊0経産で、原発性不妊症に対し凍結胚移植を施行され妊娠した。妊娠糖尿病を疑い精査したがOGTT試験は陰性であった。妊娠32週0日分娩目的で受診し、頻回の子宮収縮、子宮頸管長短縮を認め、入院管理となった。リトドリン点滴投与し、経過観察したが不規則な子宮収縮が継続し、増量し、その後、頻回の子宮収縮を認めず、同量でリトドリン点滴投与を継続した。35週3日の定期採血検査で著明な白血球、好中球の減少を認めた。リトドリンによる薬剤性無顆粒球症を強く疑い、リトドリン点滴を中止し、塩酸イソクスプリン点滴へ変更した。薬剤変更後も特に子宮収縮増悪は認められなかった。感染予防目的に患者を個室隔離とし、G-CSF製剤ならびにセフェピム塩酸塩水和物(CFPM)投与を開始した。また、薬剤性と思われる軽度肝機能障害も認めたため、グリチルリチン製剤投与も開始した。妊娠35週5日には白血球漸増、肝逸脱酵素減少を認めた。妊娠36週には総白血球数、好中球数は著変ないものの、肝逸脱酵素は正常範囲内まで下降したためグリチルリチン製剤を中止した。イソクスプリンは60mg/日内服に変更後、妊娠36週2日には総白血球数6320/μL、好中球数3918/μLと正常範囲内まで回復した。患者の個室隔離を解除し、G-CSF製剤とCFPMの投与を終了した。退院後、外来経過観察としたが、その後、白血球数の再下降は認められず、妊娠38週3日には自然陣痛が発来し、経腟分娩された。出生児は4044g(HFD)の男児でApgarスコア8/9点、臍帯動脈血ガス所見はpH7.264であった。産褥経過は特に問題なく、産後5日目に退院した。

Atypical polypoid adenomyomas (APAMs) are rare uterine tumors that occur predominantly in premenopausal women. They are benign mixed epithelial and mesenchymal tumors; however, they may recur after treatment, and may coexist with complex atypical endometrial hyperplasia (C-AEH) and endometrial carcinoma. A 36-year-old woman presented to our facility with infertility and intrauterine tumor. She underwent transcervical resection and laparoscopic tubectomy for in vitro fertilization-embryo transfer. Pathological examination after surgery revealed APAM with CAEH. Subsequently, the patient was treated with medroxyprogesterone acetate to preserve fertility. Optimal management of APAM has not yet been established. However, fertility preservation should be considered, under close monitoring, for women who desire to get pregnant.

Objective: To evaluate the factors influencing operating time and intraoperative blood loss in patients undergoing total laparoscopic hysterectomy (TLH). Methods: We enrolled 59 patients who had undergone TLH at our institution between March 2013 and March 2014. We examined the correlations between the operative time of each surgical procedure and uterine size (longitudinal diameter, occipitofrontal, diameter, and transverse diameter) and between operative time and body mass index. Results: Regression analyses revealed that transverse diameter of the uterine corpus affected the operating time. In addition, we found that only the transverse diameter of the uterine corpus affected intraoperative blood loss while processing the cardinal ligament, and that body mass index affected the operating time while processing the suture of vaginal stump. Conclusion: Transverse diameter of the uterine corpus is a factor influencing operating time.

A case with no history of exposure to diethylstilbestrol with vaginal clear cell adenocarcinoma

Background : Primary vaginal clear cell adenocarcinoma (PVCCA) is extremely uncommon with squamous cell carcinoma, the most common type of vaginal cancer. PVCCA of the vagina developing in patients without a history of exposure to diethylstilbestrol (DES) and without urogenital anomalies or adenosis is very rare. We present a case of PVCCA in a patient with no congenital urogenital anomalies or history of exposure of DES.Case : The patient was a 50-year-old postmenopausal woman, gravida 1, who presented with abnormal genital bleeding. MRI showed an enhancing tumor in the mid-to lower section of the vagina. The pathological diagnosis was clear cell adenocarcinoma. The patient was treated by local resection and adjuvant chemotherapy in deference to her wish based on her religious beliefs. The final diagnosis was PVCCA, FIGO stage Ⅰ (pT1NXM0). She remains recurrence-free to date.Conclusion : This rare PVCCA may be associated with a poor prognosis. It is important to detect this malignancy its complications in the early stage by cytology.

Objective : To examine the clinical implications of atypical squamous cells of undetermined significance (ASC-US) in cytology of the uterine cervix. Study Design : We examined the cases of ASC-US diagnosed in our hospital : a total of 12141 women who participated in the screening for cancer of the cervix between January 2010 and December 2011 were included in our study. Results : Of the patients, 1.9% were found to have ASC-US, and 66% of these cases had evidence of infection with high-risk HPV. After one-year follow-up, CIN 1/2 was confirmed in 25/21% of the cases. In 14 of the 19 cases of CIN 1, atypical cytological features could not be detected at the follow-up. We treated 10 cases of CIN 2/3 and followed up 6 cases. Multivariate modeling identified only one factor affecting the risk of CIN, that is high-risk HPV infection. Conclusion : The results of the multivariate analysis identified high-risk HPV infection in cases of ASC-US as an independent risk factor for CIN.

A case of ovarian clear cell adenocarcinoma with humoral hypercalcemia of malignancy and positive endometrial smear

Background : Ovarian clear cell adenocarcinoma with malignancy-associated hypercalcemia is rare and associated with a poor prognosis. Perioperative management is therefore significant. Case : This menopausal woman presenting with genital bleeding, thirst and generalized malaise was found to have a left adnexal mass and hypercalcemia. The serum level of parathyroid hormone related protein (PTHrP) was found to be elevated. Endometrial cytology showed columnar cells with irregular nuclear distances, dense chromatin and nuclear anisocytosis were seen in a serum background. It showed a Marimo-like arrangement. That cells might migrate from the ovary. After surgery and staging, the postoperative diagnosis was stage IIc clear cell adenocarcinoma of the ovary with humoral hypercalcemia of malignancy. Immunohistochemical staining showed that the cells were weakly positive for PTHrP. She received adjuvant therapy with carboplatin and paclitaxel. On long-term follow-up, the patient has remained well. Conclusion : This rare paraneoplastic manifestation of ovarian cancer may be associated with a poor prognosis. Immunohistochemical positivity for PTHrP is useful for making the diagnosis. In the current case, PTHrP became a useful marker of recurrence. It is important to detect this malignancy with its complications in the early stage by laboratory tests, cytology and immunostaining.

