Effect of ankle flexion on the quantification of MRS for intramyocellular lipids of the tibialis anterior and the medial gastrocnemius

Muscle proton magnetic resonance spectroscopy (MRS) has been developed for non-invasive measurement of intramyocellular lipid (IMCL) levels. The majority of previous studies measuring IMCL with MRS have been performed on the calf muscle. The appearance of muscle MRS is influenced by bulk magnetic susceptibility and residual dipolar couplings, which depend on the angle between the muscle fibers and the main magnetic field. Our objective in this study was to evaluate the effect of ankle flexion and of the pennation angle on IMCL quantification in the calf muscle using proton MRS. The subjects comprised ten healthy male volunteers. In proton MRS, the ankle flexion angle was changed, and the pennation angle was measured from the tibialis anterior (TA) and the medial gastrocnemius (MG), respectively. We considered the relationship between the quantification of IMCL with 1H MRS and the pennation angle by ankle flexion angle. The pennation angle of the TA and MG changed with the ankle flexion angle. The IMCL on the TA decreased significantly with plantar flexion (p < 0.05). However, the IMCL on the MG demonstrated no significant difference. The MR spectrum and IMCL quantitation changed with the pennation angle. Therefore, when spectra of individual subjects in longitudinal studies or between subjects are compared in cross-sectional studies, the foot position or calf muscle orientation must be considered.

筋収縮不全を認めた膝関節術後患者7名(男2、女5、平均20.4歳)および健常成人7名(男5、女2、平均24.0歳)を対象に、超音波剪断波エラストグラフィを用いて大腿四頭筋スティフネスを測定した。膝関節術後患者における安静時の筋の弾性率は患側と健側で差は認められなかったが、大腿四頭筋セッティング(膝窩にタオルを置き、臀部および踵部が浮き上がらないようにタオルをつぶしながら大腿四頭筋の等尺性収縮を行う)時は患側の弾性率が健側より有意に小さかった。一方、筋厚や羽状角に健患差は認められず、弾性率の変化は筋形状とは関連の低い独立した指標である可能性が示唆された。超音波剪断波エラストグラフィは運動制約の強い術後早期においても、簡便かつ直接的に筋機能を捉えられる定量的な評価手段となり得ると考えた。

Acute decrease in the stiffness of resting muscle belly due to static stretching

The purpose of the study was to examine the acute effect of static stretching exercise on the resting stiffness of gastrocnemius muscle belly. Ten healthy young adults performed standing wall stretching in dorsiflexion for 1min at a time and repeated five times. Before and after stretching, the shear modulus was measured in medial and lateral heads of the resting gastrocnemius muscle with ultrasound shear-wave elastography. After the stretching, dorsiflexion range of motion (ROM) of the ankle joint increased (P<0.01) by 3.9 degrees and returned in 20min. Immediately after stretching, shear modulus decreased (P<0.01) by 14%, compared with before stretching across muscle heads. The decrease in shear modulus returned in 20min after stretching. In the comparison group of 10 additional subjects, the standing intervention without stretching had no influence on these measures. There was a negative correlation between dorsiflexion ROM and shear modulus in either head before and after stretching. The results demonstrate the transient decreases in the stiffness of the resting gastrocnemius muscle belly and indicate that joint flexibility is greater in individuals with lower resting stiffness of the muscle belly.

スポーツ選手の腰痛の多くは、重篤な基礎疾患を伴わない非特異的腰痛である。また、成長期のスポーツ選手では、腰椎分離症も腰痛の原因となる。非特異的腰痛では急性期の腰痛の軽減を待ち、早期からリハビリテーションを開始する。腰椎分離症では、骨癒合の可能性がある場合は、コルセットの装着とスポーツ活動の中止により、骨癒合を目指す。骨癒合後は再骨折を予防するため、十分な柔軟性を獲得させてからスポーツへ復帰させる。骨癒合の可能性がない症例では、疼痛コントロールを行いながら、スポーツ復帰を目指す。実際のリハビリテーションでは、脊柱-骨盤-股関節を1つの複合体として捉え、この複合体の可動性と安定性を高めることを主眼にエクササイズを行う。常に痛みをコントロールしながら行い、症状が増悪しないようトレーニング負荷を調整する。(著者抄録)

Three-dimensional analysis of the tibiotalar joint of intact male feet under unloaded and axial-loaded conditions

