To predict and ensure a healthy and high-performing nursing workforce, it is necessary to identify the antecedents that promote work engagement, especially among early-career nurses. To date no study has focused on this. This longitudinal survey, administered to 1204 nurses working in seven general hospitals with 200 or more beds in four prefectures in Japan at two different times in 2019, aims to examine the causal relationship between the personal and professional resources for nurses to work vigorously (PPR-N) and work engagement among nurses in the early stages of their careers, considering time as a key mediating factor. The analysis of structural equation modeling using the cross-lagged effect model supported that PPR-N had significant and positive effects on work engagement after 3 months among early-career nurses with less than 10 years of nursing experience. The PPR-N is a reliable antecedent of work engagement, which is typical of early-career nurses. These results may be provided guidance for managers in overseeing the work environment to ensure a thriving sustainable nursing workforce.

BACKGROUND: For current medical education, community-based primary care for the elderly is an essential topic. This study aimed to establish a scale of community-based assessment for clinical and emergency practice (C-CEP). METHODS: A self-assessment scale for C-CEP was developed according to four steps. Initially, we reviewed publications from the societies of the United States, British, and Japan regarding educational goals. In addition, we searched MEDLINE for educational goals regarding attitude, skills, and knowledge. Getting together, we established 23 items as the educational goals of the C-CEP. Second, we collected responses for these 23 items from 5th-grade medical students (n = 195). Third, we conducted an exploratory factor analysis (EFA) using their responses to determine the fundamental structure of the self-assessment scale. Finally, a confirmatory factor analysis (CFA) was performed to assess the fitness of the self-assessment scale developing the EFA, resulting in modification of the items. RESULTS: In EFA and CFA results, C-CEP Scale consisted of four factors with 15 items: "Attitude and communication in emergency care," Basic clinical skills," "Knowledge of community healthcare," and "Knowledge of evidence-based medicine perseverance." The model fit indices were acceptable (Goodness of Fix Index = 0.928, Adjusted Goodness of Fit Index = 0.900, Comparative Fit Index = 0.979, and Root Mean Square Error of Approximation = 0.045). The values of McDonald's omega as an estimate of scale reliability were more than 0.7 in all four factors. As for test-retest reliability, the intraclass correlation coefficients were ≥ 0.58 for all factors. All four factors of the C-CEP Scale correlated positively with the Medical Professionalism Evaluation Scale subscales. CONCLUSIONS: We developed a valid and reliable self-assessment scale to assess student competence.

In the 1990s, interprofessional education (IPE) was introduced to health professionals in higher education; it has come to be regarded as a standard learning method. However, major barriers remain regarding its introduction and smooth operation. This study conducted a questionnaire survey at educational facilities for health professionals in Japan; it developed scales to measure the recognition of barriers to and benefits of IPE implementation. We surveyed chairpersons responsible for 2,690 courses in Japanese health education facilities. We used for analysis the responses related to 767 courses (valid response rate, 28.5%). In all, 216 courses (28.7%) implemented IPE. We conducted exploratory factor analysis and developed scales for measuring the recognition of barriers to IPE implementation (15 items) and its benefits (11 items). We observed a significant relationship between the state of IPE implementation and recognition of barriers to and benefits of IPE. Using information and communication technology and faculty development for faculty members would be effective in removing the barriers to IPE implementation.

GANBATTA: ("Well done!") is a term used frequently in Japan to praise children in medical settings when they have worked hard or made a courageous attempt. Therefore, the aim of this study was to clarify the factor structure of behaviors associated with ganbatta in young children (age 3-7 years) undergoing blood sampling or vaccine injection. A cross-sectional questionnaire survey was carried out on 150 selected pediatric nursing educators holding a nursing license (age ≥20 years). An exploratory factor analysis was then conducted using the principal factor method with promax rotation to confirm the construct validity. Cronbach's α coefficients were calculated for each identified factor to verify the internal consistency of the item. The mean (standard deviation) lengths of pediatric nursing and pediatric nursing education experience were 7.8 (4.7) and 9.9 (8.1) years, respectively. The cumulative contribution rate was 70.4%. The KMO measure of sampling adequacy was 0.927. The p value of Bartlett's test of sphericity was <0.001%. Finally, among the children's behaviors, 5 factors were identified and 25 items were selected: Factor 1, Proactive participation, containing 7 items; Factor 2, Sense of relief and joy after needle removal, 6 items; Factor 3, Negative emotional expression, 4 items; Factor 4, Information-seeking, 5 items; and Factor 5, Moving on from the needle procedure, 3 items. These findings indicate that ganbatta is a complex concept that encompasses a variety of aspects in young children in relation to needle procedures. Evaluating a young child's ganbatta behaviors during a needle procedure can serve as an indicator to evaluate the quality of care provided by nurses. Sharing the evaluated results with children and their parents may serve as a foundation to open discussions in regard to higher-quality care. In addition, observing ganbatta behaviors during a needle procedure and providing feedback to the child could help them feel like they have done a good job.