30歳女性(未経妊)。前医より子宮附属器茎捻転に伴う急性腹症のため救急搬送され、腹部エコーで腹水の貯留と右下腹部から上腹部に及ぶ充実成分を含む巨大卵巣腫瘍を認め、悪性腫瘍などが疑われた。精査を行なったところ、採血ではAFPの異常高値がみられ、開腹手術では軽度の血性腹水が貯留し、右卵巣腫瘍は3回転捻転し被膜破綻していた。対処として子宮右附属器切除術+膀胱子宮窩の播種病巣切除術を施行した結果、病理診断では右附属器、播種病巣とも未熟奇形腫であり、未熟な神経管様構造が多数認められ、卵巣癌のStage 2bであった。術後は化学療法としてBEP療法が行われたが、1コース目終了時点でGrade 3の好中球の減少を認めたため2コースと3コースでは減量が行われた。以後、頸部色素沈着以外は特に問題なく化学療法を終了したものの、術後2年3ヵ月目に経腟超音波でダグラス窩に32×27mm大の辺縁明瞭で内部構造が粗造な腫瘤が確認された。精査によりGrowing teratoma syndrome(GTS)が強く疑われ、病巣の生検と治療目的で腹腔鏡下切除術を行なったところ、病理診断は骨盤底病変および腹膜のサンプリング部位ともに成熟奇形腫であったが、原発巣に認められた未熟神経管組織や悪性所見はみられなかった。尚、術後にAFPの変動はなく、再発の徴候も認められていない。

Endometriosis in a Canal of Nuck Hydrocele that Contained a Metallic Clip used in Laparoscopic Cholecystectomy

A 43 year-old woman, gravida 3, para 3, presented with a right inguinal mass. It was growing slowly and associated with cyclic pain during the menstrual cycle. She had no other symptoms such as dysmenorrhea or pelvic pain. On examination, the right inguinal mass measured 50 mm. It was relatively fixed and nontender. Laboratory values were normal, including CA19-9, CA125, and CEA. She had a history of a laparoscopic cholecystectomy at 33 years of age. Magnetic resonance imaging (MRI) showed a multiple cystic lesion with in inguinal fatty tissue or in the inguinal canal; it measured 52 mm at its larger axis. We diagnosed it as inguinal endometriosis and resected the tumor. A 30 × 50 mm cystic tumor was removed; it contained a metallic clip. The pathological examination was reported as endometriosis in a canal of Nuck hydrocele. The origin of the metallic clip was thought to be an instrument, which was used in laparoscopic cholecystectomy, because she had no history of trauma or surgery in the inguinal area. The metallic clip migrated from the cystic duct or the artery of gallbladder, transited the inguinal canal, then fell into the canal of Nuck hydrocele. This is a very rare case of endometriosis in a canal of Nuck hydrocele, which contained a metallic clip.

35歳(初妊初産婦)。祖母、父、叔母、姉にヘテロ型プロテインC欠損症を認め、本人31歳時に6年間の不妊で某医を受診し、子宮鏡下ポリープ切除術、開腹下癒着剥離術を受け、1ヵ月後に左下肢と肺に血栓形成を認め、ヘテロ型プロテインC欠損症の診断でワルファリン内服を行っていた。妊娠を希望しワルファリンを自己中止し、膝裏に疼痛と浮腫を認め、前医で妊娠と下腿に血栓を認め当院へ搬送・入院した。妊娠9週2日で胎児発育に問題なく、母体に総腸骨静脈分岐部から下腿に広汎な血栓を認め、妊娠継続希望でヘパリンナトリウムを開始し、自覚症状消失でヘパリンカルシウム自己皮下注、アスピリン内服の外来とした。妊娠40週1日に陣痛発来で入院し、経腟分娩で3060gの男児を出産した(Apgar score 8/9)。分娩後出血の止血後にヘパリンナトリウムを開始し、産褥3日よりワルファリンを併用した。以後順調に経過し、血栓の発生は認めず産後13日目に退院した。

Objective: To report a rare case of spontaneous ovarian hyperstimulation syndrome (OHSS) associated with spontaneous pregnancy and a FSH-secreting pituitary adenoma. Design: Case report. Setting: University hospital. Patient(s): A 32-year-old woman with spontaneous OHSS. Intervention(s): Transsphenoidal resection of the tumor. Main Outcome Measure(s): Regression of the symptoms of OHSS and hyperestrogenemia. Result(s): At presentation, the patient's ovaries were markedly enlarged and massive ascites was seen. Her serum E-2 level was markedly elevated, but her LH level was low, and FSH was within the normal range. In addition, her TSH level was normal, and hCG was appropriate for the date of pregnancy. Subsequently, the patient developed massive thrombophlebitis in her right internal jugular and subclavian veins. Termination of the pregnancy ameliorated the accumulation of ascites, but ovarian enlargement and hyperestrogenemia persisted. No mutations of the FSH receptor, LH receptor, or aromatase genes were detected, but magnetic resonance imaging (MRI) of the head revealed a pituitary adenoma. After transsphenoidal resection of the tumor, the patient got better. Conclusion(s): A gonadotropin-secreting adenoma caused ovarian hyperstimulation (ovarian enlargement and hyperestrogenemia). In addition, spontaneous pregnancy and intrinsic hCG increased vascular permeability, which complicated the patient's disease. (Fertil Steril(R) 2009;92:390.e1-e3. 1989年以降に治療を行ったBulky腫瘍を有する臨床進行期IbからIIIbの子宮頸部腺癌で,術前paclitaxel+CBDCA併用療法(TJ療法)を施行した5例について,従来のPAM療法を施行した28例との間で臨床的比較検討を行い,術前化学療法(NAC療法)としてのTJ療法の有用性を検討した.PAM療法群はCDDP 70mg/m2,AclacinomycinA 15mg/m2,MitomycinC 10mg/m2を4週ごとに2〜3コース,TJ療法群はPTX 175mg/m2,CBDCAをACU6にて4週ごとに2〜3コース施行後に,根治手術を施行した.TJ療法後,5例中4例,80%で腫瘍サイズの減少(縮小率61〜89%)を認めた.また,画像診断上のTJ療法後の骨盤リンパ節および子宮傍組織浸潤の所見と実際の摘出物の所見はほぼ相関し,TJ療法のNAC療法としての良好な一次治療効果を示したものと考えられた.副作用の発現はGrade3の好中球減少の発現がやや多いものの,G-CSFの投与にてコントロール可能であった

In the present study, we analyzed the immunohistochemical findings for the RA receptor (RAR), retinoic X receptor (RXR) and hypermethylation of promoter-region CpG island methylation of RAR-beta2. Immunohistochemistry indicated that though RXR-alpha and -gamma were present in endometrial hyperplasia and cancer, other retinoid receptors were only weakly detected. The hypermethylation of RAR-beta2 was found in 75.0% of endometrial hyperplasia samples and 92.2% of carcinomas. No normal endometria had methylation. This evidence may point to one of the reasons why endometrial hyperplasia acquires high proliferative capacity without differentiation, and the hypermethylation of RAR-beta2 may occur in the early stage of endometrial carcinogenesis. Nerve-preserving techniques for radical hysterectomy

Aims. Postoperative bladder dysfunction of varying severity is common after radical hysterectomy due to damage to the pelvic autonomic nerves during surgical resection. Pelvic autonomic nerve damage occurs at two main sites: the fibrous tissue at the lateral aspect of the uterosacral. ligament (FLUSL) and the posterior layer of the vesicouterine ligament (PLVUL). In this article, we classify the main sites of pelvic autonomic nerve damage based on previous reports and our own experience, outline the problems with current nerve-preserving techniques, and describe an improved nerve-preserving technique that we have developed. Results. We present urodynamic data for 25 patients who underwent radical hysterectomy using the nerve-preserving technique we described previously and preliminary results for four patients treated by the latest technique. To avoid pelvic autonomic nerve damage, injury to the FLUSL and PLVUL should be minimized. For the FLUSL, we have obtained relatively good Madder function with the cardinal ligament pull-through technique. However, a new method is needed for management of the PLVUL, since the only way to minimize nerve damage at this site currently is to limit the extent of PLVUL resection. (C) 2004 Elsevier Ltd. Identification of SCN3B as a novel p53-inducible proapoptotic gene

