    The purpose of this study is to develop a recurrence prevention support program for elderly patients with minor strokes that continues from the acute phase to home. Elderly patients with minor stroke within one week after stroke onset were interviewed twice, once on admission and once at the first outpatient visit, regarding the onset and recurrence of stroke and their health management. These results revealed a vague sense of urgency regarding recurrence and a sense of pride in their own health care. An approach to risk perception is considered necessary for continued recurrence prevention. Therefore, in order to measure the effect of the intervention, we worked on scaling risk perception regarding recurrence in elderly patients with minor stroke.

    The purpose of this study was to develop delirium care management for elderly patients in acute hospital settings. The research program was included to develop and administrate an educational program for nurses, and to assist delirium care leaders in the efforts to improve delirium care at participating wards. One year after the start of the program, the development process and outcomes of the program were evaluated by quantitative and qualitative analyses. The findings showed that nurses were improved awareness of the needs of delirium sufferers, resulting in changes in care quality. To improve the delirium care quality provided at general hospitals, it is necessary to organize the tasks involved in the wards, improve instructions to nurses in how to use the evaluation tools, and simplify recoding methods to allow nurses to concentrate on the delirium care, as well as to support and encourage the efforts of the nurses.

    The purposes of this study is to examine a method that, from reactions displayed by the elderly to the environmental factors, predicts the risk of onset, and reduces the risk of onset due to the environment.
    In the first stage, the hospitalized elderly patients' level of recognition of environmental factors involved in the onset of delirium was investigated. In the second stage, the target group was divided into those with onset of delirium and those without onset of delirium and a comparative verification process was conducted.
    The results of this study showed that differences were observed(<. 05) between the two participant groups regarding the physical and human environment in the following categories :
    ●The location of the room
    ●The frequency of visits of the nurses
    ●The presence or absence of medical examinations that exacerbate stress
    ●Interaction with other patients
    ●The presence of elements that heighten anxiety
    These differences are thought to be connected to the onset of delirium.

    The purpose of this research is to develop a preventive care program by predicting the occurrence of delirium among the elderly patients in acute care setting.
    In the first stage of the research, a concept framework for the risk factors of delirium at an early stage of hospitalization was created, and this framework was used to investigate the relationship between the risk factors of delirium and the development of delirium in 97 elderly patients. The results showed significant correlations between the development of delirium and the following nine items : age>85,impaired cognitive function, previous episodes of delirium, leukocytosis, drinking habit, physical immobility, fasting, dissatisfaction with hospital treatment, and disorientation.
    Using these results in conjunction with a literature review of the previous studies on delirium intervention, the framework of a delirium preventive care program for acute-stage elderly patients was examined and a proposal was created. The preventive care program comprises a series of processes, including selection of subjects, assessment upon hospitalization, planning and implementation of an intervention program, and evaluation.
    Further study to evaluate the validity and effects of the program will be conducted by applying the delirium preventive care program created in the present study to elderly patients in clinical situations.

    (1)The approach of public health research did not work well for this research topic, and then. we used the recruitment strategies and more effective with Japanese women than non-Japanese women. The response rates showed high.
    (2)The percentage of good for self-awareness of health was 75%, but the percentage of suggested interventions based on SMI score was 52% in Japan study.
    (3)The rates of self-check for bone density and self breast examinations were lower than annual health check-ups. WHOQOL-BREF indicated that the women reported less satisfaction in the physical domain than other domains. They may need to reduce nervimotor symptoms, and to improve physical quality of life.
    (4)To obtain the support in the self-care questionnaire was related all domains of WHOQOL.
    (5)The experience of Domestic Violence (DV) decreased the score of all domains of WHOQOL.
    (6)Women's health among Japanese-American may be influenced of social and cultural factors and the American health care system upon menopausal experiences.
    (7)QOL scores and self-awareness of health were significantly different in the three groups ; Japanese women in Japan, Japanese-American women in San Diego, and American Women in San Diego. QOL scores showed American group was higher, Japanese-American group was middle and Japanese group was lower. The percentage of excellent for self-awareness of health was 34.6% (American group), 14.6% (Japanese-American group), and 5.3% (Japanese group). On the other hand, there was no statistically significant difference in SMI scores among the groups, but SMI-10symptoms' patterns were different.
    (8)A comparison of the three groups showed significantly different QOL scores and self-awareness of health even though members of each group experienced similar levels of menopause-related symptoms. The findings that cultural attitudes to menopause influence the percentage of quality of life.
    (9)The nursing guidance clinic started from May 2004 and open on Saturday between 2 to 4 pm twice a month at a citizen's plaza. The purposes of the nursing guidance clinic in Joetsu city are to empower and encourage the clients and the community through corresponds to the health promotion needs, and to create the nurses' network among nursing faculties nursing students and nurses in the community. Nursing guidance, health measurement (bone density, resting energy expenditure, body weight and fat, blood pressure), health diary and information, referral services were provided. The clients can choose the services free of charge. We examined the feasibility of the new, nursing care provide system in the community. Community-based participatory research was introduced and the nursing guidance clinic was opened fifteen times in the first year and 211 clients used. Adhesive program development related to outcome would be the second year's agenda.