BACKGROUND AND AIM OF THE WORK: The Chiba Inter-professional Competency Scale (CICS29) validated in several languages, it is a self-report instrument that investigates the degree of interprofessional collaboration in six areas: attitudes and beliefs of the professional; ability to manage a work group; actions to achieve goals; providing assistance that respects the patient; attitudes and behaviours that improve the cohesion of the working group; fulfilling or performing the own professional role. In addition to being recent, the scale investigates collaboration among all health professionals, and is not limited to specific professionals. The aim of the study was to validate the Italian version of CICS29. METHOD: A questionnaire-based study was conducted with an Italian sample consisting of 530 health professionals (419 women¸ mean age = 40 years, SD = 10.7; range 23- 58 years). The internal validity was measured using factor analysis. To verify the convergent validity, the Italian Version of Interprofessional Collaborative Competency Attainment Survey (ICCAS) was correlated with the CICS29; Results: The reliability and the internal validity of the CICS29 revealed 6 factors corresponding to the original subscales. The analysis presents an excellent sample adequacy measure (KMO = .933) with the scores ranging from 0.62 to 0.78 for the interclass correlation coefficients of the 6 domains. A significant level of correlation was found between the subscales of the CICS29 and the ICCAS. CONCLUSIONS: In conclusion, the Italian version of CICS29 has a satisfactory level of reliability and validity and it is recommended for measuring interprofessional collaboration of the health professionals. PURPOSE:The undergraduate version of the Interprofessional Learning Scale (UIPLS) was developed to evaluate students' competency in interprofessional education because the existing scales were too professional for entry-level students to understand and respond to effectively. In addition, existing scales have some noted problems with validity and reliability. METHODS:An early 49-item version of the UIPLS was completed by 837 first-year students from two universities and one technical college, including departments for various professionals, in Hokkaido prefecture, Japan. The most common age group was 18-19 years (89.4%) and 61.2% were women. Ten items from the Minnesota Multiphasic Personality Inventory were used to exclude respondents who tended to create a socially favorable impression and/or a possible random and incorrectly scored profile. Kikuchi's Social Skills Scale (18 items) was also used as an external standard to demonstrate the convergent validity of the UIPLS. RESULTS:Eighteen items across four factors were extracted by factor analysis: "reflection on group work," "attitude towards group work," "knowledge of interprofessional working," and "skill in group work." Internal consistency was confirmed by Cronbach's alpha coefficient (>0.75). CONCLUSIONS:With good indicators of its validity and reliability, the UIPLS shows some promising aspects to evaluate students' competency for interprofessional working.

The purpose of this study was to refine the items on a scale measuring interprofessional collaborative competency that was developed by the authors in an earlier pilot study. A questionnaire-based study was conducted with a sample of 2133 health professionals using the reformulated questionnaire. Construct validity was tested by comparing the survey results with a covariance structure analysis and the domains of interprofessional collaboration competencies presented in previous studies. A second survey was conducted 2 weeks later with a sample of 571 nursing professionals, using the same survey form to test its reliability. We constructed a model comprising 29 observed variables and six latent variables (the Chiba Interprofessional Competency Scale: CICS29), and obtained the following values for the model's goodness of fit: GFI = 0.925, AGFI = 0.908, CFI = 0.950, RMSEA = 0.049. With regard to reliability, we obtained scores ranging from 0.65 to 0.77 for the intraclass correlation coefficients of the six domains. Compared with the interprofessional collaboration scales indicated in previous studies, the CICS29 was found to have subsumed the key concepts that should be configured as interprofessional collaboration competencies. The CICS29 appeared to have satisfactory levels of reliability and validity and is recommended as a scale for measuring competencies of interprofessional practice.

In the present study, we focus on the extent to which elderly living in solitude are satisfied with their lifestyles. To examine this issue, semi-structured interviews were conducted with twelve elderly persons (ages 69-90). The transcripts from these interviews were analyzed through a qualitative-inductive approach. Overall, the obtained information could be classified into seven categories. Concerning their self-relationships, five categories could be noted: (1) Manners of interpretation regarding what it means to live alone; (2) attitudes and behavior exhibited in order to find pleasure and meaning in one's life; (3) attitudes and behavior employed to survive hardship; (4) pride felt in one's endeavors and past achievements; and (5) intentions behind self-confrontation. Concerning relationships with others, two categories could be seen: (6) An existence that provides encouragement to the elderly and (7) feelings of being useful to others. The information provided during the interviews suggests that in order for the elderly to better accept their present situation, it is necessary that they are able to find ways of deepening and improving both their self-relationships and relationships with others.