Tumor suppressor p53 is a transcription factor that induces growth arrest and/or apoptosis in response to cellular stress. To identify novel p53-inducible genes, we compared the expression of genes in normal mouse embryo fibroblasts (MEFs) to p53-null cells by cDNA representational difference analysis. We report here that expression of endogenous sodium channel subunit beta 3 (SCN3B) is upregulated in mouse embryonic fibroblasts by DNA damage in a p53-dependent manner. In addition, we found that SCN3B levels are upregulated in human cancer cell lines by DNA damaging agents, as well as by overexpression of p53, but not significantly by p63 or p73. Furthermore, we identified two putative p53-binding sites upstream of the first exon (RE1) and in the third intron (RE2). The p53 protein can directly interact with the putative p53-binding sites in vivo, as assessed by chromatin immunoprecipitation. A reporter gene assay revealed that these two p53-binding sites are functional response elements. The SCN3B protein appears to be localized to the endoplasmic reticulum (ER). Introduction of the SCN3B gene into T98G and Saos2 cells potently suppressed colony formation. Furthermore, we found that adenovirus-mediated transfer of SCN3B induced apoptosis when combined with anticancer agents. The results presented here suggest that SCN3B mediates a p53-dependent apoptotic pathway and may be a candidate for gene therapy combined with anticancer drugs.

STUDY DESIGN: There were 8 cases of uterine carcinosarcoma (CS), 2 of leiomyosarcoma, 2 of endometrical stromal sarcoma (ESS), 1 of endocervical stromal sarcoma (ECSS) and 1 of malignant fibrous histiocytoma (MFH) of the vagina. The presence of sarcomatous components was retrospectively investigated by microscopic observation of preoperative specimens from the endocervical canal and endometrial cells. Characteristic features of sarcomatous cells were then investigated by cytodiagnostic micrometry of malignant cells. RESULTS: Of the 14 patients, 1 with low grade ESS and 1 with homologous CS were diagnosed as negative for sarcomatous components. One case of high grade ESS had been overlooked, as were 4 cases of CS. Thus, 7 cases (50%) were diagnosed as positive for sarcomatous cells by preoperative cytologic observation. Based. on these findings, 12 of the 14 cases (85.7%) were positive for sarcomatous elements on retrospective reexamination of the specimens. CONCLUSION: Careful attention should be paid to small sarcomatous cells since cases of ESS or ECSS with such cells show morphologic characteristics similar to those of stromatous cells. Furthermore, careful microscopic observation of an entire specimen is required to avoid misdiagnosis as carcinoma since it is easy to overlook sarcomatous elements in smears with carcinosarcoma if there are only a few sarcomatous cells.

Overexpressed progesterone receptor form B inhibit invasive activity suppressing matrix metalloproteinases in endometrial carcinoma cells

In this study, we focused on the influence of progesterone and its receptor in invasion and MMPs on endometrial carcinoma cells. The growth of lshikawa cells, to which an progesterone receptor form B (PR-B) expressing vector was transfected, was inhibited by progesterone as was the inhibition of the expression of cyclin D1. By invasion assay, in conditions with progesterone, the invasiveness of Ishikawa cells was inhibited as well as the expression of (metalloproteinase) MMP-1, -2, -7 and -9 and Ets-1 decreased. These results suggest that activation of PR-B by progesterone results in tumor suppression by inhibiting cell growth and invasiveness via suppression of the expression of MMPs. (C) 2004 Elsevier Ltd. Neoadjuvant chemotherapy with cisplatin, aclacinomycin A, and mitomycin C for cervical adenocarcinoma - a preliminary study

Between 1989 and 2002, 28 patients with locally advanced cervical adenocarcinoma (bulky IB-IIIB) were recruited for a pilot study aimed at evaluation of the effectiveness of neoadjuvant chemotherapy with cisplatin, aclacinomycin-A, and mitomycin-C (PAM), followed by radical surgery. This regimen was administrated intra-arterially or intravenously. In addition to patients treated with PAM, we retrospectively analyzed the prognoses of 26 patients in stage I and II, who had been treated between 1975 and 1981 with radical surgery with/without radiation therapy. Twenty-eight patients received PAM therapy as neoadjuvant chemotherapy, and 75.0% of the 16 intra-arterially infused patients showed a response, as did 66.7% of the 12 intravenously infused patients. There was a significant difference in the 5-year prognosis of stage II (PAM group, 72.9%; without-PAM group, 36.4%). The results suggest that, as the free space in the parametrium is widened by neoadjuvant chemotherapy with PAM, it is possible that the tumor could be completely resected by radical hysterectomy. Thus, neoadjuvant chemotherapy with PAM is expected to improve the survival rate of patients with advanced cervical adenocarcinoma by the preliminary study. However, the survival rates of stage II with lymph node metastasis in the without-PAM group seem low, and we must also consider that the various technologies to evaluate and treat the cervical adenocarcinomas, e.g. computed tomography, magnetic resonance imaging, and surgical equipments, had improved during 1989-2002 than was the scenario during 1975-1981, and these improvements contributed to better prognosis. A prospective-randomized study is needed to assess the value of this approach compared with standard management.

Expression of uteroglobin in normal and carcinogenic endometrium and influence of hormone replacement therapy

Uteroglobin, first reported in 1968 as a steroid secreted in rabbit uterine fluid during early pregnancy, is a progesterone-regulated and progesterone-binding protein. There is evidence that indicates that uteroglobin is inversely correlated to neoplastic growth but its role to endometrial carcinogenesis is not known. Therefore we analyzed the expression of uteroglobin in 13 normal endometrium, 19 hyperplasia and 21 endometrial carcinoma samples and the relation to estrogen receptor-alpha (ER-alpha) and progesterone receptor (PR) by immunohistochemistry and Western blotting. We also analyzed the expression of uteroglobin in 15 menopausal women who received hormone replacement therapy (HRT). The expression of uteroglobin was higher during the secretory phase than in the proliferative phase; however, it was detected in endometrial hyperplasia as weakly as in the proliferative phase and decreased according to the loss of differentiation in endometrial carcinoma. The results were basically in accord with those for PR; however, the expression of uteroglobin was weak, though PR was most detected in endometrial hyperplasia. In menopausal endometrium,the group treated with estrogen plus progesterone exhibited higher expression of uteroglobin than the group treated only with estrogen. The evidence suggests that uteroglobin expression is regulated by progesterone in the normal endometrium but that the regulation by PR is lost in endometrial hyperplasia and carcinoma according to acquirement of tumorigenesis and that estrogen plus progesterone therapy reduces the risk for endometrial carcinoma by restoring uteroglobin. Overexpression of estrogen receptor-alpha gene suppresses gap junctional intercellular communication in endometrial carcinoma cells

Stimulation of the endometrium by estrogens without the differentiating effect of progestins is the primary etiological factor associated with the development of endometrial hyperplasia and adenocarcinoma. However, the correlation between sex steroids and gap junctional intercellular communication (GJIC), which is considered to play an important role in the control of cell growth and differentiation, is not well known in endometrial carcinoma. In this study, we focused on the influence of estrogen and its receptor in connexin (Cx) expression and GJIC in endometrial carcinoma cells, established stable clone IK-ER1 overexpressing ER-alpha to transfect the expression vector and analysed them in various hormonal conditions. The growth of IK-ER1 was accelerated by 17beta-estradiol and the acceleration of the 5-bromo-25-deoxyuridine labeling index was observed. GJIC was assayed by scoring the number of dye-coupled cells after microinjecton of single cells with Lucifer-Yellow, and subcellular localization of Cx26 and Cx32 was analysed by immunocytochemistry. In the presence of estradiol, dye-coupled cells of IK-ER1 were significantly reduced compared to those without estradiol and the reduction was completely inhibited by adding ICI182.780, a pure antiestrogen substrate. Cxs were detected as only small spots by immunocytochemistry, and Western blotting showed that the expression was decreased. These results suggest that activation of ER-alpha by estrogen results in tumor progression by stimulating cell growth and suppressing GJIC via suppression of the expression of Cxs in endometrial carcinogenesis.

