    Objective Connectedness is essential for maintaining the physical and mental health of older adults. However, measurements that consider the presence of community-dwelling people and explicitly focus on an individual's subjective sense of connectedness have not yet been established. This pilot study aimed to develop a draft scale that comprehensively measured older adults' perceptions of community-based connectedness with people.Methods The scale development comprised three phases; item review, pre-test, and field verification. Phase 1 (item review) involved creating an item pool and evaluating content validity. Phase 2 (pretest) involved evaluating face validity and conducting item analysis on the preliminary version of the scale. Phase 3 (field verification) involved distributing the preliminary scale to 800 Japanese men and women aged ≥ 65 years residing in Asahikawa, Hokkaido, Japan. Validity was assessed by factorial validity using exploratory factor analysis and concurrent validity using correlation analysis. Reliability was confirmed by Cronbach's α coefficient using the internal consistency method.Results The evaluation of content validity in Phase 1 and face validity in Phase 2 yielded 30 items. In phase 3, of the 800 questionnaires that were distributed, 343 were returned, of which 309 included responses to all items and were analyzed. Exploratory factor analysis resulted in a 22-item scale comprising three factors that assessed perceptions of community-based connectedness. The Cronbach's α for the total scale was 0.967, and the α coefficients for inclusion, reciprocity through provision, and reciprocity through reception were 0.941, 0.915, and 0.928, respectively. The total scale scores were significantly positively correlated with purpose in life (rs = .453, P < .001) and negatively correlated with loneliness (rs = -.307, P < .001).Conclusion The 22-item draft scale exhibited adequate reliability and validity. Perceptions of connectedness measured by this scale can be used to support public healthcare interventions for community-dwelling older adults.

    OBJECTIVE: To clarify the circumstances of help-seeking preference among older adults in depopulated areas and the relationship between help-seeking preference and social support. METHODS: This was a cross-sectional study that included a sample of 271 men and women aged 65-79 years living in Japan. A questionnaire survey was conducted to assess help-seeking preference, social cohesion (community trust, norms of reciprocity and community attachment), socialising (interaction with neighbors and social network) and social support. Multiple regression analysis was performed with items that were found to be significantly associated with social support in the univariate analysis. Then, we examined the significance of the change in the coefficient of determination between a model that included help-seeking preference as an independent variable and a model that did not. RESULTS: Social support was related to help-seeking preference, living arrangements, economic status, norms of reciprocity and social networks. There was a significant difference in the coefficient of determination between the models with and without help-seeking preference. CONCLUSIONS: For older adults in depopulated areas to obtain social support, it is important to build trusting relationships with others and shows a willingness to seek and accept help.

    Given the rapid increase in health information available, health literacy is now more important than ever when searching for health information online. Furthermore, health literacy and online health information searching are reported to be associated with support from others. In this study, we examined the associations of health literacy and social support with online health information searching among individuals in their 50s and 60s. Between March and May 2022, an anonymous self-administered questionnaire was administered among 750 middle-aged and older adults (age 50-69) living in northern Japan. The questionnaire items included personal characteristics, frequency of online health information searching, health literacy domains, and social support. A total of 237 respondents answered all the items (response rate: 31.6%). Multiple logistic regression analysis revealed that communicative health literacy was significantly associated with searching for information about illnesses and health promotion, while critical health literacy was significantly associated with searching for information about medical institutions. Controlling for personal characteristics, health literacy, was associated with online health information searching, suggesting that improving health literacy is important in a digital and information society.

    Abstract

    Aim

    We developed a self‐assessment scale—Older Adults' Perceptions of Community‐based Connectedness with People—to assess older adults' comprehensive perceptions of their connectedness with others in the community. A specific aim of this study is to evaluate the reliability and validity of this scale.

    Methods

    Participants consisted of 1000 men and women aged 65 years or older, living in Sapporo, Hokkaido, Japan. Factorial validity was assessed using exploratory and confirmatory factor analysis, while concurrent validity was assessed using correlation analysis. Reliability was confirmed by Cronbach's α coefficient using the internal consistency method, and the stability coefficient was confirmed using the test–retest method.