Purpose: We investigated the distribution of three-axis rotational directions of the tibiotalar joint in intact feet under axial loading and categorized them according to the combinations of the three-axis rotational directions of the tibiotalar joint as three-dimensional (3D) rotational patterns. Methods: The differences in rotational orientation of the talus relative to the tibia under unloaded and axial-loaded conditions were calculated from the 3D models of the tibiotalar joint that were reconstructed from magnetic resonance (MR) images in 27 intact male feet. Results: In the sagittal plane, the talus rotated into plantarflexion in 63% of the feet and into dorsiflexion in 30%. In the coronal plane, the talus rotated into inversion in 37% of the feet and into eversion in 33%. In the transverse plane, the talus rotated into adduction in 85% of the feet and into abduction in 4%. A total of 10 3D tibiotalar joint rotational patterns were observed under axial loading. There were no significant differences in the frequencies of the 10 tibiotalar joint rotational patterns (p > 0:05). Conclusions: The 3D tibiotalar joint rotational patterns under axial loading were consolidated into 10 directions, which provides insight into the subject-specific kinematics of the tibiotalar joint.

Validity of fascicle length estimation in the vastus lateralis and vastus intermedius using ultrasonography

The purpose of this study was to determine the validity of fascicle length estimation in the vastus lateralis (VL) and vastus intermedius (VI) using ultrasonography. The fascicle lengths of the VL and VI muscles were measured directly (dFL) using calipers, and were estimated (estmFL) using ultrasonography, in 10 legs from five Thiel's embalmed cadavers. To determine the validity of the estmFLs, FL was estimated using five previously published models and compared with dFL. The intraclass correlation coefficients (ICCs) of two of the five models were >0.75, indicating that these estimates were valid. Both of these models combined measurement of the length of the visible part of the fascicle with linear extrapolation of the length of the part of the fascicle that was not visible on the sonographic image. The ICCs and absolute% difference were best in models that used appropriate pennation angles. These results suggest that two of the five previously published models are valid for obtaining estmFL of the VL and VI using ultrasonography. (C) 2014 Elsevier Ltd. All rights reserved.

静的ストレッチングが安静時の筋スティフネスに及ぼす影響

若年健常成人10名(男性5名、女性5名、平均21.8歳)を対象に、超音波剪断波エラストグラフィを用いて筋スティフネスに及ぼす静的ストレッチング効果について検討した。ストレッチング介入前に計測した剪断弾性係数の良好な再現性(腓腹筋内側頭:ICC=0.98・CV:1.2、腓腹筋外側頭:ICC=0.97・CV:2.0)を確認した。腓腹筋の剪断弾性係数は筋頭と時間に有意な主効果を認め、外側頭に比べ内側頭で高値を示した。ストレッチング介入直後に両筋頭とも剪断弾性係数は減少し、時間経過に伴って減少率が小さくなったが、15分後までは有意な低値を示し、20分後で初期値に戻った。羽状角はストレッチング介入前と直後で内側頭、外側頭とも有意差は認めなかったが、背屈角度は介入直後で有意に増大し、20分後に初期値に戻った。

Osgood-Schlatter病の遺残骨片を外側アプローチを用いて摘出した2例

症例1(中学1年生、女)。3〜4年前からスポーツ活動時に両膝前方疼痛を自覚、Osgood-Schlatter病と診断され、保存治療を受けるも疼痛が増強したため受診となった。所見では両側脛骨粗面の突出、脛骨粗面から膝蓋腱上に圧痛が認められ、膝関節可動域制限はないものの、スクワット動作は困難であった。一方、X線では両脛骨粗面近位に複数の遊離骨片ほか、左膝には脛骨粗面の近位に骨隆起がみられ、MRIでは両膝とも膝蓋腱脛骨付着部の肥厚や不整、信号変化、また複数の骨片と深膝蓋下滑液包内の水腫が認められた。以上より、治療として膝蓋腱外側より進入して膝蓋腱付着部近位腹側を展開し、複数の遊離骨片の摘出を行い、次いで滑液包、脛骨粗面近位の骨突出部を切除した。その結果、術後のX線検査では骨片や骨突出は消失し、4ヵ月経過で患者はスポーツに完全復帰することができた。尚、術後1年目に膝立ち動作を行なったところ、痛みは認められなかった。症例2(高校2年生、男)。3年前からスポーツ活動時に左膝前方に疼痛を自覚、Osgood-Schlatter病と診断され、保存治療を受けていたが改善しなかったため受診となった。所見では左脛骨粗面に突出がみられ、症例1と同様の症状、画像所見であったことから同様の手術が施行された。その結果、術後3ヵ月で患者はスポーツに復帰することができ、術後6ヵ月目の膝立ち動作では痛みは認められなかった。

