    Objective: This work aimed to investigate the effects of prism adaptation on unilateral spatial neglect following right hemispheric stroke.Design: Systematic review and meta-analysis of randomized controlled trials (RCTs).Patients: Patients with unilateral spatial neglect following right hemispheric stroke.Methods: RCTs comparing prism adaptation with placebo therapy were systematically searched across 4 databases (PubMed, Web of Science, CINAHL, and Cochrane Library). Screening, data extraction, and quality assessment were performed by 2 independent reviewers.Results: A total of 7 RCTs, involving 227 participants, satisfied the eligibility criteria. The results showed significant short-term effects of prism adaptation on neglect outcomes (SMD: 0.49 [95% CI: 0.07 to 0.92], p = 0.02) but not on the Catherine Bergego Scale (CBS) (SMD: –0.38 [95% CI: –1.27 to 0.51], p = 0.40). Subgroup analyses revealed that larger prism angles (exceeding 10°) had greater prism adaptation effects on both neglect outcomes and CBS (SMD: 0.71 [95% CI: 0.30 to 1.12], p = 0.0007 and SMD: –0.77 [95% CI: –1.51 to –0.02], p = 0.04, respectively).Conclusion: This study demonstrated that larger prism angle with greater than 10° was identified as a crucial factor in eliciting prism adaptation effects. These findings support the use of prism adaptation with angles exceeding 10° as a therapeutic approach for unilateral spatial neglect.

    <b>Objective:</b> Recently, we developed a line bisection task (LBT) that could be performed on a tablet personal computer (PC) by recording the results and automatically calculating the lateral deviation from the true center for each trial. The present study aimed to verify the validity and reliability of the LBT on a tablet PC. Additionally, we examined whether there were any differences between the results of the tablet and paper LBTs.

    <b>Methods:</b> The LBT was performed both on paper and tablet PC by 26 right-handed healthy adult volunteers (mean age 21.2 ± 2.1 years). Lines of three different lengths (200, 100, and 50 mm) were presented five-times in a pseudorandom order in each task condition. The participants were asked to mark a subjective midpoint with a pencil or a dedicated pen. Additionally, eight participants performed the LBT on the tablet twice to evaluate reliability.

    <b>Results:</b> Data analysis revealed a significant and “strong” to “very strong” correlation between the results of paper and tablet LBTs for each line length. Additionally, “good” test-retest reliability was confirmed by the results of the tablet LBT. There was no difference in the LBT results for each line length between the two task conditions.

    <b>Conclusions:</b> The validity and reliability of the newly developed tablet LBT were confirmed. Additionally, the LBT performed on the tablet PC yielded results equivalent to those obtained when performed on paper.

    <b>Introduction:</b> Prism adaptation (PA) is a promising treatment to improve unilateral spatial neglect (USN). Egocentric neglect and allocentric neglect are the two types of USN in terms of spatial reference frames, but no study was reported the PA affects allocentric frame selectively. This study therefore aimed to investigate whether the PA effect could be observed only on allocentric frame in healthy participants.

    <b>Method:</b> Thirty right-handed participants (12 females and 18 males, mean age 22.6 years), wore the right wedged prismatic glasses that created an optical shift of 5.7° to the left. They were asked to reach two visual targets with the right index finger 100 times alternately. Outcomes were measured through the line bisection task where lines of two different lengths (200 mm and 50 mm) were presented one by one in front of or on the left or right side of the participants.

    <b>Results:</b> The subjective midpoint on the 200-mm line shifted significantly to the right after the PA, but not on the 50-mm line. Additionally, the degree of rightward deviation was not affected by the position of the presented line regardless of its length.

    <b>Discussion:</b> These results indicate that the PA task induced allocentric neglect-like behavior. Hence, PA affected only the allocentric frame among healthy participants. The results of this study may contribute to the evaluation and treatment of allocentric neglect.

    Objective: This study aimed to explore the influence of coronavirus disease-2019 (COVID-19) on occupational therapy (OT) for physical disorders, including changes in the assessment, treatment, other restrictions, and measures of OT.

    Methods: A questionnaire survey was conducted among occupational therapists working in Hokkaido, Japan, and 123 out of the 334 (36.8%) were from OT facilities that target physical disorders. The responses were classified the categories and codes by [ ] and &lt; &gt;, respectively.