    Results

    Responses were received from 380 participants, and 358 participants who responded to all items were included in the analysis. The developed scale comprised 22 items with three factors: “Perception of Inclusion” (α = .947), “Perception of Reciprocity through Reception” (α = .937), and “Perception of Reciprocity through Provision” (α = .910). Correlation analyses indicated that concurrent scales were positively correlated with Ikigai and negatively correlated with loneliness on the total scale. The model fit was comparative fit index = 0.933, goodness‐of‐fit index = 0.854, adjusted goodness‐of‐fit index = 0.818, and root mean square of approximation = 0.081. The stability coefficient of the total scale scores was 0.875 (95% CI: [0.830, 0.908]).

    Conclusions

    The developed scale had adequate reliability and validity. The perceptions of connectedness measured using this scale can be used by public health and nursing care professionals to prevent loneliness and isolation among older adults living in the community.

    Abstract

    Background

    Frailty is a syndrome related to declining functioning attributed to aging. As a reversible, often improved condition, preventive measures for frailty can be effective.

    Objective

    This study aimed to evaluate a frailty prevention program for older men comprising face‐to‐face meetings and an original social mobile application. We also examined the effect of COVID‐19‐associated social isolation on the implementation of this program.

    Methods

    We conducted a mixed‐methods pilot study featuring a single group before and after comparison of 11 man participants, ≥65 years, living in Japanese cities with a spouse or relatives. Program outcome measures included frailty, loneliness, grip strength, step count and mental health status. Process evaluation considered the experience of participants through a semi‐structured interview.

    Results

    The intervention period included an interval of self‐isolation owing to the spread of COVID‐19. Among the outcomes, grip strength decreased after intervention. No significant negative changes were observed in other items. Themes emerging from participant experiences with the program included ‘Cooperation with researchers and contributing to research’, ‘Greater awareness of health’, ‘Interest in and experience of “newness”’, ‘Hesitation over active engagement’, ‘Casual ties with members’ and ‘Relationships that do not breach personal boundaries’.

    Conclusions

    Although the COVID‐19 pandemic overlapped with the intervention, participants maintained their mental health. The program was a novel, stimulating experience for older men and demonstrated the usefulness of information and communication technology in supporting their social activities.

    Implications for practice

    As a community health nursing initiative for older people, we propose activities that combine group and online support. Frailty prevention programs for older men must contain content that stimulates their interests, such as ‘health’ and ‘newness’.

    Abstract

    Aim

    This study aimed to clarify the concept of older people's connectedness with people in the community within the literature.

    Methods

    This study was based on Walker and Avant's concept analysis method. We used PubMed, MEDLINE, CINAHL, and Web of Science databases and included peer‐reviewed articles that were published in English between January 2010 and April 2023. After assessing eligibility and applying exclusion criteria, 25 articles were analyzed.

    Results

    Attributes included sense of belonging to the community achieved through interactions and feeling of togetherness through interrelationships, which consequently led to the maintenance of healthy physical and mental states and independent living, the acquisition of the ability to overcome difficulties, and the creation of a sense of meaning in life through coexistence.

    Conclusion

    We identified the concept of older people's connectedness with people in the community in a contemporary context. Connectedness is a useful concept to consider in nursing care because it seems to evolve into comprehensive health, including the physical and psychosocial health of older adults.

    The spread of COVID-19 is considered to have strengthened people’s awareness of others. Additionally, the COVID-19 pandemic has reduced connection with others among older adults and increased loneliness. This study aimed to investigate the factors affecting loneliness among older adults in rural areas during the COVID-19 pandemic by focusing on the connection with others. The target group included 932 rural Japanese adults, aged 65–74 years. An anonymous, self-administered questionnaire survey was conducted. Valid responses were obtained from 405 participants (valid response rate: 43.5%). A multiple regression analysis was performed using the forced entry method with loneliness as the dependent variable. The independent variables were those showing significant associations with loneliness based on the univariate analysis. Sex (β = −0.139), economic situation (β = −0.103), neighborhood ties (β = −0.260), independent view of self (β = −0.213), interdependent view of self (β = 0.171), and communication through phone (β = −0.128) were significantly associated with loneliness. Connection with others and subjective views of the relationship between self and others were associated with loneliness in situations where one was more aware of the behavior of oneself and others in an infectious disease pandemic.