Correlation between responsiveness of neoadjuvant chemotherapy and apoptosis-associated proteins for cervical adenocarcinoma

Objective. Adenocarcinoma of the uterine cervix appears to be increasing in prevalence and it has been suggested that these tumors tend to be less sensitive to radiation therapy and to chemotherapy than squamous carcinomas. In the present study, 29 patients with locally advanced cervical adenocarcinoma (bulky IB-IVB) were treated with neoadjuvant chemotherapy (NAC) using cisplatin, aclacinomycin-A and mitomycin-C, followed by radical surgery or irradiation. Methods. To predict the prognosis and response to the chemotherapy, the expression of apoptosis associated-proteins, p53, p21WAF1/ CIP1, Bcl-2 and activated caspase-3 was evaluated for tumor samples by immunohistochemistry. Results. Of the analyzed clinicopathological factors, the overexpression of p53 was frequently observed in endocervical-type adenocarcinoma, nonresponders to chemotherapy and the grade 0 histologic effect of the chemotherapy. Positive staining of Bcl-2 was frequently observed in the early stage and had a better prognosis than for patients with the negative staining; however, there was no correlation between responders and nonresponders to chemotherapy. The expression of p21 WAF1/CIP1 and caspase-3 was not correlated to the clinicopathological factors. Conclusion. In this study, the overexpression of p53 was found to be a factor to predict the chemoresistance and positive expression of Bcl-2 indicated as a better prognostic value. The reproducibility of a binary tumor grading system for uterine endometrial endometrioid carcinoma, compared with FIGO system and nuclear grading

Objective: A binary grading system has been proposed to assess the amount of solid growth, the pattern of invasion, and the presence of necrosis, and thereby divide endometrial endometrioid carcinomas into low- and high-grade tumors. We analyzed this system for predicting the prognosis, with respect to inter- and intraobserver reproducibility and treatment modalities. Methods: A total of 200 endometrial carcinomas, based on hysterectomy specimens, were graded according to the binary grading system, for comparison against The International Federation of Gynecology and Obstetrics (FIGO) system and nuclear grading. Results: Both inter- and intraobserver agreement using the binary grading system (kappa = 0.57; percent agreement: 82% and kappa = 0.62; 84%) were superior compared with the FIGO system (0.50; 60% and 0.62; 73%) and the nuclear grading (0.23; 49% and 0.43; 65%). Patients with early-stage low- grade tumors had a 98% rate for 5-year survival (5YS). Patients with early-stage high-grade tumors, and those with advanced-stage low-grade tumors, had respectively 86% to 87% rates for 5YS. But patients with advanced-stage high-grade tumors had a 49% rate for 5YS. In binary low-grade early-stage tumors, the patient outcome was better with no adjuvant therapy and chemotherapy, compared with other therapies. Conclusion: A binary grading system was superior to others in permitting greater reproducibility and predicting the prognosis of endometrial cancer patients. Copyright (C) 2004 S. Karger AG, Basel.

Changed expression of heat shock proteins in various pathological findings in placentas with intrauterine fetal growth restriction

Heat shock proteins (HSPs) are activated in the cells of most organisms in response to sublethal heat shock and other stressors. It has been reported that HSP27, HSP60, HSP70, and HSP90 are expressed in normal human placenta, and it was thought that these HSPs play a role in the demonstration of cell viability and function. In this study, we performed an immunohistochemical (IHC) study of these HSPs for 27 placentas that had complicated intrauterine fetal growth restriction (IUGR) and compared the IHC findings with the pathological findings. To quantify HSP27, HSP60, HSP70, and HSP90, immunoreacted cells in the chorionic villi, syncytiotrophoblasts (ST), and cytotrophoblasts (CT) were counted. In thrombus, excessive syncytial knots, and avascular villi, the expression of HSPs was higher in the pathological sections compared to control in both ST and CT. In contrast, all HSPs decreased in both ST and CT around the infarction region. The data suggested that chorionic villi cells locally responded to some stresses, e.g., hypoxia and increase or decrease in the expression of HSPs. Although the villous cells around the infarction histologically appear viable, they may have received lethal damage, and as a result the expression of HSPs was decreased. These results are expected to improve our understanding of the pathological findings of IUGR in placentas, including the quality, damage, and function of the chorionic villi. © The Clinical Electron Microscopy Society of Japan 2004.

Suppression of gap junctional intercellular communication via 5 ' CpG island methylation in promoter region of E-cadherin gene in endometrial cancer cells

Previously, we demonstrated that connexins (Cxs) showed aberrant localization and expression in most endometrial hyperplasia and carcinoma samples, indicating that during endometrial carcinogenesis, loss of gap junctional intercellular communication (GJIC) may occur at relatively early stages. In the present study, we focused on the correlations between GJIC and the expression of the E-cadherin and its 5' CpG island methylation in endometrial cancer cells and tissues to investigate their roles in the carcinogenesis and tumor progression of endometrial cancer. In this study, three of the 10 cell lines investigated, Ishikawa, RL-952 and KLE, in which both Cxs and E-cadherin mRNA were expressed, exhibited GJIC by scrape-loading/dye transfer. On the other hand, the other seven cell lines, in which either or both Cxs and E-cadherin mRNA were negative or weakly expressed, did not show GJIC. HEC-50, HEC-1B and HEC-108, in which Cxs were positively expressed but E-cadherin was negatively expressed, showed cytoplasmic localization of Cxs by immunohistochemistry. All five lines, which showed the weak expression of E-cadherin, had E-cadherin 5' CpG island methylation. By immunohistochemistry of 56 endometrial carcinomas, 13 of 27 methylated samples showed weak expression of Cx26 and the other 14 showed diffuse localization in cytoplasm. On the other hand, of 29 unmethylated samples, two showed cell-cell localization, 25 weak expression and two diffuse localization. Furthermore, E-cadherin expression was revealed to be drastically down-regulated by E-cadherin antisense oligonucleotides that post-transcriptionally down-regulated E-cadherin expression and in the cell, the localization of Cxs were changed from the cell-cell borders to the cytoplasm, and GJIC also decreased. The results indicated that 5' CpG island methylation, which caused loss of E-cadherin expression, indirectly caused the suppression of GJIC by aberrant localization of Cxs in endometrial carcinoma cells.