    Results: The number of patients decreased in 47.1% of the OT facilities after the pandemic declaration. Only one facility reported &lt;decrease in intervention with palpation&gt;. Therefore, [thoroughness of standard precautions] including &lt;disinfection of materials and common use areas&gt;, &lt;hand hygiene&gt;, and [changes in treatment structure] including &lt;restriction on use of materials&gt; were implemented. Additionally, there were not only [restrictions on participation of patients] and [restrictions on outpatient services], but also [restrictions on operations of OT], such as &lt;cancellation or change of the methods of meetings and workshops&gt; and so on. Furthermore, [changes in treatment structure] and [setting criteria for discontinuation of participation] were utilized in some facilities to prevent and to reduce the risk of infection.

    Conclusions: By revisiting the assessment and treatment guidelines on infection control, it is possible to provide continuous OT services and to tackle the challenges posed by the pandemic.

    Although the illusion that the mirror image of a hand or limb could be recognized as a part of one's body behind the mirror, the effect of adding tactile stimulation to this illusion remains unknown. We, therefore, examined how the timing of tactile stimulation affects the induction of body ownership on the mirror image. Twenty-one healthy, right-handed participants (mean age = 23.0 ± 1.0 years, no medical history of neurological and/or psychiatric disorders) were enrolled and a crossover design was adopted in this study. Participants' right and left hands were placed on the front and back sides of the mirror, respectively, then they were asked to keep looking at their right hand in the mirror. All participants experienced two experiments; one was with tactile stimulation that was synchronized with the movement of a mirror image (synchronous condition), and the other one was with tactile stimulation that was not synchronized (asynchronous condition). The qualitative degree of body ownership for the mirrored hand was evaluated by a questionnaire. Proprioceptive drift (PD), an illusory shift of the felt position of the real hand toward the mirrored hand was used for quantitative evaluation of body ownership and measured at "baseline," "immediately after stimulation," "2 min after stimulation," and "4 min after stimulation." The results of the questionnaire revealed that some items of body ownership rating were higher in the synchronous condition than in the asynchronous condition (p < 0.05). We found that PD occurred from immediately after to 4 min after stimulation in both conditions (p < 0.01) and there was no difference in the results between the conditions. From the dissociation of these results, we interpreted that body ownership could be elicited by different mechanisms depending on the task demand. Our results may contribute to the understanding of the multisensory integration mechanism of visual and tactile stimulation during mirror illusion induction.

    Unilateral spatial neglect (USN) is defined as a failure to pay attention to the contralesional space due to the brain damage. As this symptom affects on many aspect of activities of daily living, a lots of therapeutic approaches were developed so far. Among them, prism adaptation (PA) technique is one of the most promising methods because it could produce generalized and long lasting after-effect and is effective even for chronic USN patients. It is, however, true that there are USN patients showing no PA effect. Unfortunately, we cannot tell in advance who can adapt to prisms and show improvement of their neglect symptom at this moment. In further studies, to know who is more likely to adapt to prisms we should analyze clinical data from USN patients showing improvement by PA. In addition, to be an effective PA technique for much more patients with USN we should clarify what it the best condition in PA task.

    The main aim of this study is to determine the reference frame of the pointing errors that characterize patients with unilateral optic ataxia (OA). The reaching errors of seven patients with unilateral OA when pointing on a 2D matrix in peripheral vision were investigated in order to better qualify the reference frame of their deficit. Patients were asked to fixate a central target and then to point at one of 24 visual targets presented in their left or right peripheral visual fields, with their left or right hands. The four left and the three right hemisphere lesion patients with OA exhibited an identical pattern of results. In the contralesional visual field, error vectors were systematically directed toward the fixation point, rather than horizontally toward the side of the lesion. OA results from a deficit in transforming targets' eye-centred coordinates into appropriate motor commands.

    BACKGROUND: Previous studies on unilateral neglect have shown that there are at least two types of neglect-i.e., body-centered and stimulus-centered neglect. These symptoms suggest that the human brain has at least two different reference frames for processing external space. It is unknown, however, whether these two frames are represented independently in the brain and if so, which areas (or networks) of the brain are responsible for each frame of reference. OBJECTIVE: To determine whether body-centered neglect and stimulus-centered neglect can be dissociated in patients with brain injury. METHODS: New figure discriminative cancellation tasks were designed to simultaneously assess body-centered neglect and stimulus-centered neglect. Two neglect patients with lesions located in different anatomic regions were required to circle every complete figure and to cross out every figure with a missing portion on a sheet of white (29.7 x 42 cm) paper. RESULTS: Patient 1 omitted leftward stimuli on the paper, but the stimuli he found were correctly circled or crossed out. On the other hand, Patient 2 marked stimuli across the paper although he mistakenly circled stimuli that were missing a portion of their left side. Neither patient manifested interaction between the two types of neglect. CONCLUSION: The results of this study clearly showed double dissociation between the two types of neglect. Furthermore, it not only provides evidence that there are two distinct systems of reference frame for external space in the human brain, but also adds new knowledge indicating that these two systems function independently, at least in part.