    Ikigai is a unique Japanese concept that encompasses not only joy and happiness, but also the purpose and meaning of life. The purpose of this study was to explore the factors that contribute to Ikigai, an important concept in the health of older women, and to clarify the relationship between Ikigai and the enhancement of the social aspects of their health. The participants in this longitudinal study, conducted between October 2017 and February 2020, were physically healthy older women aged 65 years and above (N = 132). Physical function and social activity were used as predictors of Ikigai. In addition, willingness for new interactions was used to examine the social aspects of health. A causal model was created from these factors, and path analysis was performed. The results demonstrated that participating in numerous interpersonal activities was predictive of increased Ikigai, but physical function was not. In addition, Ikigai was found to affect the willingness for new interactions. Further, Ikigai could be related to the willingness for new interactions. To enhance the social aspects of older women’s health, it is necessary to focus on Ikigai.

    Previous studies have indicated that older men often experience disconnection from the community after retirement. Social activities have been shown to be effective in preventing social isolation among older urban men. Nevertheless, it has been reported that they often do not participate in community social activities and tend to be reluctant to do so. We explored the values and meanings of social activities for retired older men living in an urban area of Japan to understand support using social activities that are more suitable for them. Semi-structured interviews were conducted with 15 older men (aged 68–80 years; M = 74.6 ± 3.79 years) about their interactions with family and non-family members, and their participation in various community social activities. The grounded theory approach was used for the analysis. As a result, the following five categories were derived as the values that participants place on the social activities that they engage in: “health as a resource and reward for social activities,” “feeling I am still useful,” “feeling that something is my responsibility,” “feeling of time well spent,” and “finding interest through interactions.” In addition, the following three categories were extracted as meanings of social activities: “fulfilling social life,” “maintaining stable family relationships,” and “maintaining safety and peace in the community.” When considering the social activities that older urban retired men are interested in and likely to participate in, these five values can be considered indicators. In contrast, to maintain stable family relationships and safety and peace in the community, participants sometimes used strategies to stop or abandon social activities. Therefore, in situations where a peaceful life within a family or neighborhood is threatened, it may be useful to help set aside sufficient time and allow for psychological leeway in advance to incorporate social activities into their lives.

    The coronavirus disease 2019 (COVID-19) pandemic has exposed older adults to health and social risks. This study examined the perceptions of community-dwelling older adults regarding how COVID-19 restricted their daily lives. Six focus-group interviews were conducted with 24 participants (mean age, 78.2 ± 5.5 years) living in urban and rural areas in Japan. Then, a qualitative inductive content analysis was performed. Six themes were generated: “fear of infection and public, watchful eyes,” “consistency in daily personal life,” “pain from reducing my social life,” “readiness to endure a restricted life,” “awareness of positive changes in myself,” and “concern for a languishing society.” There was no change that would make their lives untenable, and they continued their daily personal lives at a minimum level. However, their social lives were reduced, which over the long term can lead to a lost sense of purpose in life. This was reported as an adverse factor in the development of other diseases and functional decline in previous studies. While there is no doubt that infection prevention is important, supporting older adults in engaging in activities that provide a sense of purpose in life could contribute to their present and future overall health including mental health.

    Aim: This study aims to develop the Social Activities Scale for Community-Dwelling Older People Requiring Support (SASOS). Design: This study is a cross-sectional investigation. Methods: The participants were healthy older people (HOP; N = 140) and those requiring support (OPRS; N = 250). An anonymous questionnaire included items on SASOS, subjective health and ikigai (Japanese well-being concept). Criterion-related validity was examined using the Social Activity Index for Elderly People (SAI-E). Results: Seventy-five HOP and 157 OPRS provided effective responses. The scale (α = 0.805) had three subscales: "interactions with friends and neighbors (F1)," "close relationships with family (F2)" and "interactions with others through activity programs (F3)." SASOS and SAI-E scores were correlated (r = .558, p < .01), indicating criterion-related validity. In known-groups validity analysis, F1 were significantly higher among HOP and F3 were significantly higher among OPRS. Total scores correlated with perceived health (r = .240, p < .01) and ikigai (r = .419, p < .01).