Three-dimensional coculture of endometrial cancer cells and fibroblasts in human placenta derived collagen sponges and expression matrix metalloproteinases in these cells

Objective. Collagen gel constitutes a valuable tool for the study of cell-matrix interactions; however, it is sometimes difficult to use the gel, in which tumor and stromal cells are cocultured, for immunohistochemistry, because it is easily broken during the process of fixation and embedding in paraffin, especially after long-term culture. Methods. To examine the interaction between endometrial cancer cells and fibroblasts in tumor invasion, we carried out three-dimensional (3-D) coculture of various endometrial cancer cell lines and fibroblasts in human placenta derived collagen sponges and analyzed the expression and localization of matrix metalloproteinases (NIMP) and plasminogen activators (PA) in these cells by immunohistochemistry. Results. After 4 weeks of culture on the collagen sponges, endometrial cancer cells composed stratiform or glandular structures on the layer of extracellular matrix, which was composed from fibloblasts and extracellular matrix. Compared to Ishikawa cells, which were rarely invasive, HEC-1A and HEC-1BE and cocultured fibroblasts showed high invasiveness and strong expression of some proteins. In cell line HEC-1BE, MMP-1, -7, and -9, MT1-MMP, tissue inhibitors of metalloprotemases 2, and uPA showed intensive staining in both cancer cells and fibroblasts by immunohistochemistry. HEC-1A cells and cocultured fibroblasts showed expression patterns similar to those of HEC-1BE. Conclusion. These results suggested that expression of MMPs and uPA was accelerated in fibroblasts surrounded by cancer cells. We believe that our 3-D coculture system has merit in that the interaction between cancer cells and stromal cells can be visually analyzed by immunohistochemistry and that experiments for a long period, at least 2 weeks, are possible. Treatment and prognosis of the invasive cervical cancer with pregnancy

Hypermethylation in promoter region of E-cadherin gene is associated with tumor dedifferention and myometrial invasion in endometrial carcinoma

BACKGROUND. Loss of E-cadherin expression is associated with aberrant 5' CpG island methylation in various tumors. METODS. The authors analyzed the methylation status and immunohistochemical expression of E-cadherin in 142 endometrial tissues, consisting of 21 normal endometria, 17 endometrial hyperplasias, and 104 endometrial carcinomas. RESULTS. All normal endometria and endometrial hyperplasias showed positive staining of E-cadherin, and methylation of the E-cadherin gene was not detected in any samples. In endometrial carcinoma, the positive ratio of methylation was higher and was associated with tumor dedifferention and myometrial invasion. In G1 endometrial adenocarcinomas, 66.7% showed positive staining and 33.3% showed heterogeneous staining. Methylation of the E-cadherin gene was detected in 15.6%. In G2 tumors, 19.0% showed positive staining, 69.0% showed heterogeneous staining and 11.9% showed negative staining. Methylation of the E-cadherin gene was found in 50.0%. In G3 tumors, 9.1% showed positive staining, 54.5% showed heterogeneous staining and 36.3% showed negative staining. Methylation of the E-cadherin gene was found in 81.8% of the tumors. Of the samples with no-myometrial invasion, 23.1% had methylation. In those with invasion in less than half of the myometrium, 28.6% did and in those with invasion of half or more of the myometrium, 55.6% had methylation. Of samples that did not have lymph node metastasis, 33.7% had methylation, whereas of samples that had lymph node metastasis, 60.0% had methylation. CONCLUSIONS. This is the first report to analyze methylation of the E-cadherin gene promoter of endometrial carcinoma and the evidence suggests that methylation of the E-cadherin gene occurs in association with the acquisition of invasive capacity.

Usefulness of silver intensification of immunostaining for cytologic diagnosis of primary melanoma of the female genital organs

OBJECTIVE: To improve the procedure for diagnosing vaginal melanoma with cytopathologic analysis of HMB-45. STUDY DESIGN: The study examined silver intensification of immunostaining of HMB-45 in nine cases of primary melanoma of the vagina and vulva using archival Papanicolaoustained smears. RESULTS: All nine samples showed positive staining for HMB-45. Five cases showed intensive staining, two moderate and two weak. The positive staining was black in the cytoplasm of melanoma cells but was detected in neither the background nor normal squamous cells. Though destaining of Papanicolaou stain was not performed before immunostaining, the positivity of immunostaining was easily judged. CONCLUSION: After morphologic observation, immunocytochemical study of HMB-45 is possible even though time has passed since the cytologic specimen was obtained. When there is a suspicion of amelanotic melanoma or scantily pigmented melanoma of the vagina and vulva, cytogenesis with HMB-45 is helpful, especially because it involves little invasion.

Imaging studies in patients with malignant melanoma in the female genital tract

The purpose of this study was to review magnetic resonance imaging (MRI) and pathologic features of primary malignant melanoma (melanoma) in the female genital tract. We retrospectively evaluated MRI in six women with melanoma of the genital tract. The signal intensity of the tumor on T1-weighted images (WI) was compared with the amount of melanin granules in hematoxylin-eosin stained sections of resected specimen. On T1WI, four melanomas showed a high signal intensity, one intermediate, and one low. The four melanomas with a high signal intensity on T1W1 were rich in melanin granules, while the one intermediate tumor had few granules. The other one was amelanotic. We believe that a high signal on T1WI is characteristic of primary melanotic melanoma of the female genital tract. Our findings suggest that it is strongly influenced by the presence of melanin granules.

Acceleration of invasive activity via matrix metalloproteinases by transfection of the estrogen receptor-alpha gene in endometrial carcinoma cells

It is well known that the functions of reproductive organs are regulated by sex steroids and their receptors and it is hypothesized that the progression of neoplasms that originate from the reproductive organs is influenced by them. However, the correlation between sex steroids and tumor progression, especially tumor invasion, is not well known in endometrial carcinoma. In our study, we focused on the influence of estrogen and its receptor in invasion and matrix metalloproteinases (MMI's), which are known to be important in tumor invasion, as well as on endometrial carcinoma cells. The growth of Ishikawa, cells, to which an estrogen receptor-alpha expressing vector was transfected, was accelerated by 17beta-estradiol as was the acceleration of the expression of cyclin D1. By invasion assay, in conditions with 17beta-estradiol, the invasiveness of Ishikawa cells was enhanced. Furthermore, according to the accelerated invasiveness, the expression of MMP-1, -7 and -9 and Ets-1 was enhanced. These results suggest that activation of ER-alpha by estrogen results in tumor progression by stimulating cell growth and invasiveness via acceleration of the expression of MMPs. Loss of gamma-catenin expression in squamous differentiation in endometrial carcinomas

Squamous differentiation occurs in 25 to 50% of endometrial endometrioid adenocarcinomas. In this study, we analyzed the immunohistochemical expression and localization of beta- and gamma-catenin, molecular changes in the beta-catenin gene, and the subcellular localization of the desmosomal component protein, desmoplakin, by laser scanning microscopy in 35 endometrial carcinomas with squamous differentiation. In the glandular component, beta-catenin showed nuclear localization in 10 of 35 (28.5%) samples, whereas in the squamous component, nuclear localization was found in 15 of 35 (42.9%). Of the 15 samples that showed nuclear localization of beta-catenin in the squamous component, seven samples (46.7%) did not express gamma-catenin. The phenomenon was not correlated with mutation in exon 3 region of beta-catenin gene. Furthermore. in these samples, there was diffuse cytoplasmic staining for desmoplakin. These observations have not been reported in other tumors. Our results suggest that unique molecular events. i.e., stimulation of beta-catenin and suppression of gamma-catenin expression. occur within endometrial carcinomas with squamous differentiation.

