    本研究の目的は,せん妄リスクのある患者へのケアの質を評価する指標を開発するために質問項目を精選することである.先行研究から作成したせん妄ケアの質評価指標55項目について,臨床経験1年以上の看護師291名を対象にWeb調査を実施した.123名の協力が得られ,対象基準に合致した119票を分析対象とした.対象者の平均臨床経験年数は13.0±8.3年であった.天井効果,床効果,I-T相関分析の結果をもとに探索的因子分析(主因子法,プロマックス回転)を試行した結果,「ストレスなく生活するためのケア」「安心感をもたらすコミュニケーション」「排泄や活動のニーズを満たすケア」「呼吸・循環・水分・栄養ケア」「せん妄患者の世界を理解した関わり」「感覚障害・見当識へのケア」「覚醒を促すケア」など先行研究と類似した11因子46項目が抽出された.(著者抄録)

    本研究は、在宅において認知症介護または身体介護をしている家族介護者におけるインフォーマルな支援の必要性を概観し、介護の種類の違いによる家族介護者が求めるインフォーマルな支援の特性を検討することを目的とした。対象者は、北日本の農村地域であるA町の全世帯を対象として実施されたアンケート調査結果より抽出した、認知症をもつ者の介護者(認知症介護)23名と身体障がいをもつ者の介護者(身体介護)27名の計50名とした。調査内容は、介護負担度、介護協力者の状況、地域住民との助け合いの状況、地域住民に求める支援であった。介護負担度は全体的に軽度であったが、認知症介護が身体介護よりも有意に高かった。介護協力者は両介護とも家族による協力が多く、地域住民による支援は少なかった。両介護者とも約9割が地域住民とのつながりや助け合いは必要であるが、約8割が手伝ってほしいことはないと回答した。また、地域住民に求める具体的な支援内容は、認知症介護では「見守り」と「被介護者の話し相手」、身体介護では「車での送迎」であった。介護者は地域住民からのインフォーマルな支援に対する遠慮や羞恥心があることが推測された。本研究で明らかになった認知症介護と身体介護それぞれの介護者が求める具体的な支援内容について、地域住民側から介護者に対して積極的に発信してく仕組み作りが必要である。(著者抄録)

  • Nursing Assessment and Care Processes to Avoid Employment of Physical Restraints at the Onset of Delirium in Elderly Patients

    Purpose: This study describes assessment and care processes of nurses used to avoid physical restraint at the onset of delirium in elderly patients. Methods: Semi-structured interviews were conducted with eight nurses working in general wards and the interview data were qualitatively and descriptively analyzed. Results: When the nurses ‘sensed unusual changes in symptoms and behaviors in patients’, they were ‘accepting the behaviors of patients in a manner to ease the mind of the patient, and trying to calm the patients’, and ‘observing the patients while being ready for a further immediate response’ to avoid employing physical restraints with elderly patients. The nurses provided care to ‘make the patient conditions closer to those of ordinary life by reducing the discomfort and distress’ based on the ‘interpretation that the changes in the symptoms and behaviors are caused by physical and mental distress’, while repeatedly ‘estimating the risks arising from the symptoms and dangerous behaviors’ and ‘focusing on the patient background and factors affecting the onset of delirium, to analyze the reasons for changes in the symptoms and behaviors’. Conclusions: The findings suggest that nursing care practices to make patients comfortable from the patient view point without bias from medical safety concerns may be linked to maintaining avoidance of physical restraints.

