    研究実施に先立ち、covid-19感染症拡大下での看護及び助産教育に関する研究背景をより明確にするため、特に実習科目の実施状況に関する情報収集を行った。その結果、「新型コロナウィルス感染症に関連する保健師助産師看護師養成学校における臨地実習等の実施状況調査」によると、289教育課程中臨地実習の代替措置を行ったのは281課程であり、代替措置の方法の90.7%が「学内実習・学内演習」、88.6%が「オンライン実習」、83.3%が「紙面による課題学習」であった。ほとんどの大学が臨床・臨地実習を学内実習やオンラインなど複数の方法での代替実施をした状況であることが明らかになった。また、助産学実習においても全国助産師教育協議会が実態調査を行っており、学内実習やオンライン実習への切り替えにより、例年通りの実習が困難であった教育機関が多かった一方で、例年通りの分娩介助実習等が行ていた教育機関も一定数あり、助産師養成機関による教育内容の格差が課題として挙げられている。
    臨地・臨床実習の代替実習の取り組みについては、いくつか調査報告が出されており、学内やオンライン等で行うシミュレーション教育は、学生の知識と技術の統合といった点ではある一定程度の学習効果は得られることは共通見解といえる。しかし一方で、乳幼児の啼泣や母子の相互作用の様子、リアルな分娩経過中の産婦の様子や急変も含めた複数の事象が同時進行する臨床のリアリティを再現することには限界がある。
    先行研究の調査により、看護教育および助産教育ともに臨床・臨地の実習でコアとなる学習内容と臨床・臨地以外でも修得可能なものを整理し、新たな枠組みの中で教育プログラムの調整・再考を迫られており、現有のリソースを基に継続検討をしている状況であることが明らかになった。

    国内の看護管理者が行う病床管理とその役割、および周産期母子医療センターにおける病床管理について、文献検討をおこなった。
    周産期母子医療センターの病床管理については、研究者が医師であることが多く、その大半は新生児集中治療室の病床数が医療圏に対し充足しているか検討していた。周産期母子医療センターの母体胎児集中治療室および産科病床数が適正であるかの検討は、解説・総説、限定された地域や医療圏での調査報告が散見された。
    看護管理者が行っている病床管理に関する研究は、病院全体での病床稼働率の向上を図るうえで課題となる在院日数の短縮と院内全体での空床活用に関する看護部での組織的取り組みについて扱っていた。集中治療室管理料が算定される集中治療室等の有効活用に関する報告はあったが、母体胎児集中治療室の病床活用についての研究結果は見出されなかった。入室する妊産褥婦にとっての環境という視点で、母体胎児集中治療室の病床を分析した研究はわずかに認められたが、医療圏にとっての必要病床数の検討、対象者に必要な療養環境の判断に基づく病床管理、診療報酬を得るために母体胎児集中治療室の病床の効率的な運用に関する研究は認められなかった。総合周産期母子医療センターにおいては、母体胎児集中治療室および産科病床の両方を使って、入院に応需し、療養環境が選択されていると考えられる。総合周産期母子医療センターの産科および母体胎児集中治療室の病床管理について看護管理者が果たす役割について、明らかにしていく視点が明らかになった。

    MFICU看護職者のストレス影響要因,職場適応状況等について明らかにすることを目的に,全国の総合周産期母子医療センターの母体胎児集中治療室(MFICU)に勤務する看護職者を対象としてストレスの程度や影響要因,職場適応状況について,web入力による調査を行うことを計画している。まず,「職業性ストレス」「看護職のストレス」「労働環境」などについて文献検索を行った。その結果をふまえ研究会議を開催し,既存の尺度の利用可能状況を検討した。一つは,日本国内で広く用いられている「職業性ストレス簡易性尺度」を利用することができるかもしれないが,通常の産科病棟や助産師のストレスとの差異や,MFICU特有のストレス要因を明らかにするためには,独自に作成する必要があると考えた。これまでの,MFICUの研究で明らかにしたMFICUの特徴やストレス要因を活用する必要があると考えた。また,助産師のストレスに関する研究報告を中心に,もう少し文献検討を必要とする方向で意見がまとまった。その後,再度文献検索を行い,検討している。

    本研究は、病院前周産期症例の搬送の実態、救急隊と医療機関の連携の実態、関係職種の認識を調査し、分娩対応や搬送に係る一連の機能性を高める要素を明らかにすることを目的としている。本研究は、2段階の調査を実施する予定である。
    2021年度は、6か所の地域の病院前周産期症例の搬送経験を持つ救急隊員11人にインタビュー調査を実施した。搬送中に実際に分娩の補助を行った者、自宅で出産したばかりの母子を搬送した者、言語が通じない外国人夫婦、未受診妊婦の搬送を担当した者等、様々な経験からの語りが得られた。現在、インタビューデータの逐語録作成後、分析を行っている段階である。搬送を経験した救急隊員の語りからは、通報時の情報と現場到着時の状況が変化していることへの対応の難しさ、未受診妊婦などの特殊な状況下での対応の難しさがあること、普段から地域の医療職種とのコミュニケーションがあることが搬送時の連携を速やかにしていること等が明らかになってきている。これらをもとに病院前周産期症例の搬送に係る職種の機能性を上げる要素についてまとめる。

    A questionnaire survey and drawing analysis conducted with managers of regional liaison department in a regional medical care support hospitals revealed the following; staffing of outpatient departments, methods of collaboration and ideal placement of each department, and architectural forms of each department.