The aim of this study was to evaluate the effectiveness of Interprofessional Education (IPE), which is implementedsoon after students enter university. Sapporo Medical University began implementing IPE in the Medicine, Nursing,Physical Therapy and Occupational Therapy departments in 2004. The program currently runs from the first to the fourthyear of university and comprises lectures, exercises and practicum training. The optional course for-first year students wasupdated in 2012. We examined changes in social skills (KiSS-18) and Readiness for Interprofessional Learning Scale (RIPLS)scores before and after practicum training. The subjects included 39 students who completed both surveys (valid responserate: 67.2%). Dependent variable scores on the three RIPLS subscales improved after practical training. A significantdifference was observed in KiSS-18 scores between male and female students before the practicum. Female students hadalready received a high KiSS-18 score before the practicum. Scores on KiSS-18 and two RIPLS subscales (Teamwork andCollaboration, and IPE Opportunities) increased significantly for male students, whereas scores on the third RIPLS subscale(Uniqueness of Profession) increased significantly for female students. We found that early implementation of practicaltraining in IPE had at least a short-term effect, and that these effects varied depending on gender.

We conducted a questionnaire survey to explore respondents' and their families' experience of medical accidents. The survey involved 2000 people living in Fukuoka Prefecture, who were sent a questionnaire by mail. The valid response rate was 27.9%. One hundred and seventy-five medical accident cases were reported. We categorized all cases using ICD-10 codes and clinical categories regarding the nature of the problem. As a result, 14.3% were classified as "neoplasms" by ICD-10 codes, and 32.0% as "diagnosis" by the clinical categories. We identified three conditions of medical accidents recognized and judged by patients and their families. In the first, there was an unanticipated family member's death, or a condition suddenly worsened under treatment. Secondly, a result contrary to their expectation based on the prognosis was given. Thirdly, there were differences in the diagnosis, treatments, and outcome of medical care by medical professionals.

The present study was conducted to investigate the recognition and experience of adverse events by patients and/or their families at dental clinics. A self-administered questionnaire mail survey was conducted in 2,000 subjects (response rate, 29.0%). The responses included descriptions of 19 experiences of adverse events at dentists, and one of these subjects was interviewed. In regard to the recognition of adverse events at dental clinics, the patient and his/her family first recognized unusual pain, a decline in oral function, or other unusual changes, and then recognized and judged adverse events according to the information provided by medical professionals, such as discrepancy between the diagnosis/treatment and treatment outcomes. In addition, inappropriate communication by medical professionals had some influence on the recognition of adverse events. Medical professionals are required to interview patients regarding their past treatments and outcomes, and the treatments and outcomes they want, and then are required to provide patients with sufficient explanation about potentially negative treatment outcomes and symptoms.

There are many researches on patient satisfaction in Japan in the last decade and we have to utilize the information from the researches effectively. Very few attempts have been made at collecting data from more than one institution. The purpose of this study is to fill in this gap and to examine the differences in the degree of outpatient satisfaction and the factors which affect outpatient satisfaction depending on hospital types classified by bed size and function. The results indicated that factors such as the evaluation of the doctor, the evaluation of the amenity services, perceived change in health had a statistical y significant effect on patient satisfaction among all the types of the hospitals. The standardized regression coefficient of the evaluation of the doctor was the highest in all the types of hospitals. Moreover, in an analysis of the university hospitals, patient satisfaction was affected by further factors in addition to the three factors mentioned above. Lower patient satisfaction was associated with younger age, longer waiting time, and a longer span of continuous hospital visits. University hospitals and large-scale hospitals need to take various measures such as decreasing the waiting time and creating a comfortable environment to spend the waiting time in order to improve medical service quality for patients.

This study conducted a questionnaire survey at educational facilities for medical and welfare professions in Japan and developed scales to measure the recognition of barriers to and benefits of IPE implementation. Chairpersons of a total of 2,690 courses in medical/welfare educational facilities in Japan were surveyed. The responses from 767 courses were used for the analysis (valid response rate: 28.5%). There were 216 courses (28.7%) implementing IPE. The exploratory factor analysis was conducted, and scales for measuring the recognition of barriers to IPE implementation (15 items) and its benefits (11 items) were developed. Significant relationship between the state of IPE implementation and the recognition of barriers to and benefits with IPE was observed. Notably, using Information and Communication Technology (ICT) and Faculty Development (FD) for faculty members would be effective to remove barriers to IPE implementation.

