    Purpose<BR>The purpose of this study was to identify factors related to the decision-making on stages of labor done by midwives in practice.<BR>Method<BR>The subjects of this study were eight midwives in practice who had 14.6-year clinical experience on the average. The data were collected using semi-structured interviews andrecorded with participant's consent. They talked freely about how they had assessed the cli-ent and provided care during one normal delivery. Aftereach interview was transcribed, the data were compiled and then analyzed qualitatively using a grounded theory approach.<BR>Results<BR>As a result, seven factors were extracted from the data as follows:<BR>1) judging the normalcy of the labor and it's progress, 2) respecting and understanding toward the woman's desires, 3) assuring the woman's self-determination, 4) providing appropriate care for the family to keep its balance, 5) providing physical and psychological care for the woman, 6) identifying the woman's anxiety about potential risk and reassuring thesafety of the mother and the baby, and 7) integrating midwives' beliefs with knowledge.<BR>Conclusion<BR>Belief in respect for natural and daily rhythm on delivery was a basis for the midwife's decision-making. They provided a support based on such belief integrated with sophisticated knowledge through advanced experience.

    This study was conducted to observe the trends among- graduates who specialized in PHN and Midwifery Program in the Department of Nursing and to assess the integrated education of public health nurses and midwives. The followings were obtained from a survey of 367 students who graduated. 1) The occupation rate among the graduates was 79% in March 2001 and 76% of them were full-time employees. 2) The sorts of professions could be categorized as 132 public health nurses (36%), 77 midwives (21%), 29 hospital nurses (10.6%), and 28 teachers (7.6%). Most of them engaged in municipal health centers or general hospitals. 3) Immediately after graduation, 216 subjects were recruited as midwives (58.9%) and 108 as public health nurses (29.4%). The 56% subjects were resigned within the first five years from the fresh appointment due to their marriage, childbirth, childcare, and an interest in some other fields or institutions. 4) Total number of 101 subjects (21.5%) used both of their qualifications as public nurses and midwifes. 5) The contents used in the program helped 94.6% and 73.6% in their professional and daily lives, respectively. The techniques practiced in the lessons improved to get the knowledge specialized and contributed to gain self-worth with the deep human understandings. These were consisted in the core of the Integrated Program for PHN and Midwifery. 6) Respondent's hopes for their Alma Mater be with spiritual values for health science based on Christian point of views such as human affection and compassion to enhance the basic nursing education and to progress as a long-term academic institution.

    The purpose of this study was to articulate the phase model for the acquisition of practice-based knowledge by midwives from their experience in labor and delivery care. Data were collected from midwives working at hospital or maternity home through interview and questionnaire survey. Data from selected 475 respondents were statistically analyzed to examine if there was relationship between the number of years in midwifery and the types of practice-based knowledge acquired. Knowledge and skills acquired significantly more by those with at least ten years of experience related to how to determine if the labor progress was normal or not and how to respect the pregnant woman’s emotions and keep her emotionally stable. They acquired these knowledge and skills from the experience of assessing the fetal weight by palpation and assisti ng delivery without perineal protection. Those under ten years of experience had significantly higher scores in learning the importance of ensuring that the delivery should take its natural course and of maintaining good communication with the pregnant woman. They acquired this knowledge from the experience of witnessing the work of senior midwives and having a habit of reflecting own performance.
