Purpose: This study aimed to clarify postpartum care provided by midwives for primiparas aged over 40.Methods: Data were collected through semi-structured interviews from 9 midwives with over 10 years of experience and analyzed by qualitatively and descriptively analyzed.Results: Midwives provided care [to help primiparas aged over 40 to continuously understand that the lives they had built for a long time would change], [to help primiparas can continuously care for the child, while adapting to the primiparas' pace and physical strength], [to help primiparas be convinced regarding childcare methods and be able to ask for help themselves]. On the other hand, since some primiparas aged over 40 depend on others for decision-making, midwives provided care [to help primiparas who depend on others for decision-making to be able to make decisions on their own and care for the child]. Since many primiparas cannot get sufficient support after leaving the hospital, midwives provided care [to create a support system so that primiparas who cannot get support need not persevere alone].Conclusion: This study clarified that midwives provided care that comprises understanding and respecting the sense of values and life experience of primiparas aged over 40.

平成27年度にA病院の他部門研修を受講した新人看護師69名(看護師65名、助産師4名、男性12名、女性57名、平均年齢23.1歳)を対象に、チーム医療研修の一環として実施した他部門研修における新人看護師の学びについて、研修終了レポートの内容を分析することにより考察を試みた。レポートの記載内容から「他部門研修の学び」を記述した一文を一記憶単位として抽出し、意味内容の類似性に従って、カテゴリー、サブカテゴリーを抽出した。分析の結果、20のサブカテゴリー、「他職種の役割機能の理解」「他職種への感謝と配慮の思い」「他職種連携の実感」「他職種連携の必要性の認識」「自己の看護の質向上につながる学び」「看護職としての自覚」「病院経営に対する意識の高まり」の七つのカテゴリーが抽出された。新人看護師は、他部門研修を通して他職種への理解に関する学び、他職種との連携に関する学び、看護職としての学びを得ていることが明らかになった。

We conducted the attitude survey toward the influence, caused by the merging of obstetric hospitals in Hokkaido, on midwives who worked in these merged hospitals due to the closing of the other obstetric services in the surrounding areas. It was cleared that the merging of obstetric hospitals imposed physical and mental burdens on midwives due to the increased amount of work. There were one for, 20 undecided about, and 11 against the merging of obstetric hospitals. Four categories, including 'the deterioration in the safety of medical care', 'the increase in the amount of work', 'the deterioration in the anxiety and the burden of pregnant women', and 'the insufficiency of cooperation and reliance on doctors', were reached by categorizing the opinions concerning the merging of obstetric hospitals. The reasons for pointing out 'the deterioration in the safety of medical care' were the absolute shortage of the staff, the pressure of work, the shortage of time to care for patients, and the increase in number of emergency admission and high-risk pregnancies. Finding a solution to the problems mentioned above might hold the key to the success of the merging of obstetric hospitals. Therefore, it might be necessary that midwives working in the merged hospitals give their opinions independently and participate in a project for improvement.
