本研究では、①心疾患等の重篤な状態にある成人・高齢患者の全身（高次脳、循環・呼吸など）および摂食嚥下・栄養状態に応じた嚥下機能回復に関わる要因を把握するとともに、②これら要因を踏まえた効果的実践内容を明確にしながら、外科・内科別に早期リハビリテーションプログラムを開発し、介入効果に関する検証を行う。誤嚥や誤嚥性肺炎を含めた２次的弊害予防のための高度実践プログラムの構築ならびに看護ケア提供の系統的在り方を検討し、臨床への還元を図ることを目指す。本年度は心疾患等の重篤な状態にある成人・高齢患者の全身（高次脳、循環・呼吸等）および摂食嚥下の状態に応じた嚥下機能回復に関わる要因を把握することを主目的ととした調査のために、文献レビューを多職種で進め、高次脳、循環・呼吸等の生理学的指標や臨床所見の整理を行った。さらに、心疾患と摂食嚥下障害を専門としている有識者の意見をもとに項目を精選し、特に基礎疾患の治療等に伴って生じる治療・ケア、および摂食嚥下の関連所見などについても検討した。文献レビューの結果、調査に参考となる国内外の論文は散見される程度で、新たな知見を得ることに鑑みた調査項目を設定し、必要症例数を得るために、多機関共同研究として倫理審査委員会からの承認を得た。さらに各研究機関よりデータを得ている段階で、データの精選を行っている。

今年度は本共同研究の初年度にあたり、共同研究相手国・研究機関への連絡と調整、研究に関する打ち合わせ、研究に必要な消耗品の供与、実際の解析に関する指導を行った。ミャンマーにおいては2ヵ所と連絡、調整を取り、ヤンゴン小児病院では小児由来の全検体、ピンロン病院では血液検体に由来する黄色ブドウ球菌株を分離、保管することとし、その作業が2024年1月から開始されている。バングラデシュにおいても2ヵ所の共同研究先と連絡を取り、マイメンシン医科大学病院で全検体、首都ダッカのBIHS総合病院では血液検体に由来する黄色ブドウ球菌株を分離、保管することとした。菌株の収集は2024年3月から開始されている。バングラデシュでは今回の研究以前に共同研究先のマイメンシン医科大学病院で2年間にわたり収集されていた菌株があり、バングラデシュへの訪問に合わせてそれらの解析も行った。それらは170株の黄色ブドウ球菌であったが、MRSAは36％でありその8割がSCCmecIVを保有していた。それらの解析において特徴的であったのは、高病原性に関連するPVL遺伝子の陽性率の高さであり、MSSA（62％）のほうがMRSA（26％）より高率であった。MRSAの中に米国で優勢な市中感染型MRSAであるUSA300クローンが1株検出され、またリネゾリド耐性を付与するcfr遺伝子がMSSA1株で検出された。S. argenteusが2株同定（ST2250）され、これはバングラデシュで最初の報告となった。昨年10月に研究費の使用が可能となって以降、本研究の実施体制は今年度内に整えられ、予定通り進められている。

本研究の目的は，人工知能（AI）を使って高齢者の口腔内をOral Assessment Guide(OAG)の項目の口唇，舌，唾液，歯肉，歯と義歯の口腔内画像を用い，OAGのスコアを診断する畳み込みニューラルネットワーク（Convolutional Neural Network: CNNまたはConvNet）を構築することである．本研究では，高齢者の口腔内の写真を収集し、収集した画像を用いてCNNモデルを構築し，モデルの適合性を確認することであった． １．データベースについては，すでにあるデータベースの基本属性等の項目の点検し改修した．２．歯科クリニックにある既存の画像を収集し、第１次モデルCNNを作成した．３．第１次モデルCNNを使用したアプリ開発を試みた．４．第１次モデルCNNは、歯科に通院できる高齢者の画像に限定されたため、画像の収集に偏りが出たため、病院や施設にいる人の画像が必要であった．次に病院や施設の画像を取集開始した．COVID-19流行下にあって依頼した施設も業務多忙であったため、画像の収集がなかなかできなかった．収集された画像については、第2次モデルCNN用に分類中である．５．特許を出願中である．６．2022年度大学見本市～イノベーション・ジャパンに参加し、第１次モデルCNNを使って臨床活用に結びつく企業探しをしたが、見つからなかった．

Propylhydroxyproline (Pro-Hyp) increased cell proliferation in human oral fibroblasts (HOrF) at a concentration of 200 nmol/mL, which was considered the optimal concentration, as in previous studies. In addition, it was revealed that high-concentration Pro-Hyp inhibited the proliferation of HOrF. It was found that serum components do not significantly affect cell proliferation ability. Radiation alone was investigated because toxicity was high in mice with oral mucositis induced by chemoradiotherapy, making it difficult to continue the study. Administration of Pro-Hyp exacerbated weight loss and tended to exacerbate oral mucositis. Administration of Pro-Hyp at the onset of oral mucositis may promote aggravation of oral mucositis.

When creating a program for critically ill patients (patients with cardiovascular diseases) according to their feeding and swallowing functions, the number of valid English-language articles on the survey items is still insufficient. Based on the specific discussions among the experts on the content of this project, it was confirmed that it is important to conduct a pre-survey in the first phase, as well as to identify missing survey items and select necessary items from a clinical perspective, in order to create a clinical practice program of higher quality. It was considered necessary to obtain information from experts in the cardiovascular system (surgery and internal medicine) and advanced practitioners providing advanced medical care. Based on visits to the USA, we gained a qualitative understanding of the factors that are necessary for a medical system of patients with cardiovascular diseases.
