本学部で開講している3学科合同科目のうち、保健医療総論Ⅱの学習状況を把握するため学生を対象に質問紙調査を行った。対象は平成17・18・19年度履修学生288名で、調査票の回収数は260部(90.3％)であった。調査内容は教員のオリエンテーション、実習、報告会準備、報告会、保健医療総論Ⅱ全体について尋ねる質問から構成した。教員のオリエンテーション、実習、全体に関する各質問への回答には3学科間の相異はほとんどなく、「良い」「やや良い」と肯定的な回答をした学生の割合が高かった。報告会への参加について尋ねた質問では「良い」「やや良い」「普通」と回答した割合が高かった。以上より保健医療総論Ⅱに対する学生の評価は概ね良好であった。ただし少数ではあるが、実習指導者との連絡や報告会の運営方法について改善を望む意見が出された。今後は実習施設との連携、教員のオリエンテーション、報告会の運営について検討する必要がある。

Analysis of gender differences in correlation of brachia-lankle pulse wave velocity with body mass index and blood pressure in healthy youths

生活習慣病を予防するための基礎資料を得ることを目的に、心血管系の既往歴のない青年男子34名（22.6±3.4歳）と女子46名（20.0±2.1歳）を対象にBMI、baPWV、血圧を測定した。そしてbaPWVとBMI、およびbaPWVと血圧値との相関における性差を検討した。その結果、１）BMIの平均値±標準偏差は男子群22.2±2.6、女子群20.7±2.4（p<.01）、血圧値は男子群、女子群の順に収縮期血圧値は113.6±9.8㎜Hg、101.8±6.3㎜Hg（p<.01）、拡張期血圧値は61.1±8.7㎜Hg、56.0±6.1㎜Hg（p<.01）であった。baPWVは男子群1116.7±148.5cm/s、女子群989.0±101.9cm/s（p<.01）といずれも男子群が有意に高い値であった。２）baPWVとBMIとの相関係数は男子群r=0.511（p<.01）、女子群r=0.036、baPWVと収縮期血圧値との相関係数は男子群r=0.698（p<.01）、女子群r=0.242といずれも男子群にのみ有意な相関が認められた。３）baPWVと拡張期血圧値との相関係数は男子群r=0.762（p<.01）、女子群r=0.452（p<.01）と両群とも有意な相関を示した。以上より、今回の対象である青年におけるbaPWVの値、およびbaPWVと収縮期血圧値との相関には性差があり、女子の場合は血管の柔軟性が高いことが示唆された。The present study examined brachial-ankle pulse wave velocity［baPWV］, body mass index［BMI］and blood pressure as well as their intercorrelations in healthy young males （N=34） and females （N=46）. BMI was 22.2±2.6 in males and 20.7±2.4 in females （p<.01）. BaPWV was 1116.7±148.5 cm/s in males and 989.0±101.9 cm/s in females（p<.01）. The significant correlation coefficient between baPWV and BMI was 0.511（p<.01）in males, and between baPWV and systolic blood pressure it was 0.698 （p<.01）in males. In conclusion, there appear to be gender differences in baPWV, and the elasticity of arteries in females was higher than in males.

看護者に必要とされる転倒事故予見能力の基盤を卒業時までに育成するための基礎資料を得ることを目的とし、転倒事故に関する学生のアセスメントの実態を調査した。同意の得られた４年次看護学生18名に易転倒状態にある紙上事例に対するアセスメントを行ってもらった。学生の記述を、ゴードンの「機能面から見た健康パターン」を用いて事故防止の観点から内容の分析を行った。対象学生のアセスメントの特徴として、事例に表されている身体機能面には目を向けやすいが、身体機能以外の療養生活のあり方や人間の行動特性には着眼しにくい傾向があった。さらに、身体機能面と療養生活のあり方や人間の行動特性を関連付けた転倒事故予測が不十分であることが明らかになった。また、環境要因は約半数の学生が着眼していたが、看護体制など看護者側の要因に目を向けた学生はいなかった。今後対象数を増やし、学生のアセスメントの実態をより明確にし、療養者の生活全体を見通した転倒事故予見能力の育成に向けた教育方法を検討する必要があると考えられた。The purpose of this study was to investigate the risk assessment ability of students. This was done by asking them to provide a written assessment of the risk of such accidents so as to evaluate their nursing ability at the time of graduation. Eighteen fourth-year nursing students agreed to participate in the study. We analyzed the assessments of the students from the point of view of accident prevention considering Gordon's Functional Health Pattern.The results showed the following characteristics. They found it easy to pay attention to the body function pattern, but they did not pay attention to the conditions of life and the characteristic patterns of human actions. It also became clear that the assessments did not sufficiently consider the relationships among these factors. Though about half of the students paid attention to the environmental conditions that could lead to a fall, most emphasized the physical condition of the person being cared for and none considered the nursing system and its relation to the patient. We need to increase the number of targets in the future and improve the assessment abilities of students. To do so it is necessary to examine educational methods to improve systematic risk assessment ability.