    OBJECTIVES: This study examines the structure of the process that public health professionals (PHPs) use to organize preventive care groups for older adults and the elements that strengthen this process. DESIGN AND SAMPLE: The study was conducted using a quantitative descriptive design. Anonymous self-administered questionnaires were distributed by mail to 919 PHPs, including nurses and social workers employed by local governments in a Japanese prefecture, who facilitated recreational groups for older adults for the purposes of preventive care. Measures Items related to the process and the awareness of support were based on previous research. The process structure was examined using exploratory factor analysis, while multiple logistic regression analysis was used to study strengthening elements. RESULTS: The process yielded six factors (encouraging clarity with respect to the group's activity policy; creating connections with other resources; fostering independence; encouraging activity evaluation; creating relationships with group members; understanding the strengths and weaknesses of communities and individuals) with a total of 23 items. Two of three indicators of awareness of support were significantly related to the process. CONCLUSIONS: Understanding the importance of strengthening elements might improve support groups for older adults.

    The meaning of participation in care prevention group activities may encourage continuous participation, making older adults active and healthy throughout their lives. This study developed a scale to assess the meaning of participation in care prevention group activities. It involved 427 participants in care prevention group activities (CPGAs) in Japan who filled out a self-administered questionnaire between October 2017 and February 2018. The meaning of participation was assessed using 15 items. In total, there were 379 valid responses. A factor analysis yielded two factors: “promotion of self-growth” and “enrichment of daily life”. The goodness of fit index (GFI), comparative fit index (CFI), and root mean square error of approximation (RMSEA) were satisfactory (GFI = 0.923; CFI = 0.960; RMSEA = 0.073). Cronbach’s α was 0.939 for the entire scale. The scale scores were significantly correlated with scores of the social activity-related daily life satisfaction scale and Ikigai-9. The scale’s reliability and validity were confirmed, indicating its usability for promoting care prevention efforts.

    OBJECTIVES: This study aims to develop a scale that can comprehensively measure public health nurses'(PHNs) career development and to verify the validity and reliability of that scale. DESIGN AND SAMPLES: This study is a cross-sectional investigation. The participants were 1,009 PHNs working in a government facility in Japan. A total of 586 participants who had been working for more than 3 years as PHNs were analyzed. MEASUREMENTS: Anonymous self-response questionnaires included items on professional awareness, practical competency, and job satisfaction. We conducted exploratory factor analysis and confirmatory factor analysis to clarify the construct validity of the scale. The reliability was examined by the Cronbach's α coefficient. The validity was examined by an external reference relevant validity and the known-group method. RESULTS: The scale was structured with 32 items covering three factors: "community activities, policy, and management," "PHN identity," and "foundations as a PHN." The Cronbach's α coefficients were all greater than 0.9. In confirmatory factor analysis, the scale showed acceptable goodness of fit. CONCLUSIONS: The comprehensive career development scale for PHNs proved its reliability and validity. This scale may be useful to promote PHN identity and competency comprehensively in basic education and in-charge education.

    BACKGROUND: Parents often struggle against the negativism of 3-year-old children in exerting parental control. Although most research on parental control is from western cultures, Japanese mothers may face unique challenges because of their sociocultural background. We therefore explored mothers' challenges in exerting parental control over 3-year-old children in Japan. METHODS: A descriptive qualitative approach was adopted. The participants were 10 Japanese mothers with children aged 3-4 years. We conducted individual semistructured interviews and analysed the data via qualitative content analysis. RESULTS: We found four categories and a structure of the challenges mothers faced in exerting parental control. Mothers' control attempts had four main intentions, each with a different subject (i.e., the children, themselves, the family, and society). Most challenges with parental control stemmed from mothers' "consciousness of parental responsibility." In confronting the child in their attempts to exert parental control, mothers perceived a "struggle with the child's negativism," which led them to recognize that they could not find a correct method of discipline. Furthermore, in the face of conflicting roles, mothers experienced "confusion in seeking the righteous answer as a parent." Some mothers began to feel that they were discovering their maternal role in their own way rather than finding a correct answer, which formed a "conviction that my own parenting is not incorrect." CONCLUSIONS: The challenges mothers faced in exerting parental control over their 3-year-old children served as a turning point for them to reflect on themselves as parents. However, due to societal monitoring of parents and mothers' multiple roles in the family, it can be difficult for mothers to fully overcome the challenges of parental control. It is necessary to foster social capital in the community to which the mother belongs and to create an atmosphere in which community members can watch over parents warmly.