    The purpose of this study is to clarify the ability of midwives to learn from experience and the ability to nurture learning from experience, and to create tools to evaluate them. In order for midwives to learn from care experience, six elements are required, such as[learning from the practice and advice of senior midwives][reflecting on my own judgements and actions],[understanding how the patient feels by putting myself in her position] became clear. In order for seniors midwives to nurture the learning of junior midwives, there are four elements such as [ability to grasp the daily changes and growth of junior midwives] [ability to look inside oneself and correct the direction in which they interact with junior midwives] became clear. In the future, it will be an issue to repeat the operation of the created evaluation tools and build an education and training program.

    With the aim of constructing a foundation for facility planning guidelines for perinatal medical centers, we conducted a hearing and questionnaire survey of perinatal center directors and nursing managers throughout Japan in this study.
    The results suggested that of the facility standards under the current medical remuneration system, a bio clean room for the Maternal Fetal Intensive Care Unit (MFICU) is not a high priority, and that 7 ㎡ floor surface of the Neonatal Intensive Care Unit (NICU) is too small. These results demonstrated that planning guidelines required further examination. Our results also suggested that guidelines for department allocation within perinatal centers should position the MFICU, NICU, obstetrics ward and operating theater adjacent to the delivery room.

    This study aims to characterize the bacterial species inside closed infant incubators in a high temperature, high humidity environment and to determine the effectiveness of cleaning and disinfecting such incubators. We compared the effectiveness of different cleaning methods: we cleaned one group of incubators in use with disinfectant and another group using a dry wipe without any disinfectants. We did not detect any bacteria from both incubator groups. These results suggest that a high temperature, high humidity environment alone does not increase the amount of bacteria inside an incubator without any other predisposing factors. This implies that the bacteria detected inside an incubator originates from a newborn infant who acquires a bacterial flora after birth or through increased opportunities for contamination through treatment and care.

    Our aim was to develop a training program for nursing professionals working in the maternal-fetal intensive care unit (MFICU). We began by clarifying the training program’s requirements by conducting group interviews with mid-career MFICU nursing professionals. Afterwards, we developed a systematic training program based on the findings of previous research studies. To determine the program’s essential content, we surveyed nursing professionals with experience planning MFICU workshops. Our results showed that they agreed with over 90% of the workshop’s items. We also piloted-tested a portion of the program’s content. The participants’ feedback, which we requested, showed that almost all of the workshop’s goals were achieved and that the participants agreed with its necessity. In the future, we will make further refinements to the training program and improve the workshop’s planning and management.

    The aim of this study was to develop an educational program to increase perinatal safety for pregnant women who must travel long distances to give birth. We interviewed such women who live in town A, as well as public health nurses in town A and midwives working at delivery facilities, to determine the educational needs of these pregnant women and their current educational opportunities, and to investigate possible educational programs. We found that the program should include instructions for pregnant women how to prepare themselves during labor and how to call an ambulance in an emergency. while these pregnant women hoped to receive education before delivery, they have few educational opportunities. The primary challenge is to hire professionals with specialized knowledge about delivery.

    The purpose of this study was to articulate the phase model for the acquisition of practice-based knowledge by midwives from their experience in labor and delivery care. Data were collected from midwives working at hospital or maternity home through interview and questionnaire survey. Data from selected 475 respondents were statistically analyzed to examine if there was relationship between the number of years in midwifery and the types of practice-based knowledge acquired. Knowledge and skills acquired significantly more by those with at least ten years of experience related to how to determine if the labor progress was normal or not and how to respect the pregnant woman’s emotions and keep her emotionally stable. They acquired these knowledge and skills from the experience of assessing the fetal weight by palpation and assisti ng delivery without perineal protection. Those under ten years of experience had significantly higher scores in learning the importance of ensuring that the delivery should take its natural course and of maintaining good communication with the pregnant woman. They acquired this knowledge from the experience of witnessing the work of senior midwives and having a habit of reflecting own performance.

    One study involved pregnant women who lived a long distance away from the hospital or clinic at delivery. The results demonstrate that they felt it was important for information to be given to their friends and family, suggesting the necessity of health education programs including group work and peer support during preparations for delivery. Subjects of another study were nurses and nurse midwives who work in obstetric hospitals and clinics. Nurses and nurse midwives address pregnant women's problems ; pregnant women may have physical, economic, and mental problems and have to coordinate with their family to travel a long distance to the hospital and clinic. In addition, they have medical problems because they may deliver in their house or car without attending medical professionals. The results suggest that the program should include instructions for pregnant women who need a long-distance transfer to hospitals or clinics on how to prepare themselves during labor progression and when to call an ambulance in an emergency.

    This study aimed at clarifying the current nursing issues in maternal-fetal intensive care units(MFICU). As the first study, a nationwide questionnaire survey was conducted towards nursing managers concerning difficulties and solutions within managements in MFICU. It was found that difficulties were stress among nurses and administrative tasks, whereas training programs were provided as solutions in order to improve skills in nurses in MFICU. As the second study, interviews were conducted to clarify difficulties on the basis of characteristics and backgrounds of these hospitals, and also difficulties and skills to be MFICU. Difficulties in terms of administration were different in each hospital because of a various characteristic and backgrounds. Thus, in the second study, the research team obtained data concerning managements, nursing systems and education for the high-risk pregnancy through visiting hospitals in the United States of America and through a systematic review. It was suggested that nursing management systems had to be divided between the high-risk pregnancy and the low-risk pregnancy, and that intense education for the high-risk pregnancy were required.