本報告は、平成15年度、札幌医科大学保健医療学部看護学科と札幌医科大学医学部附属病院看護部が、看護学生の臨地実習における同意書のあり方に関して検討を行った経過を報告するものである。検討の経過において、同意書の導入には、責任体制や事故時の対応といった実習の指導体制の確立、看護技術項目と水準の明確化が必要であることが確認された。同意書のあり方に関しての検討は、書面を用いるか否かだけの検討ではなく、安全な実習を保障することの大切さを確認する機会となった。実習施設や対象者の特性に応じた同意のあり方に関して検討することは不可欠と思われ、実習施設と教育機関が共に検討しそのプロセスを共有することが重要である。1年に亘り検討を行った結果、同意に関するガイドラインとして「臨地実習における同意に関する取り扱い」を作成するに至り、平成16年度の実習において同意書を導入することとなった。今後、同意書を実際に導入した際の課題について検討を進めていく予定である。This report presented the process of discussion about the informed consent form of clinical practice in undergraduate nursing education. The discussion was held in the Committee of the Department of Nursing, Sapporo Medical University and Division of Nursing, University Hospital from April in 2003 to March in 2004. Though the process of discussion, it was confirmed that establishment of guidance and support for nursing students and delimitation of standard of nursing techniques were necessary in clinical practice. In other words, it was important not only to discuss how informed consent should be done but to discuss how the safety of clinical practice should be ensured. Therefore it is essential that educational facilities and clinical institutions discuss about informed consent suited for characteristic of institutions and subjects. As a result of the discussion, the guide for informed consent was redacted to introduce the informed consent form of clinical practice in September, 2004. In future, the actual problems on introduction of the informed consent form should be explored and reviewed.

Data were collected via questionnaire responses gathered from 43 nursing students of the first year and 52 nursing students of the second year. Morals and the level of moral development were analyzed from the perspective of the inclination of nursing students to use a soft-drink vending machine and the responses to three stories in which moral dilemmas were resented. These three stories were based on the DIT (Defining Issues Test) which has been developed for the Japanese adolescent. The results of the survey can be summarized as follows. 1. Nineteen of the first- year and 23 of the second- year nursing students expressed that they would buy a soft drink the right way. 2. Based on the second story, 30 of the first- year and 40 of the second- year nursing students were classified into stage 4 of moral development. For the first and third stories, the results were similar for the first year and the second year nursing students. The survey suggested that moral judgments could be changed by a mutual act with another person in nursing students in the first year and second year. In addition, it was shown that they had reached the stage of compliance with law and order in their moral development.

本研究の目的は、看護倫理の教育プログラム作成への示唆を得るため、初期の看護学実習における学生の倫理的体験の特徴を知ることである。対象は看護系４年制大学２年次学生50名であり、実習での倫理的体験について倫理原則に基づく６つの観点から記述してもらった。その結果、プライバシーや安全・安楽に関する記述が多く、授業で強調されてきた観点に関する倫理性は内面化しつつあること、対象学生の多くは問題状況を分析するレベルにとどまっており、倫理的体験の内容は学生の倫理的認識力により違いがあることが明らかになった。以上から、初学者における看護倫理教育では、看護の倫理的な側面を早期から強調すること、学生の倫理的認識力の実態を考慮した看護倫理教育を行う必要があることが示唆された。This study is an attempt to identify the ethical issues that have been encountered by nursing students in the early stage of their clinical experiences. The purpose of the study is to obtain actual issues in the clinical area to assist in developing an educational program of an early nursing curriculum content. The sample of 50 nursing students (sophomores in the nursing program of an university) were asked to participate in this study by describing their ethical experiences according to the six ethical principles for professional conduct during clinical practices, such as problems, difficulties and conflicts. The result of the study is that most of nursing students were aware of the principles of ethical implications in "privacy", "safety", and "comfort" which have been discussed in classrooms. Those nursing students were able to analyze ethical issues. However, their abilities in problem-solving and decision making dealing with ethical dilemmas were limited. In addition, as for the ability to recognize ethical issues, we found that there were divergences among individual nursing students. These results provide beneficial support for development of an educational program for nursing ethics in the early stage of nursing curriculum.

看護倫理教育の検討に関する基礎的資料を得るために、看護学生を対象に、道徳的推論および道徳的発達段階に関する質問紙集合調査を実施した。有効回答数は48名（98.0%）、調査対象の平均年齢は18.7±1.2歳で、以下の結果が得られた。１）道徳的推論のうち、社会的規範意識を高齢者と障害者を例にあげて援助を行うかどうかをたずねたところ、高齢者へは16名（33.3%）、障害者へは37名（77.0%）が積極的に援助を行うと回答した。２）DIT日本版のうち、３つの例話をもとに道徳的発達段階を検討したところ、各個人の道徳的発達段階は例話１では30名（62.5%）、例話２では33名（68.8%）、例話３では35名（72.9%）が第４段階であった。以上のことから、本調査対象の道徳的推論は社会的環境による影響を受け、また道徳的発達は社会秩序の維持に価値が置かれる段階へと発達を遂げていることが示唆された。The purpose of this survey was to gauge the level of moral reasoning and moral development of nursing students. Data was collected via questionnaire responses gathered from 48 nursing students. The level of moral reasoning was analyzed from the perspective of the inclination of nursing students to help elderly persons and persons with a disability. The level of moral development was examined through responses to three stories in which moral dilemmas were presented. These three stories are based on the DIT（Defining Issues Test）which has been developed for the Japanese adolescent. The results of the survey can be summarized as follows. １．Sixteen of the nursing students expressed an interest in helping elderly persons, while thirtyseven of the nursing students expressed an interest in helping persons with a disability. ２．Based on the first story, thirty of the nursing students exhibited a stage 4 level of moral development. For the second and third stories, thirty-three and thirty-five students respectively were classified into the stage 4 level of moral development. The survey revealed that the nursing students'level of moral reasoning is impacted by the social environment, and that the students have an adequate level of moral development.

