The purpose of this study was to analyze the actual situation of medical care for the elderly from a long-term perspective by utilizing medical big data covering the entire population and integrating clinical epidemiological methods. As studies utilizing medical big data of the entire population targeting the elderly, we have clarified diseases that could be targeted, and published clinical epidemiological studies on influenza, heptitis C, adverse reaction-related deaths, amyloidosis, and sarcoidosis in international journals, respectively.

In Japan, patients with Hansen‘s disease were forcibly detained, followed by lifetime isolation. To prevent the same mistakes, it is necessary to establish medical ethics for Hansen‘s disease. This project primarily constructed a curriculum on "Hansen‘s disease and Medical Ethics" (for medical students), and on “Hansen‘s disease and Human Dignity” (for elementary to university students), which initiated a series of on-site lectures. Subsequent qualitative analysis of survival narratives elucidated the ethical issues of lost sight due to clinical trials, the structure of spiritual pain from lifelong isolation, perceptual disorders, and their perception of the COVID-19 pandemic. Our published book, "The Beauty of Nature and Prayer in Sanatoriums," and the digital teaching materials, convey the wisdom gained through a life of hardship. Finally, the Society for the Ethics of Hansen‘s disease was established, and the journal “Hansen‘s disease and Human Dignity” was published.

The claim database was obtained from JMDC. Between January 2005 and July 2017, there were 5,767 patients diagnosed prostate cancer. The total number of hospitals was 17,001, of which 14,487 (85.2%) were clinics with 0-19 beds. The total number of medical prescriptions was 1,426,115. Among them, the drugs used for the treatment of prostate cancer, 4,288 cases of Leuplin, 4,220 cases of Casodex tablets (6,680 generic products), 3,155 cases of Zoradex, and 1,028 cases of Odyne tablets (424 generic products). The total number of laboratory tests was 5,225,046, with Creatinine 91,672, AST 85,736, ALT 85,523, and PSA 7,877. It was considered that this large-data could be used to estimate the risk of adverse events occurring in renal and liver functions.

I developed the system for searching the literature on recurrence of the non-muscle invasive bladder cancer performed in Japan. The database used MEDLINE (PubMed) and a Igaku Chuo Zasshi (Ichushi Web). I developed the software which saves the dounloaded file of PubMed and Ichusi Web with a standardized format. I downloaded the paper which contains "Superficial" in a paper title, and the paper containing "Non-muscle invasive" using this software. And I verified that this software operated normally. Moreover, I prepared the Cloud server for saving these files, and saved data. From now on, I release this technique and will enable use of the developed software.

We analyzed methylation status of tumor suppressor gene RASSF1A in 84 Wilms tumors and found out that it was significantly associated with a poor outcome. Moreover, we analyzed the mutation of WTX gene and revealed that this also was significantly associated with a poor outcome. Therefore these biomarkers were considered be candidates of the new biomarkers for risk classification to create the new therapeutic protocols. The survival rate of bilateral Wilms tumors (BWT) has been improved; however, the results are not satisfactory in terms of the renal preservation. We reviewed the outcome of BWT registered in JWiTS. Bilateral nephron-sparing surgery (NSS) was achieved only in 10 of 28 cases (36%) and 10 children (40%) developed impaired renal function. The protocol should be changed to improve the rate of preservation of the renal parenchyma. Preoperative chemotherapy should be performed to shrink the tumors, and subsequent NSS should be carried out after a central imaging evaluation.

We built the system that a scanned handwritten interview sheet make electronic data. And we built the clinical data collecting system using the secure server.

The results of the Japan Wilms Tumor Study group JWiTS-1 was that the OS of patients in the JWiTS-1 study were comparable with the results of other multicenter studies in the USA and Europe. The outcome for patients with nephroblastoma and CCSK was fair. In contrast, the cure rate for those with RTK was not satisfactory. In the patients with bilateral tumor, preservation of renal function was not satisfactory. New treatment strategies are needed for patients with RTK and bilateral Wilms tumor.

It is important to investigate the interferon-alpha (IFN-a) sensitivity-related gene and regulation of susceptibility to IFN-α in renal cell carcinoma (RCC). First, we have identified seven candidate genes, which are associated with IFN-a-response and then established prediction formula using four genes, I.e. ADFP, MITF, MTUS1, and TNNT1, in RCC cell lines. Validation of this prediction model was performed in other RCC cell lines, including primary culture cells from patients with RCC. We next focused MITF gene, which individually correlated with IFN-α-sensitivity, to analyze relationship between its expression and IFN-α-response status by gene transfection. MITF transfectant demonstrated susceptibility in IFN-α-resistant line as compared with wild type or mock transfectant. Because MITF is one off transcription factor, which can bind to p16 and HIF-1α, it is suggested that MITF may be associated with development and progression of RCC acceleration of cell cycle. Thus, we evaluated expression of p16 and anti-tumor effect of biological active p16 peptide (p16-MIS) by peptide transduction system with Wr-T transporter peptide. All RCC cell lines used are absent for p16 protein, even in the IFN-α-resistant RCC MITF-transfectant. P16-MIS successfully transferred into cellular cytoplasm and nucleus of RCC cells by concentration dependent manner. Interestingly, IFN-α-susceptibility was acquired at 2.5 to 100 times intensity in IFN-α-resistant RCC cell line. These results suggested that IFN-α-response related gene, MITF and its related cell cycle regulator, p16 could be molecular targets in therapeutic strategy of RCC.