We were able to classify the obtained information into ten categories. Four of these concerned their self-relations: (1) Changes after one’s bereavement; (2) changes with time; (3) past events that changed their life style and (4) creative ability, absorbing oneself in a hobby and performing various tasks. Concerning relations with others, six categories could be noted: (5) Relations that provide encouragement to the elderly; (6) joyful interactions with other people; (7) becoming more distant from others; (8) finding connections with others; (9) ways of improving relationships with others and (10) the presence of significant others.
The analysis provided suggests that the elderly imagine others while they grapple with their own problems, and at the same time express spontaneous intentions concerning their relations with others.

We developed the EBP self-assessment scale (EBPQ-J). And semi-structured Interviews conducted with nurses, and content analysis revealed the impeding factor of EBP implementation. Based on this, we created a questionnaire and surveyed targeting the rehabilitation wards nationwide. As a result, the impeding factors of EBP implementation were "poor quality of interprofessional collaboration," and the other three factors. These impeding factors were not related to EBPQ-J scores. And We built a website as an EBP implementation support system (EBPISS), and use this EBPISS for six wards and carried out. There was no significant change in EBPQ-J scores before and after the overall intervention, but two wards improved significantly, two wards decreased significantly, and interaction observed in each ward(p = 0.0046.ANOVA). The EBP implementation support model in the rehabilitation ward was corrected, and the improvement points of the support system were clarified.

Elderly people living alone find themselves forced to try to accept their situations, which often includes the stress of having to cope with the loss of a loved one. It is important that they are able to find ways of deepening and improving both their self-relationships and relationships with others.
The elderly seek interdependent human relations, which provide comfort and support for people in weak positions. This study considers the importance respect has in enhancing life satisfaction in the elderly, examining how it relates not only to the ethical principle of autonomy, but also to the principle of vulnerability.

With the rapid increase of the number of the nursing university, the importance of the evaluation of the nursing practice ability required by the end of graduation of the university increases.
Therefore, it is urgent to develop a common test available in the nursing university throughout Japan to evaluate quality of Student's competency and knowledge required for a nursing student by a stage to begin the clinical trial in hospitals.
I received a scientific research funds subsidy from 2008 through 2010 and, for the purpose of developing such common test to evaluate ability and knowledge required before the clinical trials in hospitals, 1,500 multiple choice items from 18 nursing domains were made and carried out monitor trials based on paper-pencil tests and computer tests (by means of CBT : Computer Based Testing) for the 730 students and 220 students respectively., By comparing both results,it follows that the usefulness of the computer examination was confirmed in comparison with of the paper-pencil tests,

The goals of this research project were to : (1) describe ethical nursing practice in clinical settings, and(2)develop a system to support clinical nurses as they gain the ability to implement ethical practice. In order to capture the major components of ethical practice for nurses, the Moral Sensitivity Questionnaire (MSQ) and Ethical Behavior Test (EBT) were translated to Japanese, modified, and tested (-J suffix). MSQ-J and EBT-J were tested with a group of Japanese nurses working in a hospital. Reliabilities of both instruments were established. Content and construct validities of the instruments were sought using qualitative data from individual follow-up interviews. The result of factor analysis with MSQ-J showed some similarities with the original (Norweigen) instrument, though follow-up interviews revealed other potential factors unique to the moral sensitivity of Japanese nurses. This suggests the need of further exploratory studies for moral sensitivity. Content validity of EBT-J was supported by the interview data.Based on the findings from MSQ-J, EBT-J, and interviews, an Ethical Nursing Practice Study Group was introduced and conducted by the researchers with staff nurses in the subject hospital. Monthly meetings with the group provided a Place where participants could share and reflect upon their clinical experiences and learn from each other. The group-meeting approach seemed to be effective for participants, though attendance was limited due to a nursing shortage in the hospital. In addition, since the group meeting was introduced by outside researchers--- not from clinical nurses within the hospital---some staff nurses felt distance to the topic and were not interested to participate. It indicates that this kind of support system needs to be developed with close collaboration with clinical nurses.

The standardization in health care and patient safety education is indispensable for ensuring patient safety in all healthcare settings. In addition to securing patient safety, undergraduate and graduate medical education processes are necessary to prevent medical profession from causing medical accident. There are various possible methods for training and education, and the method with software for PC is the one. The advantages of the software for PC are to be able to study myself everywhere and study according to their proficiency level. Then, we developed the software for patient safety education for medical profession and undergraduate. In 2003, we examined the specification of the software and made a part of its. The software was designed as a database that serous incidents were able to be registered and retrieved, and users could study the process of the plan of the measures of safety. Moreover, the software had the mechanism that the incidents and the improvements could be shared with medical profession. In addition, it had the mechanism that could link from serious incidents with the medical equipment manufacturers and academic societies through website to obtain useful information. In 2004, we completed the software and evaluated, reexamined it. It seemed that the software developed by this research was effective to patient safety education.