    Unilateral spatial neglect (USN) is defined as a failure to pay attention to the contralesional space due to the brain damage. As this symptom affects on many aspect of activities of daily living, a lots of therapeutic approaches were developed so far. Among them, prism adaptation (PA) technique is one of the most promising methods because it could produce generalized and long lasting after-effect and is effective even for chronic USN patients. It is, however, true that there are USN patients showing no PA effect. Unfortunately, we cannot tell in advance who can adapt to prisms and show improvement of their neglect symptom at this moment. In further studies, to know who is more likely to adapt to prisms we should analyze clinical data from USN patients showing improvement by PA. In addition, to be an effective PA technique for much more patients with USN we should clarify what it the best condition in PA task.

    上腕骨外側上顆炎と診断後、鏡視下手術を施行した8例8肘を対象に、術後1ヵ月、2ヵ月、3ヵ月時に握力と手関節伸展力の計測を行い、握力と手関節伸展力の回復過程の関係を調査した。健患比の平均握力は術後1ヵ月時と比べ、2ヵ月、3ヵ月時ともに有意に改善したが、平均手関節伸展力は有意な改善を認めなかった。また術後2ヵ月、3ヵ月時には、手関節伸展力と比べ握力が有意に改善した。本研究より、上腕骨外側上顆炎鏡視下手術後は握力の回復が先行し、手関節伸展力の回復はより長期間要すると考えられた。(著者抄録)

    本研究の目的は、ヒトを対象として、先行研究で作成した課題を元に、選択的注意機能に関連する脳活動および活動特性を解明することである。本研究では、類似した集合から特定の対象を探索して選択する、先行研究の課題を更に発展させ、より簡便に、かつ精度の高い検査課題の開発を目指す。併せて脳活動計測を行い、類似した対象からの探索に関わる選択的注意機能を、どの脳領域がどのような時間推移で活動・制御しているかについて、明らかにすることを目指す。本研究により、注意機能検査のエビデンス向上および、注意機能障害に対する効果的なリハビリテーション介入の方法検討につながる可能性がある。本研究では、以下の２段階に分けて検討を進める。段階１は健常者を対象とし、先行研究で作成した課題を元に、類似した集合からの探索課題を作成・施行し、課題実施時間を計測する。併せて、注意機能評価に用いられてきた検査を含む関連検査を行い、本課題との関連性について検討する。段階２では、段階１にて作成した本研究課題の実施中における脳活動の計測を行い、類似した集合から特定の対象を探索するのに関連する脳機能を解明する。
    本年度の成果として、段階１で使用するプログラムを作成し、ボタン押しにより反応時間を計測する課題を作成した。また作成したプログラムを健常被験者に施行し、行動実験のデータ取得が開始することができた。しかし、新型コロナウイルスの感染拡大に伴う緊急事態宣言、及びまん延防止等重点措置の発令により、ヒトを対象とした研究の実施が大きく制限され、十分に研究を進めることが困難であった。
    次年度は、段階１の行動実験のデータ取得を進めるとともに、段階２における課題遂行中の脳波や近赤外光スペクトロスコピー（NIRS）などの生理学的指標の計測に向けた環境設定、及びその計測を進める。

    Prism adaptation technique with a few ways of target presentation was carried out in normals to clarify which condition is the most promising. Based on this result, three different ways of target presentation was adopted for prism adaptation in patients with left unilateral spatial neglect. Amelioration of left unilateral spatial neglect by prism adaptation was evaluated with line copying task, which was newly developed. As prism adaptation effect was positive in every target condition, it was not possible to tell that which is the best for neglect patients. In addition, finding behavioral and anatomical differences between patients with the positive effect and those with the negative effect, the results of paper and pencil tasks and lesion site were analyzed, respectively. These analyses, however, could not detect any differences between them.
