Prostate cell lines secrete IL-8 in response to Mycoplasma hominis through toll-like receptor 2-mediated mechanism

BACKGROUND. The roles of Toll-like receptor 2 (TLR2) and Mycoplasma in Prostate inflammation remain unclear. We investigated whether Mycoplasma induced inflammatory cytokine secretion through TLR2-mediated mechanism in prostate cancer cell line. METHODS. Cell surface expression of TLR2 on PC-3 cells were examined by flowcytometry. PC-3 cells were stimulated with Mycoplasina hominis (M. hominis), and IL-8 secretion and NF-kappa B activation were examined. RESULTS. PC-3 cells expressed TLR2 mRNA and cell surface TLR2 protein. The membrane fraction of M. hominis induced IL-8 secretion from PC-3 cells and NF-kappa B activation in a concentration-dependent manner. Transient transfection of the dominant negative mutant TLR2(P681H) into PC-3 cells attenuated M. hominis-induced IL-8 secretion and NF-kappa B activation. Antibody against the extracellular TLR2 domain significantly suppressed M. homnis-induced IL-8 secretion from the prostate cell lines including PC-3, PrEC, and transformed myofibroblasts. CONCLUSIONS. These results clearly demonstrate that the prostate cell line can secrete inflammatory cytokine in response to M. hominis through a TLR2-mediated mechanism.

Disinfection by antiseptics in management of postoperative surgical wounds in urologic operations

The objective of this study was to confirm that frequent disinfection by antiseptics is unnecessary on surgical wounds of urologic operations. Patients who received urologic operations were divided into 3 groups that had the same surgical dressings but different frequencies of disinfection and different antiseptics used. Surgical wounds were evaluated clinically and bacteriologically for the period until removal of sutures. Of the 97 patients randomly recruited for the study, 3 developed surgical-site infection (SSI). There was no significant difference in the incidence of SSI among the 3 groups. Bacterial counts of surgical wounds increased over time after operation, to similar extents in the 3 groups. The major isolate was Staphylococcus epidermidis, one of the normal florae on the skin. This strain was found at almost equal frequencies in all groups. In conclusion, our study suggested that covering with the surgical dressing without frequent disinfection by antiseptics was effective for prevention of SSI. Thus, traditional frequent disinfection should be abandoned.

Bacterial colonization on intraluminal surface of urethral catheter

Objectives. To determine the relationship between catheter-colonizing bacteria and urinary planktonic bacteria. The significance of catheter-colonizing bacteria for the pathogenesis of urinary tract infection remains unclear, although they are involved in the development of catheter-associated urinary tract infection refractory to antimicrobial chemotherapy. Methods. The subjects were 86 inpatients with urethral catheters. Urinary culture and catheter culture were performed simultaneously. The intraluminal catheter surface was swabbed to prepare a suspension. The suspension and a urine specimen from each patient were quantitatively cultured with the dip slide method and the microorganisms identified. Results. The catheters had been mainly placed to monitor urine output after urologic surgery, and their median indwelling period was 3.0 days (range 1 to 35). The overall positive rate of catheter culture was significantly greater than that of urine culture (53.5% and 30.2%, respectively, P < 0.01), even in patients without a recent antibacterial agent history. The difference was observed at day 2 (60% and 13.3%, catheter versus urine culture, respectively, P = 0.011) and days 3 to 6 (52.4% and 14.3%, respectively, P = 0.0 10) of the indwelling period, but was indistinguishable at day 14 and thereafter. The percentage of patients who had the same bacterial species isolated from both specimens increased in a time-dependent manner. Conclusions. The present results revealed considerable differences in the positive rates and bacterial species isolated from urine and catheter cultures, indicating that not all species of bacteria colonizing the intraluminal surface of the urethral catheter are detected as urinary bacteria. These results also suggest that bacterial colonization on the intraluminal catheter surface can precede the emergence of bacteriuria. (c) 2005 Elsevier Inc.

Surgical antimicrobial prophylaxis in transurethral ureterolithotripsy

The aim of this study was to establish a standard protocol for surgical antimicrobial agents for patients who received transurethral ureterolithotripsy (TUL). We retrospectively reviewed the medical charts of patients who received TUL. From October 2002 to December 2003, 2 days (total, four times) of antimicrobial prophylaxis (AMP) was done, and from January 2004 to December 2004, single prophylaxis was done. We analyzed the incidence of postoperative fever and other factors associated with TUL and compared these factors between the 2-day-AMP and single-AMP groups. Of 135 patients with TUL, 66 patients were in the single-AMP group and 69 in the 2-day-AMP group. The incidences of postoperative fever were 4.5% in the single-AMP group and 11.6% in the 2-day-AMP group. No statistically significant difference was found in the incidence of postoperative fever between the two groups. Our study showed that single AMP was effective for patients receiving a TUL operation. In the future, a prospective randomized study will be needed for a large series, with various types of lithotripters being included. © Japanese Society of Chemotherapy and The Japanese Association for Infectious Diseases 2005.