看護倫理教育の検討に関する基礎的資料を得るために、看護学生を対象に、道徳的推論および道徳的発達段階に関する質問紙集合調査を実施した。有効回答数は48名（98.0%）、調査対象の平均年齢は18.7±1.2歳で、以下の結果が得られた。１）道徳的推論のうち、社会的規範意識を高齢者と障害者を例にあげて援助を行うかどうかをたずねたところ、高齢者へは16名（33.3%）、障害者へは37名（77.0%）が積極的に援助を行うと回答した。２）DIT日本版のうち、３つの例話をもとに道徳的発達段階を検討したところ、各個人の道徳的発達段階は例話１では30名（62.5%）、例話２では33名（68.8%）、例話３では35名（72.9%）が第４段階であった。以上のことから、本調査対象の道徳的推論は社会的環境による影響を受け、また道徳的発達は社会秩序の維持に価値が置かれる段階へと発達を遂げていることが示唆された。The purpose of this survey was to gauge the level of moral reasoning and moral development of nursing students. Data was collected via questionnaire responses gathered from 48 nursing students. The level of moral reasoning was analyzed from the perspective of the inclination of nursing students to help elderly persons and persons with a disability. The level of moral development was examined through responses to three stories in which moral dilemmas were presented. These three stories are based on the DIT（Defining Issues Test）which has been developed for the Japanese adolescent. The results of the survey can be summarized as follows. １．Sixteen of the nursing students expressed an interest in helping elderly persons, while thirtyseven of the nursing students expressed an interest in helping persons with a disability. ２．Based on the first story, thirty of the nursing students exhibited a stage 4 level of moral development. For the second and third stories, thirty-three and thirty-five students respectively were classified into the stage 4 level of moral development. The survey revealed that the nursing students'level of moral reasoning is impacted by the social environment, and that the students have an adequate level of moral development.

Topics coverd included concepts, guidelines for technical terms, and learning objectives for a course in "nursing process". The goals of teaching "nursing process" are the development of logic and critical thinking, and their practical application by nursing students. We defined "nursing process" as a systematic approach to providing care based on logic and critical thinking. The "nursing process" consists of six phases: assessment, identification of health-related tasks, the determination of objectives, planning, intervention and evaluation. It was clarified that we will need to compare this activity report with actual courses in "nursing process" going forward.

栄養に関する授業をもとに健康への関心が「自己」から「他者」へと広がりがあるのかを知るために、本学看護学科４年生52名を対象に質問紙集合調査を行い、有効回答数は46名（95.8％）であった。ここでは「他者」を家族の1名に限定し、以下の結果が得られた。１）自分自身のヘモグロビン、血清総コレステロール値、空腹時血糖、収縮期および拡張期血圧に「関心がある」と回答した学生は41?45名であった。２）家族の検査データへの関心の有無と健康状態との関連では、ヘモグロビン、血清総コレステロール値、空腹時血糖、収縮期および拡張期血圧いずれの項目にも関心があり、その家族の健康状態が「良好」は20名、「疾患もしくは症状あり」は25名であった。３）栄養に関する検査データの家族への活用状況では、「活用あり」が25名、「活用なし」が21名であった。以上のことから、調査対象である学生は自分自身および家族の栄養に関する検査データへの関心が高く、それらを家族への健康管理に活用していたことが示された。The purpose of this survey was to determine the achievement of learning objectives established for nutritional classes. Final year nursing students were expected to be concerned regarding health, starting with their concern for their own health and broadening this to a concern and care for the health of others. Data was collected by means of questionnaire responses from 48 students. The results can be summarized as follows. １．Approximately 90 percent of the students had an awareness of their own nutritional markers, hemoglobin, serum total cholesterol, fast blood sugar, and systolic and diastolic blood pressure. ２．The students were concerned regarding the nutritional markers, hemoglobin, serum total cholesterol, fast blood sugar, and systolic and diastolic blood pressure, of their family members. Roughly 40 percent of the family members were in good health and the remainder were not. ３．Twenty?five of the students provided support for their family embers regarding nutritional markers, while the remainder did not. The survey revealed that the final year students not only showed an interest in their own nutritional markers, but also by extending their knowledge to their family members they are able to extend their learning to their care for others.