Expression of matrix metalloproteinases in ovarian endometriomas: immunohistochemical study and enzyme immunoassay

Like carcinoma, endometriosis has the unique characteristics, of invasion and metastasis, though pathologically, it is a benign tumor. However, the mechanism of destruction of the surrounding tissue in endometriosis is still unclear. In this study, the expression and localization of matrix metalloproteinases (MMP)-1, -2, -3, -7, -9 and tissue inhibitors of metalloprotemases-1 (TIMP-1) were evaluated by immunohistochemistry for 20 cases and the amounts of MMP-1, TIMP-1 and MMP-1/TIMP-1 complex in the fluid of endometrioma, were analyzed by ELISA and western blotting for 20 cases, which were analyzed by immunohistochemical study. MMP-1, -2 and -9 were detected strongly in both stromal and epithelial cells and MMP-7 in the epithelial cells in the menstrual period. MMP-3 was mainly expressed in macrophage containing hemosiderin but the change of expression was not clear. TIMP-1 was intensively detected in both stromal and epithelial cells in the menstrual period but the expression decreased in other stages of the menstrual cycle. ELISA for MMP-1 also showed results similar to immunohistochemistry, suggesting that it was released to the cyst in the menstrual period when it was released to the extracellular space from the cytoplasm. The expression of TIMP-I was not clearly changed during the menstrual cycle. From these results, it was suggested that the destruction of the surrounding matrix by endometriosis might be caused by various MMPs, which are mainly produced in stromal cells. Mutation of β-catenin gene in endometrial cancer but not in associated hyperplasia

Endometrial hyperplasia is a recognized effect of excessive or unopposed estrogen stimulation and is considered to be a precancerous condition of endometrial adenocarcinoma. We have previously shown that the subcellular localization of β-catenin in the human endometrium is changed according to cell proliferation, suggesting a role of intercellular transduction in cell-growth control in human endometrium, not only in the physiological condition but also in the carcinogenic endometrium. In the present study, to clarify at which stage of endometrial carcinogenesis molecular alteration of the β-catenin gene occurs, we analyzed the subcellular localization of β-catenin by immunohistochemistry, and we analyzed exon 3 of the β-catenin gene, in 25 patients - with endometrial hyperplasia and 20 patients with endometrial cancers associated with endometrial hyperplasia, digesting DNA from the cancer and hyperplasia parts, separately. Fourteen of the 25 (56.0%) endometrial hyperplasia samples, 12 (60.0%) endometrial cancers, and 11 (55.0%) associated hyperplasias of the 20 endometrial cancers associated with hyperplasia showed nuclear localization of β-catenin. Mutation in exon 3 of the β-catenin gene was found in 2 of the 20 endometrial cancer samples however, it was not found in the 25 endometrial hyperplasias or the 20 associated hyperplasias. The results suggest that molecular alteration of the β-catenin gene occurs in atypical hyperplasia or cancer, rather than in simple or complex hyperplasia without atypia, during endometrial carcinogenesis.

Suppressed gap junctional intercellular communication in carcinogenesis of endometrium

To examine whether and at which stage of endometrial carcinogenesis decreased connexin expression occurs, we investigated changes in the expression of the gap junction proteins, connexin 26 (Cx26), Cx32 and Cx43, in human endometrial hyperplasia and cancer samples. Forty-eight endometrial tissue samples (15 endometrial hyperplasias and 33 endometrial cancers) were subjected to immunofluorescence and RT-PCR analysis, In endometrial hyperplasia, Cx26 war aberrantly expressed in all samples as revealed immunohistochemically. There was weak or negative expression in 12 samples (80.0%) and diffuse expression in cytoplasm in 3 samples (20.0%), Cx32 expression in those samples was similar to that of Cx26; there was weak or negative expression in II samples (73.3%) and diffuse expression in 4 samples (26.7%), In endometrial cancer, Cx26 was expressed weakly or negatively in 25 samples (75.8%), diffusely in 6 samples (18.2%) and normally in 2 samples (6.1%), while Cx32 was expressed weakly or negatively in 26 samples (78.8%), diffusely in 5 samples(15.2%) and normally in 2 samples (6.1%). It was confirmed that weak staining of Cx26 and Cx32 was due to poor expression of their mRNA, All samples showed weak Cx43 protein expression as revealed by immunohistochemical analysis, In the majority of samples, concomitant expression levels of Cx26 and Cx32 protein were observed, confirming our long-term hypothesis that Cx26 and Cx32 are both abnormally regulated in a coordinated fashion in the endometrium, Our results indicate that during endometrial carcinogenesis, loss of gap junctional intercellular communication (GJIC) may occur due to the suppressed expression and the aberrant localization of connexin at relatively early stages. Changes in expression and subcellular localization of nuclear retinoic acid receptors in human endometrial epithelium during the menstrual cycle

The endometrium is a uniquely dynamic tissue in that it undergoes monthly cycles of proliferation and secretory activity, and is regulated by ovarian steroid hormones. In this study, we focused on retinoic acid receptors (RAR and RXR) which are ligand-dependent transcription factors belonging to the large family of steroid hormones and are expected to affect to cell growth and differentiation in the endometrium. We analysed the expression and subcellular localization of the RA receptors in 57 samples of human endometrium by immunohistochemistry and Western blotting. In the nuclei of the endometrial epithelium, the RA receptors were expressed strongly in the proliferative phase, However, RAR were drastically reduced in the epithelial nuclei during the secretory phase in association with changes in serum oestradiol and in the expression of the oestrogen receptor. The expression of RXR was localized in the epithelial nuclei throughout the menstrual cycle. Confocal laser scanning microscopical observation clearly showed the difference in the localization between RAR and RXR in the secretory phase. Furthermore the findings of immune-electron microscopy showed pooled RAR around the rough endoplasmic reticulum, suggesting that transport of these receptors to the nuclei is inhibited. These findings suggest that RAR and RXR work mainly in the proliferative phase and that in the endometrium RXR may play a different role to RAR during the secretory phase.

HMB-45 staining for cytology of primary melanoma of the vagina - A case report

BACKGROUND: Malignant melanoma in the vagina is very rare, but its diagnosis is usually easy if a melanin pigment is present. With cytodiagnosis, however, it is difficult to differentiate amelanotic melanoma or scantily pigmented melanoma from other conditions. In the present case, monoclonal antibody HMB-45, the efficacy of which has been established in histologic studies, was used in the cytodiagnosis of amelanotic melanoma in the vagina. CASE: A woman, aged 78 years, presented with a brownish, nodular tumor, diameter 3 cm, in the vagina. Scraping smears with Papanicolaou staining showed nonepithelial malignant cells without granules suggesting melanin. Smears stained with HMB-45 showed positive immunoreactivity. The diagnosis underwent histologic confirmation of amelanotic melanoma on the initial biopsy. CONCLUSION: Cytodiagnosis was made with HMB-45, which proved very effective in the differential cytodiagnosis of amelanotic melanoma and scantily pigmented melanoma, particularly because it obviated the need for tissue invasion.