    痴呆高齢者のせん妄の発症過程を明らかにすることを目的とし、平成14年度は介護老人保健施設の高齢者を対象に入所後数日間の睡眠・覚醒周期への影響をとらえた。これに引き続き、平成15年度は療養型病床群の入院高齢者を対象にデータ収集を行った。
    対象はN病院の療養型病床群のアセスメント病棟に入院した高齢者6名(男性2名、女性4名)である。平均年齢は81.3歳であり、介護度2と3が各2名、介護度4と5が1名ずつであった。対象は高血圧、糖尿病、骨折、腎不全、リウマチなど複数の疾患を有しており、老人性痴呆の診断を有する者は2名含まれた。せん妄評価尺度(DRS)の点数は0から8点に分布し、入院から3日目までの間にDRS12点以上(せん妄と診断)になる者はいなかった。結果的にせん妄となった高齢者が含まれなかったため、療養型病床群でのせん妄の発症過程を明らかにするまでには至らなかった。
    しかしながら高齢者のデータ収集と同時に実施した受持ち看護師に対する対象高齢者のせん妄発症の予測とその理由についてのインタビューデータから、療養型病床群の看護師が高齢者のせん妄のアセスメントで注目しているデータが明らかになった。注目データを大別すると、入院時の高齢者の様子、入院前の生活と入院環境とのギャップ、せん妄の既往の3つに分類された。入院時の様子は、見当識障害、意味不明の言動、表情が固い・乏しい、落ち着きがない等から判断していた。生活環境のギャップは、本人の希望しない入院、ADL能力の誤認知覚、入院前の夜間不眠の有無などから入院による高齢者のストレス荷重状況をアセスメントし判断していた。これらは、せん妄を予測し適切な予防ケアを実施する上で重要な情報であり、今後のせん妄アセスメントツール開発の基礎資料として役立つと考える。

    1) Purpose : Their are few researches on passing away experience of families elderly members. So, we would like to show clearly the process of accepting death of the terminal aged people at a long-term care hospital and of their families.
    We would also like to clarify the factors of their death and study the family care of the terminal period of the aged.
    2) Method : We asked 14families who experienced their member's dying time.
    We analyzed the quality, too.
    3) Conclusion & Consideration
    From now on we will focus on the period of being in the hospital and the dying time. The family perceives the elder's sign of growing weaker getting clues of the doctor's explanation and nurse's action. They get information on the condition of the disease periodically and seem to accept of the being swayed looking at nurses' during the time of being hospitalized and the dying time.
    We can guess the family's condition at the dying time that they earnestly desire him or her to sleep peacefully. So we consider that they need a guarantee of the dying person's comfort. We understood the family's need and were suggested the necessity of our warm consideration at the time of dying

    平成12年度に実施した全国の介護老人保健施設の看護婦・士に対する調査結果をふまえ,平成13年度は,介護老人保健施設の看護婦が,入所者のせん妄の兆候や症状をどのように把握し,看護ケアに結び付けているのか,その判断過程を明らかにする目的で看護婦への面接調査を実施した.
    対象は,道内の介護老人保健施設に勤務する看護婦であり,最近3ヶ月以内に経験したせん妄状態の痴呆高齢者の事例について,インタビューを行った.内容は,(1)その入所者のせん妄の兆候や症状の内容,(2)いつ,せん妄状態であると気付いたか,(3)把握した症状などをどのように解釈して看護ケアの方法を選択したか,(4)実施した看護ケアおよび治療は何か,(5)治療および看護ケアに対して入所者はどのように反応したかである.インタビューは,各々1時間から2時間を1回実施し,許可を得てテープレコーダーに録音した.録音したテープから逐語録を作成し,グランデッド・セオリー・アプローチを参考に分析を行った.
    分析の結果,入所者の環境の変化に伴い一時的な混乱状態に陥った事例が多く語られた.看護婦は,入所者の心ここにあらずの様子,不明瞭な会話,錯視,険しい表情,「家に帰る」と言う事などを,せん妄に気付く兆候としてあげた,これらは,平成12年度に実施した調査結果と同様の内容であった.看護婦はこれらの症状を環境の変化に対する戸惑い,緊張,ストレスと捉えていた.実施したケアは,対象との関わりを多く持つ,離床を促す,本人のニーズをこちらから確認して満たす,薬の調整などであった.これらのケアによる入所者の反応として,落ち着く,満足な様子などが挙げられた.
    今後は,さらに例数を重ねつつ詳細な分析を継続し,看護婦の判断過程を明らかにしていく予定である.