    Objectives The purpose of this study was to analyze the social activities of older men who require daily support, and to clarify the purpose of such activities, in order to develop effective living support and preventive long-term care service, suitable for this population.Methods Individual, semi-structured interviews were conducted with 17 older men. Data were analyzed using inductive and qualitative methods.Results Four categories of social activities were identified, and four categories of purposes of these social activities were extracted. The following were the identified social activities: maintenance of "comfortable relationships with others," including family, relatives, friends, and neighbors; "participation and use of services and programs with clear objectives and relationships with others," such as long-term care insurance system services, clubs for the elderly, and hobby groups; maintenance of "relationships with former colleagues, depending on their experience of working with them," where some individuals actively participated in gatherings with former colleagues, while others did not keep in touch at all; and participation in "activities to enrich their feelings and quality of life within their living space," such as reading, watching TV, and doing household chores. The purposes of the observed social activities were to build "relationships with society through communication with other people" and to have a "sense of security by spending time with people of the same age and with those older than them." Hence, participants engaged in clubs for the elderly, as well as in hobby groups. In addition, participants made time for exercising regularly, which maintained their cognitive function and was intended for the "maintenance and activation of their physical functions by continuing to exercise," and "continuing to learn by thinking." Furthermore, participants engaged in the exercise or hobby groups that they were interested in, in order to "utilize their time in a meaningful way" that lead to pleasure and enjoyment.Conclusion The following were the characteristics of the observed social activities: (1) the activities helped participants to maintain relationships with their coworkers, (2) participants had comfortable relationships with others, with these relationships exhibiting different levels (i.e. intimate relationships or casual), and (3) participants actively followed current events. They have clear purpose join and participate in social activities. In addition, it is suggested that this population engage in social activities to enrich physical function and overall quality of life.

    Purpose : The purpose of this study was to investigate diffidence in proficient public health nurses who conduct self-evaluations of community-focused nursing. Methods : Subjects were seven public health nurses with 6-10 years of experience at prefectural health care centers and were also experienced in developing community-focused nursing for those suffering from mental disorders based on individual support. The data were collected using semi-structured interviews. Diffidence is defined as "a feeling mainly occurring due to self-consciousness and insecurity wherein one does not trust or respect their own judgment, competence, and skill, leading to low self-esteem." In addition, qualitative, inductive analyses were conducted. Results : Based on the data, even the proficient public health nurses working at health care centers and building networks and organizations for mental disorder patients were found to be diffident. Diffidence included "worry and anxiety about collaborating with health care, medical and welfare workers," "difficulty in building a network among health care, medical and welfare workers," "difficulty in maintaining organization for those concerned about patients with mental disorders," "strain and anxiety of developing mental health nursing in the community" and "pain and anxiety in smoothly supporting individual cases." Discussion : We consider that the diffidence of the proficient public health nurses was self-questioning about their judgments and actions from the perspective of ideal competence and proficiency. Moreover, the diffidence of these proficient public health nurses appeared to be a self-evaluation of their judgments and actions with reflection about how to provide better support. It was suggested that it is important that the proficient public health nurses evaluate their own competence adequately, and that the support organization of chief nurses and colleagues should provide guidance and advice to these proficient but diffident nurses.

    This study aimed to verify the effectiveness of an application (app) in establishing social connectedness among unacquainted older men, as well as improving their physical health. The nine participants were men aged 65 and older in the subarctic zone of Hokkaido, Japan. A mix of quantitative and qualitative methods were adopted as the study design. A questionnaire survey was completed before and after the intervention, and a semi-structured interview was conducted after the intervention. An app-installed smartphone was loaned to the participants, which allowed them to share pictures, voice recordings, and their step count. Quantitative data were analyzed using descriptive statistics and the Wilcoxon signed-rank test, and qualitative data were analyzed using qualitative descriptive analysis to generate categories. The average age of the participants was 77.7 years. The relationship between participants who were interacting for the first time through the app advanced as their understanding of each other’s personalities deepened. The average step count during the third and fourth months was significantly higher than in the first two months. By using the app, older men were able to build relationships with one another. In addition, visualizing the number of steps on the app was effective in improving the number of steps.