Human hemangiosarcomas have a common polymorphism but no mutations in the connexin37 gene

Gap junctional intercellular communication is often impaired in cancers, and the genes which encode the connexin gap junction proteins are considered to be tumor-suppressor genes. In this study, we analyzed the presence of mutations in the connexin 37 (Cx37) gene in 22 human hepatic angiosarcomas, 6 and 4 of which were associated with exposure to vinyl chloride and Thorotrast, respectively, The other 12 samples were from patients with no history of exposure to these 2 agents. In 9 samples, a proline (ACC) to serine (ACT) amino acid change in codon 319 was detected. However, DNA from non-tumorigenic tissue of the same patients also showed this amino acid change, suggesting that this is a polymorphism rather than a mutation. Subsequent analysis of 84 DNA samples from normal donors revealed the frequencies of Pro/Pro, Pro/Ser and Ser/Ser alleles to be 65.5%, 23.8% and 10.7%, respectively, while among the group of angiosarcoma patients the corresponding figures were 59.1%, 31.8% and 9.1%, respectively. Thus, there was no correlation between the polymorphism at codon 319 and hepatic angiosarcoma occurrence. However, among the 6 cases of vinyl chloride-associated angiosarcoma, the percentages of the polymorphic alleles were 33.3%, 66.7% and 0%, respectively. While the number of samples was too small to allow us to conclude that the Ser(319) allele in Cx37 predisposes to this rare type of human cancer, it may be noted that codon 319 is located at the cytoplasmic tail of Cx37, where most regulatory sequences reside, and that it could be a site of phosphorylation for some protein kinases, which may in turn affect the function of Cx37, including intercellular communication, Int. A new technique for radical hysterectomy with emphasis on preservation of bladder function

Bladder dysfunction always occurs, to varying degrees, following radical hysterectomy. In Japan, various techniques for preserving the pelvic autonomic nerves in a radical hysterectomy have been used to reduce the severity of bladder dysfunction. The authors describe a newly developed technique for preserving the pelvic autonomic nerves and present the postoperative urodynamic findings in cases in which this technique was used. Preservation techniques can be broadly classified into three types: preservation of the pelvic parasympathetic nerves, preservation of the pelvic sympathetic nerves, and preservation of the nerves to the bladder. In this new technique, as many of the pelvic parasympathetic nerves as possible are preserved when the cardinal ligament is cut. Then, the cardinal ligament is inserted into and pulled through the anterocranially located connective tissue in front of the pelvic plexus, which lies below the ureter. Finally, as many as possible of the vesical branching nerves are preserved when the posterior layer of the vesicouterine ligament is cut. Between July 1996 and February 1999, 23 women were treated for Stage IB1-IIA cervical cancer, and two women were treated for Stage IIB endometrial cancer. The results of urodynamic studies performed ∼1 month after surgery were all within normal ranges: mean duration of bladder drainage, 17.1 ± 6.4 days mean maximal flow rate, 19.5 ± 12.9 ml/sec mean postvoiding residual urine volume, 30.5 ± 25.8 ml and mean maximal desire to void, 311.4 ± 124.0 ml. None of the patients required intermittent self-catheterization. It is concluded that this new technique for preservation of the pelvic autonomic nerves is effective in reducing the severity of bladder dysfunction after a radical hysterectomy and would therefore improve the patient's quality of life.

We have previously shown that the connexin (Cx) 26 and 32 genes are expressed during the secretory phase of the human endometrium and that their expression is downregulated during the proliferative phase, suggesting a role for intercellular transduction in cell growth control in human endometrium. To further study the possible role of cell-to-cell interaction in growth regulation, we immunohistochemically analyzed 80 endometrial samples (30 of normal endometrium, 20 of endometrial hyperplasia, and 30 of endometrial cancer) for the expression of E-cadherin; alpha-, beta-, and gamma-catenin; adenomatous polyposis coli (APC) protein, and sex-steroid hormone receptors at three points in the cells: the cell-to-cell border, the cytoplasm, and the nucleus. In this study, moderate or strong staining of beta-catenin in the nuclei was observed in 60.0% of endometrial hyperplasia samples and 30.0% of endometrial cancer samples, although the beta-catenin gene was mutated in only two of the nine samples that showed the intensive nuclear staining. Western blotting analysis showed that the samples that had intense nuclear staining of beta-catenin had much higher expression of beta-catenin than the samples that did not have nuclear staining. Furthermore, normal endometrium showed nuclear localization, especially in the mid- and late-proliferative and early-secreting phases of the menstrual cycle. The results suggest that the nuclear localization of beta-catenin observed in endometrial hyperplasia and endometrial cancer, as in other tumors, implies that beta-catenin/wnt-1 signal transduction is highly activated in carcinogenesis of the endometrium as well as in normal physiological conditions. The 16 K subunit of the vacuolar H+-ATPase (ductin) has been suggested to also play a role in gap junction channels. Since mutated 16 K subunits have transforming ability when transfected into NIH3T3 cells and since aberrant gap junctional intercellular communication (GJIC) is a hallmark of cancer cells, we hypothesized that mutated 16 K subunits might transform these cells via alteration of GJIC, When GJIC was measured by the dye-transfer assay, NIH3T3 cells transfected with the mutant 16 K protein genes (deletion of the fourth transmembrane domain or a point mutation at codon 143 from glutamic acid to arginine) showed significantly lower levels of GJIC than those transfected with the vector alone or with the wild-type 16 K subunit gene. GJIC levels of NIH3T3 cells transformed by v-ras and v-src were not significantly decreased, suggesting that low GJIC levels are not necessarily the result of cell transformation pel se. NIH3T3 cells express Cx 43 as a major connexin gene. Although cells transfected with mutated 16 K subunits showed a level of Cx 43 protein expression similar to non-transfectants, their Cx43 protein was localized aberrantly, i.e. intracytoplasmically, These results indicate that mutant 16 K subunits with transforming ability translocate Cx43 proteins, thus inhibiting GJIC of NIH3T3 cells.

Electron microscopic examination of cytologic samples

OBJECTIVE: To review the results of observations of cytologic samples performed in our laboratory by light microscopy (LM), scanning electron microscopy (SEM) and transmission electron microscopy (TEM) performed in succession (LM-SEA I-TEM examination) using the same cytologic sample and to assess the diagnostic value of this method of successive examination. STUDY DESIGN: Using a previously reported method of LM-SEM-TEM sample preparation and observation, we analyzed 201 cytologic specimens over a seven-year period (1986-1993) and investigated whether the histologic origin and malignancy can be estimated from SEM and TEM findings on the cells. RESULTS: Observations of many cytologic samples over a seven-year period (by LM, SEM and TEM) showed that several basic interpretations of cellular ultrastructure are possible. In cases where cell identification was difficult by LM, electron microscopic findings were sometimes useful for determining the biologic characteristics of cells and for estimating their tissue origin. Electron microscopic findings also provided important information for cytodiagnosis. CONCLUSION: SEM and/or TEM findings are useful for determining the morphologic (including biologic) characteristics of cells in cases where they cannot be determined by LM. With the accumulation of data on electron microscopic examination of cytologic samples, it is expected that in the future, electron microscopy will continue to provide new information that can be used to improve the accuracy of cytodiagnosis by LM.

A case of protein-losing gastroenteropathy in association with mixed connective tissue disease which was successfully treated with cyclophosphamide pulse therapy

A 47-year-old man had been given a diagnosis of mixed connective tissue disease (MCTD) on 1987 when he had presented with Raynaud's phenomenon, polyarthralgia, sclerodactyly, and a high titre of anti-RNP antibody. Once his symptoms had improved following the administration of prednisolone orally and the treatment was discontinued since 1995. He noticed dyspnea and chest pain in February 1997. The bilateral pleural effusion was pointed out in the local hospital and he was admitted to our hospital in March 1997 for further examination. In addition to pleural effusion and ascites, laboratory studies revealed hypoalbuminemia and low serum levels of complements. Renal and liver function tests were normal and the urine gave a trace test for protein. The presence of protein loss in the gut was confirmed by an elevated 1/4 - antitrypsin clearance and 99mTc-albumin scintigraphy showing abnormal radioactivity in the gastrointestinal tract. Although endoscopic examination showed no abnormal findings macroscopically and gastrointestinal biopsies revealed nonspecific inflammation only, immunofluorescent studies demonstrated deposits of C3, C4 and IgG in the stomach, colon, and pleura. These findings supported the pathogenesis that immune deposits in tissues caused protein-losing gastroenteropathy (PLGE) in MCTD. Intravenous administration of cyclophosphamide started since July 1997, while the high- dose corticosteroid therapy including methylprednisolone pulse therapy were not effective. Hypoalbuminemia and low serum levels of complements improved remarkably and the pleural effusion and ascites disappeared after cyclophosphamide pulse therapy four times monthly. Cyclophosphamide pulse therapy should be considered as a possibly effective treatment for PLGE in association with collagen disease resistant to corticosteroid therapy.