  • せん妄リスクのある患者への看護実践の知 一般病院におけるエスノグラフィ研究

    本研究は、エスノグラフィの手法を用いて、看護師が日常行っているせん妄リスクのある患者への看護実践を記述し、そこからせん妄ケアの核となる文化的テーマを特定することを目的とした。研究参加者である一般病院2施設の看護師9人を対象として、せん妄リスクのある患者への看護実践場面を参加観察し、その後、半構造化インタビューを行った。参加観察のフィールドノートおよびインタビューの逐語録をSpradleyの分析手法に則り分析した。分析の結果、せん妄リスクのある患者への看護実践として7つの要素が特定され、構造化された。せん妄ケアの看護実践を統合する文化的テーマは「患者にとってストレスになるものを予測し、安楽にすごせるようにする」であった。本研究で特定されたせん妄ケアの構造と文化的テーマから、患者を個別性のある全人的な存在としてとらえ、入院治療環境における患者のストレスや基本的ニードをアセスメントし、患者との相互作用を基盤にしながら安楽の増進に向けて援助することが、せん妄ケアの本質として重要である可能性が示唆された。(著者抄録)

    わが国の認知症施策では認知症の早期診断・早期対応が推奨されているが、認知症の診断を受けてから介護サービス等を利用するまでの診断後支援が未整備である。認知症の人と家族は、診断後速やかに認知症の状態に応じた専門機関の支援につながるシステムの構築を政府に要望している。そのなかで外来看護師は診断直後から継続的に本人と家族に支援できる立場にあり、診断後支援の役割を担うことが期待される。 そこで本研究の目的は、①認知症の人と家族が認知症の診断を受けてから介護サービス等を利用するまでの期間に求める診断後支援を明らかにすること。②認知症専門外来に勤務する看護師による診断後支援モデルを作成してその実用性を検討することである。 令和3年度は研究目的①調査、認知症の人の介護家族8名を対象に実施したインタビューデータについて、認知症診断後から介護サービス等を安定的に利用できるまでの経過を複線径路・等至性モデル（TEM）を用いて分析した。その結果、安定的に介護サービスを利用する等至点として「認知症の人と家族が信頼できるサービスを安心して活用できる状態」が考えられた。そこに至る過程には、病院の主治医との信頼関係、各種相談窓口の対応、介護サービス専門職との出会いなどが分岐点となっていた。多くの対象者は認知症の診断を受けた直後には専門職から的確な支援を得られず、独力で手探りのまま情報収集をしていた。そのなかで家族の思いに寄り添い一緒に対応を考えてくれる支援者に信頼感を抱いていた。一方で、認知症外来受診の際に看護師から支援を受けたと感じている対象者は少なく、必ず外来にいる看護師が対象者に積極的に声をかけることで早く支援につながることができた可能性について言及していた。 そこで令和4年度は、認知症外来の看護師を対象として看護師の視点からの認知症診断後支援の現状と課題について調査していくこととする。

    In the present study, an educational program for promoting multidisciplinary care and improving the care skills of nurses in delirium risk management at acute hospitals was developed. First, the activities of pioneering delirium management teams in Japan were investigated, and the process of establishment of multidisciplinary teams was elucidated. Then, a DVD teaching material for nurses aimed at facilitating the introduction of delirium assessment tools in clinical settings was created. Finally, a delirium risk management program comprising the three pillars of establishment of organizational structures for promoting delirium management, establishment of systems for delirium management, and staff education regarding delirium care was created. It is necessary in the future to apply the present program to clinical settings, and to assess its usefulness and effectiveness.

  • Challenges in Nursing Care of the Elderly with Dementia at Advanced Treatment Hospitals

    Interviews were conducted with 24 nurses working at advanced treatment hospitals in Japan. A qualitative analysis of their narratives identified seven areas they found challenging in the care of elderly patients who also had dementia, including “accident prevention”, “witnessing life functioning impairment when receiving best possible treatment for disease” and “discharge arrangements”. Eight categories were extracted as supportive initiatives taken by nurses, such as “using a restraint or alternative means to prevent accidents” and “liaising with other nursing team members/other disciplines“. Accident prevention is the top priority in the advanced clinical setting and elderly patients often experience deterioration of their functioning even though their disease has been cured. When the elderly with dementia are admitted to advanced treatment hospitals, multi-disciplinary collaboration is essential from an early stage of admission to prevent their functional impairment.
