STUDY DESIGN: This is a basic science research. INTRODUCTION: Isolating excursion of the flexor digitorum profundus (FDP) in zones I and II is common practice in the current management after flexor tendon repair. During this procedure, the proximal interphalangeal joint is sometimes fully extended with unmeasured external forces at the middle phalanx when the distal interphalangeal joint is actively flexed. PURPOSE OF THE STUDY: The purpose of the study was to investigate the incremental effect of external force with palmar blocking versus lateral blocking and increased angles of flexion on internal tendon forces at the repair site for a safer application of force by the treating therapist. METHODS: Eight human cadaveric fingers were studied. To simulate palmar or lateral finger blocking, a compression force of blocking was applied from 5N (510 grams) to 25N (2,550 grams) on the skin surface of the palmar or the lateral aspect of each of these middle phalanges in 5N increments. The tensile load on the FDP tendon during distal interphalangeal joint flexion from 0° to 60° was measured in 10° increments. RESULTS: During palmar blocking, the tensile load was significantly increased with increases in palmar blocking force. However, no significant increase in the tensile load on the FDP tendon was observed at any lateral blocking. DISCUSSION: Lateral blocking exercise can be performed with less tensile force on the FDP tendon when performing blocking exercise after flexor tendon injury repair. CONCLUSIONS: This study supports the concept that lateral blocking with incremental joint angles allows a safer application of force for the healing tendon.

手術治療、リハビリテーションを実施した神経損傷・血管損傷患者82名(男性66名、女性16名、13〜81歳)を対象とした。CISS質問票スコア(CISS)と年齢には弱い相関があり、DASH、Hand20(ハンドフロンティア)とはかなり相関があった。術後経過日数とは相関はなかった。CISSが30点を上回るものは38例(46%)で、50点を上回るものは15例(18%)であった。評価時期が厳寒期では温暖期よりもCISSが有意に高かった。術式(受傷形態・部位)のCISS中央値は再接着(完全切断)29.5、再接着(不全切断)24.0、神経縫合(指)22.5、神経縫合(手部)28.5、断端形成術46.0、皮弁59.0であった。受傷部位別の中央値は、単指損傷28.0、複数指損傷30.0、手部損傷15.0で、受傷形態・術式、受傷部位で統計学的差はなかった。

指節骨および中手骨骨折に対し手術治療を行った51例59指(男性41例、女性10例、9〜87歳)を対象とした。Cold Intolerance Symptom Severity(CISS)、DASH、Hand20の各スコアは、それぞれ14.4±19.0、15.4±17.1、21.2±22.5であった。CISSが30を上回る症例は10例(約20%)、50を上回る症例は5例(約10%)で、10未満が最も多く、77が最高であった。CISSとDASH、Hand20、%total active motion(%TAM)、年齢のそれぞれに有意な相関があり、術後経過日数に相関はなかった。CISSと性別、受傷時期、回答時期に有意差はなかった。CISSと骨折形態、骨折箇所、腱損傷に有意差はなかった。CISSと損傷指、骨折部位、固定材の違いの全ての項目に有意差はなかった。

指尖部損傷における皮弁再建術後のハンドセラピィについて検討した。指尖部損傷に対して逆行性皮弁および前進皮弁で再建した30例31例を対象とした。全例生着し、手術からセラピィ開始までの日数は5.7日、セラピィ期間は14週であった。自動関節可動域において、DIP関節の可動域は、一期的な関節固定症例がないため、MP関節やPIP関節よりも低下した。逆行性皮弁は、手術からセラピィ開始までの日数は6.5日、セラピィ期間は13週であった。PIP関節の屈曲拘縮範囲は5〜46°で、強固な拘縮を1例認めた。前進皮弁は、手術からセラピィ開始までの日数は3.6日、セラピィ期間は15週であった。PIP関節の屈曲拘縮範囲は5〜16°で、強固な拘縮を呈した症例は認めなかった。

Burkhalter型splintを用いた基節骨・中手骨骨折の治療成績とハンドセラピィについて検討した。閉鎖性の基節骨骨折および中手骨骨折に対し、Burkhalter型splintを用いて治療した25例27指を対象とした。全例、X線所見上偽関節や癒合遅延例はなく骨癒合した。基節骨、中手骨とも10°を超える強い変形治癒は認めず、整復位は治療開始時期と比較し、概ね維持された。拘縮、変形.CRPS、経皮的鋼線固定例でのpin tract infectionなどの合併症は無かった。平均リハ期間は25ヵ月であった。Splintの平均装着期間は6週間であった。PIP関節の伸展不全は、17指中12指に生じたが、平均3.40°であり、最大でも12°であった。PIP関節の伸展不全は、12指中5指に生じ、平均3.40°であり、最大で14°であった。Reyesらの評価基準では全例excellentであった。

Purpose: The Cold Intolerance Symptom Severity (CISS) questionnaire was introduced by Irwin as a tool for measuring cold intolerance of hand patients. Although there have been many articles on cold intolerance and CISS from Europe, a very few were from Asia. We translated the CISS into Japanese. The aim of this study was to identify a cutoff value for the Japanese CISS in normal population and to evaluate its validity and reliability in hand patients. Translation Process: The original CISS was translated into Japanese by 2 Japanese professional translators independently. The 2 translations were synthesized and then translated backward into English by 2 professional English native translators. An expert panel, consisting of translators, a hand surgeon, and hand therapists, was held to finalize the Japanese CISS. Materials and Methods: One hundred forty-three healthy volunteers answered the Japanese CISS during winter season. The cutoff value was determined using 95% confidence interval. The validity and reliability of the Japanese CISS in hand patients were also investigated. During winter season, 111 outpatients with hand injuries completed the questionnaire and Disabilities of the Arm, Shoulder and Hand (DASH) (9 nerve injuries, 18 finger replantations, 11 tendon injuries, 24 phalangeal fractures, 35 distal radius fractures, 10 complex injuries, and 4 others). The mean age was 52.8 years (range, 16-84). To evaluate the test-retest reliability, 62 patients answered the questionnaire again with a 2-week interval. Statistical Analysis: DASH was used to assess concurrent validity of the Japanese CISS. The Spearman correlation was calculated to assess the correlation between the Japanese CISS and DASH. To evaluate the reliability of the Japanese CISS, Cronbach alpha and the intraclass coefficient (1.1) were calculated for internal consistency and test-retest analysis. Results: In a normal Japanese population, the average CISS score was 16.3 (standard deviation [SD], 15.2). Scores more than 50 were considered to show abnormal cold intolerance. In the hand patients, Spearman correlation between the Japanese CISS and DASH was 0.358, suggesting modest correlation. Cronbach alpha and the intraclass coefficient were 0.917 and 0.896, respectively, which showed the internal consistency, and the test-retest reliability of the Japanese CISS was both very good. Discussion: A cutoff value of the Japanese CISS was 50; it was similar to that of the Swedish version of the CISS. The Japanese version of the CISS was proved to be reliable with high internal consistency and test-retest reliability. It indicates good conceptual equivalence of Japanese version and comparability of interpretation.