Co-ordinated expression of connexins 26 and 32 in human endometrial glandular epithelium during the reproductive cycle and the influence of hormone replacement therapy

Hormones are involved in the regulation of intercellular communication, and gap junction intercellular communication may play an important role in the prevention of endometrial cancer. We have investigated changes in the expression of the gap junction proteins connexin 26 (Cx26) and Cx32 in human endometrial glandular epithelium during the reproductive cycle as well as the influence of hormone replacement therapy. Frozen sections from 71 endometrial tissue samples (53 taken from women who had undergone hysterectomy during the menstrual cycle, 3 early pregnancy deciduae and 15 from menopausal women, some of whom were receiving estrogen alone or estrogen plus progesterone) were analyzed by immunofluorescence and confocal laser scanning microscopy. Cx26 and Cx32 were expressed weakly in the proliferative phase, markedly during ovulation and most strongly in the mid-secretory phase; by the late secretory phase, they decreased drastically. Cx26 and Cx32 also were expressed in early pregnancy. Women who had received estrogen and progesterone expressed the Cxs, but those who had received estrogen only or no therapy did not. These results were confirmed by Western blot analysis. They indicate that expression of Cx26 and Cx32 is correlated with cell differentiation and with the glandular function of the endometrial epithelium and suggest that expression of Cxs is controlled by serum progesterone. Proliferation-associated regulation of telomerase activity in human endometrium and its potential implication in early cancer diagnosis

Telomerase activity was detected in normal endometrium in association with proliferation and regulated during the menstrual cycle in a hormone-dependent manner. The activity was maximal at the late-proliferative phase to mid-secreting phase, and was absent or extremely low at early-proliferative phase and late-secreting phase. Activity was also detected in all endometrial simple hyperplasias tested (16 of 16) and in most cancers (25 of 30), but none was detected in endometrial of either pregnant or postmenopausal women ill the absence of hyperplasia. Our data provide evidence that the telomerase activity in postmenopausal endometrium reflects a hyperproliferative condition. Therefore, we conclude that telomerase can provide a novel marker for early endometrial cancer diagnosis. Hormone-dependent regulation of telomerase suggests the possibility of therapeutic and preventive strategies for endometrial cancers through the management of ovarian steroid hormones or other agents that regulate telomerase activity. Connexin 37 mutations in rat hepatic angiosarcomas induced by vinyl chloride

Connexin genes have been shown to restore normal cell growth when transfected into certain tumorigenic cells and thus are considered to form a family of tumor suppressor genes, In this study, we have analysed mutations of the connexin 37 (Cx37) gene in rat hepatic angiosarcomas induced by vinyl chloride. A total of 25 rat liver tumors (22 hepatic angiosarcomas and 3 hepatocellular carcinomas) were analysed by PCR-single-strand conformation polymorphism analysis and DNA sequencing, Four mutations were detected in three tumors: (a) one GGG(Gly) to GAG(Glu) mutation at codon 168; and (b) three silent mutations, CGA(Arg) to CGC(Arg), at codon 166, In addition, we found that codon 88 is polymorphic (GAG(Glu) to GAA(Glu)). Cx37 proteins are detectable in endothelial cells of normal liver by immunohistochemical analysis, but none of the angiosarcomas showed Cx37-positive spots, These results suggest that Cx37-mediated gap junctional intercellular communication may be disturbed in most of these angiosarcomas, but mutation of the Cx37 gene is rare.

Expression of component desmosomal proteins in uterine endometrial carcinoma and their relation to cellular differentiation

BACKGROUND. While the assessment of the malignancy of neoplasms is based on morphologic studies of cells and tissues, use of objective molecular markers is leading to a better understanding and more biologically meaningful classification of neoplasms. In recent years, changes in the expression of cell adhesion molecules, especially E-cadherin, catenin, and adenomatous polyposis coli (APC), in carcinomas have attracted the attention of researchers. However, little is known about desmosomes in the uterine endometrium or in endometrial carcinomas. In this study, we semiquantified the desmosomal components desmoplakin I and II and desmoglein, in tissue sections using confocal laser scanning microscopy (LSM), and examined their relationship to the pathological type, the occurrence of lymph node metastases, and the extent of myometrial invasion. METHOD, Frozen sections of 31 specimens of normal endometrium, 5 specimens of atypical hyperplasia, and 41 specimens of endometrial carcinoma were stained by the immunofluorescence method using antidesmoplakin I and II and antidesmoglein, and these markers were then semiquantified in tissue sections by LSM. RESULTS, The expression and location of desmoplakin I and II and desmoglein were similar, and their expression decreased with loss of differentiation. The expression was lower in cases of lymph node metastasis than in negative cases and was lower in the cases with > one-half myometrial invasion than in cases with < one-half myometrial invasion. CONCLUSIONS. Reduction of desmoplakin I and II and desmoglein expression may play an important role in the invasiveness and metastatic activity of human endometrial carcinoma. They can therefore be used as differentiation markers for endometrial carcinoma. (C) 1996 American Cancer Society.

MONOCLONAL-ANTIBODY 7H6 REACTS WITH A NOVEL TIGHT JUNCTION-ASSOCIATED PROTEIN DISTINCT FROM ZO-1, CINGULIN AND ZO-2

The tight junction is an essential element of the intercellular junctional complex; yet its protein composition is not fully understood. At present, only three proteins, ZO-1 (Stevenson, B. R., J. D. Siliciano, M. S. Mooseker, and D. A. Goodenough. 1986. J. Cell Biol. 103:755-766), cingulin (Citi, S., H. Sabanay, R. Jakes, B. Geiger, and J. Kendrick-Jones. 1988. Nature (Lond.). 333:272-275) and ZO-2 (Gumbiner, B., T. Lowenkopf, and D. Apatira. 1991. Proc. Natl. Acad. Sci. USA. 88:3460-3464) are known to be associated with the tight junction. We have generated a monoclonal antibody (7H6) against a bile canaliculus-rich membrane fraction prepared from rat liver. This 7H6 antigen was preferentially localized by immunofluorescence at the junctional complex regions of hepatocytes and other epithelia, and 7H6-affiliated gold particles were shown electron microscopically to localize at the periphery of tight junctions. Immunoblot analysis of a bile canaliculus-rich fraction of rat liver using 7H6, anti-ZO-1 antibody (R26.4C), and anticingulin antibody revealed that 7H6 reacted selectively with a 155-kD protein, whereas R26.4C reacted only with a 225-kD protein. Anti-cingulin antibody reacted solely with 140 and 108-kD proteins, indicating that the protein recognized by 7H6 is immunologically different from ZO-1 and cingulin. Immunoprecipitation of detergent extracts obtained from metabolically labeled MDCK cells with R26.4C coprecipitated a 160-kD protein, which corresponds to ZO-2, with ZO-1. However, 7H6 did not react with the 160-kD protein. These results strongly suggest that the 7H6 antibody recognizes a novel tight junction-associated protein different from ZO-1, cingulin and ZO-2.