健常若年ボランティア13名を被験者として超音波せん断波エラストグラフィによる腓腹筋の弾性計測を試み、検者内再現性について検討するとともに、計測肢位・足関節角度の違いが計測値に及ぼす影響について検討した。結果、検者内再現性は良好であった。計測肢位・足関節角度の違いによって計測値は著明に増減し、この増減は、筋活動や筋の他動的伸張度を反映する可能性が示唆された。

J Strength Cond Res 25(8): 2255-2264, 2011-Functional performance tests (FPTs) that consist of movements, such as hopping, landing, and cutting, provide useful measurements. Although some tests have been established for kinematic studies of the knee joint, very few tests have been established for the ankle joint. To use the FPT as a test battery for patients with an ankle sprain, it is necessary to document typical patterns of muscle activation and range of motion (ROM) of the ankle joint during FPTs. Therefore, the purpose of this study was to investigate the pattern of the ROM of the ankle inversion/eversion and the muscle activity of the peroneus longus muscle (PL) and the tibial anterior muscle (TA) in normal subjects during the side-hop test. To emphasize the characteristics of ROM and electromyography (EMG) at each phase, the side-hop tests were divided into 4 phases: lateral-hop contact phase (LC), lateral-hop flight phase (LF), medial hop contact phase (MC), and medial hop flight phase (MF), and the ROM of ankle inversion/eversion, a peak angle of ankle inversion, and Integral EMG (IEMG) of PL and TA compared among 4 phases. Fifteen male subjects with no symptoms of ankle joint problems participated in this research. The ROM of ankle inversion/eversion during the side-hop test was 27 +/- 3.8 degrees (mean +/- SD), and there was a significant difference in the ROM of ankle inversion/eversion among 4 phases (p < 0.05). The phase in which the widest ROM was presented was the MF. A peak angle of the ankle inversion at MC was significantly greater than at LC and MF (p < 0.05). A peak angle of the ankle inversion at LF was significantly greater than at LC and MF. The PL remained contracting with 50-160% of maximal voluntary contraction (MVC). The IEMGs of PL in both the contact phases were significantly greater than in both the flight phases (p < 0.05). In addition, the PL activity at LC was significantly greater than at MC. The TA remained contracting at 50-80% of MVC through the side-hop test. The IEMG of TA at both the contact phases was significantly greater than at 2 flight phases. However, there was no significant difference between LC and MF. Results of this study could be useful as basic data when evaluating the validity of the side-hop test for patients with ankle sprain.

本学では、保健医療総論IIIでの討議型グループ学習法として、従来から実施してきた「役割別グループ学習」に加え、異なる役割との話し合いを行う「役割混成グループ学習」を平成21年度より新たに導入した。今回、このグループ学習を実施した学生98名のレポートを分析した結果、本学習法は「視野の広がり」「相手や自分の役割の理解」を助ける上で有効な方法であることが示された。

Quantitative measurement of the ankle joint anterior drawer test using ultrasonography

[Purpose]: Using ultrasonography diagnostic equipment, measurement of the extension distance of the anterior talofibular ligament (ATFL) was performed at rest and in the anterior drawer test (ADT), and the intra-rater and inter-rater reproducibility was examined. [Subjects]: Ten ankles of 8 university students who had suffered an ankle sprain within the last 12 months were the subjects. [Method]: Ultrasonographic images were captured with an 8 MHz linear probe on the anterolateral ankle at rest and at the time of ADT. The distance between the talus and the lateral malleous was measured as the ATFL extension on a PC, and the intra-rater and inter-rater reproducibility was examined using intraclass correlation coefficients (ICC). We also compared the correspondence of the talus - lateral malleous distance at rest as well as at the time of ADT with the t test. [Results]: At both rest and at the time of ADT, high intrarater and inter-rater reproducibilities were found. We found a significant difference between the talus - lateral malleous distance at rest, 16.5 ± 3.9 mm, and when performing the ADT, 20.9 ± 4.9 mm. [Conclusion]: From the aspects of both safety and reproducibility, we confirmed the utility of quantitative assessment using ADT with ultrasonographic imaging for ankle sprains.