    <jats:p>Objective: To identify future issues in the development and implementation of support programs for fathers with the aim of preventing child abuse, we reviewed the domestic literature to determine the characteristics of support for fathers during pregnancy and identify research trends.Methods: The domestic literature was searched on January 31, 2021, using the Central Journal of Medicine ver. 5.0. The search yielded 110 original articles and nursing literature using the keywords “pregnancy & father” and “pregnancy & husband,” wherein 100 articles with abstracts were found, excluding conference proceedings. The abstracts were then checked, and finally, 27 were included in this analysis.Results: In the articles collected, the following themes were extracted: “satisfaction from the experience of child-rearing skills from pregnancy,” “relationship between marital intimacy and relationship to mother and child from pregnancy,” “role adjustment between work and home from pregnancy,” and “vague imagination and anxiety from pregnancy.”Conclusion: During pregnancy, the deepening of the relationship between the mother and father, who is still adjusting to become a father, can affect the quality of the relationship between the mother and the child. Thus, the challenge is to develop support programs that will further strengthen the relationship between the husband and wife.</jats:p>

    This study elucidated the skills of mothers who used to build positive interpersonal relationships with people they met through child-rearing. The research method was qualitative descriptive study. The participants were 24 mothers who had children between ages of one and four years. These participants were recruited at child-rearing salons and childcare centers located in various municipalities in Japan. The survey period was from September 2013 to July 2014. The mothers' interpersonal relationship-building skills were described by six categories: "ability to strive for new encounters," "ability to try to interact socially with others," "ability to choose compatible people," "ability to continuously maintain good relationships," "ability to take action suitable to a situation," and "ability to build positive relationships with parents-in-law." Cognitive aspects of assessing and understanding the interactions that occur during exchanges and behavioral aspects guided by these cognitions were identified within these skills. This study contributed to providing a framework to understand mothers' interpersonal relationship-building skills for public health nurses involved in child-rearing support.

    今年度は、公衆衛生看護技術の体系化に向けて多面的に検討を行った。1.保健師との活動の類似点が多い社会福祉士の分野における技術の構成を教科書等から分析し、公衆衛生看護学の技術の体系化の参考になる点を検討した。公衆衛生看護学の理念の前提および理念の技術化を図った。理念型技術の前提となる理念は、人を対象とする分野に共通する「人権尊重」を置き、理念として「社会的公正」と「Health for All」を公衆衛生看護で重視する理念として設定した。2.公衆衛生看護学の教科書分析を行い、基礎教育における公衆衛生看護学の技術体系の実態を明らかにし、教科書等の分析では抽出が困難な技術についての課題を検討した。3.ミネソタモデルの技術の中から「Referral and follow up」「Counseling」「Health teaching」「Screening」「Case finding」について目的と技術について検討した。
    公衆衛生看護技術の体系化の枠組みとして、4つの理念型技術および19の理念型実践の中核型技術の概念を抽出した。さらに、公衆衛生看護の視点(対象の理解、健康の意味付け、モノの見方)、実践の基盤(思考技術、コミュニケーション技術、プロセス展開、デジタル技術)、アセスメント(健康課題の特性と状況分析)、関係構築、実践/介入/支援(エコシステム別対象支援技術、平時と危機時の技術など)の要素を抽出した。公衆衛生看護技術を理念を含めてとらえることで、活動の目的を明確に意図することができる。これによって、How toの技術から、Why/Whatを意識した技術の行使と活動の転換が図られることが期待できる。

    本研究は健康な高齢者と要支援高齢者の両者を対象に、フレイル予防のための社会活動プログラムを開発し、その効果を検証することを目的とする。各対象の心身機能や社会活動の特徴にあわせた社会活動プログラムを開発し、プログラムを通じて社会とのつながりを意味づけられるよう支援を行う。また、両対象の交流を可能にする内容もプログラムに組み込む。さらに、ICTを活用し「活動の場でのつきあい」を「地域における日々のつながり」へと本プログラムを発展させ、社会活動の維持ならびにソーシャルキャピタルの醸成を図る。
    2018年度はプログラム開発に向け、都市部および地方の高齢者約30名を対象にインタビュー調査を実施し、質的分析にて社会活動の実態と社会活動に対する価値等を抽出した。その結果をもとに地域に住む高齢者の実情にあわせた社会活動プログラム案を作成した。
    また、約10名の高齢者を対象に、ICTに関するインタビュー調査を実施した。高齢者のICTに対するニーズや生活に対するニーズを質的分析にて明らかにし、高齢者が活用できるICTの1つとして、スマートフォン用の交流アプリケーションを開発した。

    An interview survey was conducted to assess the social activities of older men who require daily support and the purpose of these activities. As a result, four categories of social activities were identified, and five categories of purposes of the social activities were extracted. Based on these qualitative results, a social activity scale for older men requiring support was developed. The previously developed social activity scale for older women requiring support was integrated with the scale developed in this study. As a result, a social activity scale for older adults requiring support useful for long-term care service, was developed.

    The purpose of this study was to develop a scale to measure the social activities of elderly women requiring support. The first step for study describes how items for a social activities scale proposal were created with a pilot study. An anonymous questionnaire was carried out through individual interviews with elderly women requiring support. The three factors of “interaction with surroundings through consideration”, “consulting with professionals”, and “proactive creative activities at home” were extracted as a social activities scale for elderly women requiring support from the results of the exploratory factor analysis. In the second step of study, to verify the reliability andvalidity of the social activities scale for elderly women requiring support, an anonymous questionnaire survey was conducted for elderly women who require daily support and elderly women. An exploratory factor analysis was performed based on the responses of the elderly women requiring support, and the three-factor structure postulated in the pilot study,as well as an internal consistency of α=0.859, were established.

    The health policy curriculum followed in the public health nursing education program was developed to unify the results of interviews of administrators and beginner public health nurses, investigation of the university faculty, and review of the relevant literature. The curriculum was based on personal support competence and communication competence. The educational process regarding health policy included 5 steps. The first step was to understand the structure of the health and welfare system. The next steps were to perform continuous analysis of community health issues, develop health programs, and understand the health measure process.     早期前立腺がんは、積極的経過観察、手術療法、放射線療法など年齢、病期、リスク分類に応じて治療の選択肢が複数あり、患者はそれぞれのリスクとベネフィットを考慮しながら治療を選択していくことが求められる。特に、前立腺がんは、根治治療後の長期生存期間が得られることから、がんサバイバーの治療後の満足を改善することは重要な課題である。
    初年度は、前立腺がん患者が治療を意思決定していく上でのアンメットニーズを把握するために、2つの調査を計画していった。まずひとつには、前立腺がん患者を対象とした調査である。この調査では、前立腺がんに対する根治治療の意思決定のプロセス、治療長期経過後の後悔、現在のHealth-related quality of life (HRQOL) についてPatient-reported outcome (PRO)を用いて評価し、これらの関連性を明らかにすることを目的とした。398名の方より回答を得ることができた。もうひとつの調査は、北海道内の地域がん診療拠点病院の泌尿器科および放射線科で勤務している看護師(病棟および外来)を対象とした調査である。この調査では、看護師による患者へのセルフケア支援の実際、外来における治療選択の意思決定支援の実際を質問紙を用いて尋ねた。また支援をする上での難しさについては自由記載でも回答を得た。95名の方より回答を得ることができた。
    今後はこれらの2つの調査で得られたデータを用いて詳細な分析を進めていく予定である。

    The aim of this study is to clarify the image and skill of public health nurses (PHN) those are educated in the Master’s courses. The common factors in the image of PHN were Research, Global viewpoint and Leadership in the future. Those skills were consisted with ‘practical research skill’, ‘basic intention as a PHN’, ‘skill to manage individuals and families’, ‘skill to manage communities’, ‘skill at the system level’, ‘skill of risk management’ and ‘skill to consider the individuals and the families, communities and the systems’.     This study aims to develop a comprehensive career development scale for public health nurses working at local government. First study, exploratory factor analysis was conducted using 62 items, and a career development scale was consisted with 3 factors comprising of 32 items. Second study, exploratory factor analysis was conducted using 42 items. Finally a career development scale was consisted with 4 factors (“community activities, policy making and management” 9 items, “foundations as a public health nurse” 6 items, “identity as public health nurse” 5 items and “job satisfaction” 4 items). The Cronbach α coefficient was .943 for all items, and .924~.851 for each factor. As the Cronbach α coefficient showed high, the reliability of a comprehensive career development scale for public health nurses was confirmed.

    We analyzed data from cases as told to us through a semi-structured interview with public health nurses who had been in charge of services for child abuse, and review of domestic and international research papers on the characteristics of fathers regarding child abuse and cases report meeting of infants abuse.From the all these data, the characteristics of fathers who commit child abuse werethe difficulty to build relationships with others includes emotional family relationship, and to express emotion and characteristics of selfish behaviors. The findings suggest the necessity for expert public health nurses to understand fathers, to be aware of the difficulty of building a supportive relationship with fathers, and to improve skills to help fathers form a good relationship with family members.