Incidence of and risk factors for surgical site infection in patients with radical cystectomy with urinary diversion

In radical cystectomy (RC), surgical-site infection (SSI) remains one of the most frustrating postoperative complications. Only a few reports have assessed SSI after RC according to the guideline authorized by the Centers for Disease Control and Prevention (CDC guideline). In this study, using the guideline, we assessed the incidence, causative organisms, classification, and risk factors for SSI in patients receiving RC with urinary diversion (UD). The subjects of this study were 104 patients who underwent RC with UD at the Department of Urology, Sapporo Medical University Hospital, between January 1996 and December 2003. As prophylactic antimicrobial agents, intravenous cephalosporins or penicillins were started before surgery and maintained until postoperative day 3. Patients who had preoperative bacteriuria were treated with antimicrobial agents before operation to eradicate the bacteria or reduce their number. The CDC guideline was used for the diagnosis of SSI. The overall incidence of SSI was 33%. The most frequent type of SSI was superficial incisional infection (71%). Methicillin-resistant Staphylococcus aureus (MRSA) was the most frequently isolated organism, accounting for 38% of the causative organisms. Operation time was shown to be a significant risk factor for SSI. Preoperative antibacterial chemotherapy for bacteriuria was not sufficiently efficacious to decrease the incidence of SSI. In our study, the incidence of SSI after RC was 33%, although 71% of the infections were superficial. It is necessary to establish more appropriate countermeasures to prevent SSI after RC with UD in particular, that caused by MRSA. © Japanese Society of Chemotherapy and The Japanese Association for Infectious Diseases 2005.

Clinical relevance of nucleic acid amplification test for patients with urinary tuberculosis during antituberculosis treatment

The nucleic acid amplification test (NAAT) has been valuable in the diagnosis of urinary tuberculosis however, no studies have attempted to determine the significance of NAAT post treatment. We encountered three patients with urinary tuberculosis who underwent sequential NAAT during antituberculosis chemotherapy and post treatment. All patients were diagnosed as having urinary tuberculosis by positive NAAT and specific renal deformity revealed by imaging. In two of the three patients, positive culture results were obtained and one was negative in standard culture. During antituberculosis chemotherapy, a negative NAAT was obtained from 3 to 5 months after the start of treatment and no positive culture results were obtained during the same period. At the end of chemotherapy, 6 months or more after the start of medication, all patients had negative NAAT results. These results suggest that NAAT for Mycobacterium tuberculosis provides an effective and rapid detection method for urinary tuberculosis both pre- and post-treatment. © Japanese Society of Chemotherapy and The Japanese Association for Infectious Diseases 2005.

A patient with seminal vesiculitis prior to acute chlamydial epididymitis

This is the first report of a case of seminal vesiculitis prior to acute chlamydial epididymitis. At the first visit to the clinic, the patient wished to check whether he had Chlamydia trachomatis in his genital tract, because his wife had been diagnosed as having chlamydial cervicitis. He had no specific symptoms at that time however, transrectal ultrasonograpy (TRUS) revealed swelling of seminal vesicles, which suggested the presence of seminal vesiculitis. Two days after the first visit, he had high-grade fever and was diagnosed as having acute epididymitis caused by C. trachomatis. We had previously reported that seminal vesiculitis was always complicated with acute epididymitis, so this case could provide important evidence that seminal vesiculitis might precede acute epididymitis. It suggested that acute epididymitis could be affected by seminal vesiculitis via the retrograde transmission route. © Japanese Society of Chemotherapy and The Japanese Association for Infectious Diseases 2005.

Incidence of sexually transmitted infections in asymptomatic healthy young Japanese men

The aim of this study was to estimate the incidence rates of asymptomatic sexually transmitted infections (STI) in young men in Sapporo, Japan. A survey of 204 healthy male volunteers was conducted. First-voided urine specimens were used for detection of Chlamydia trachomatis and Neisseria gonorrhoeae with polymerase chain reaction assay, and cotton swab wiping of the external genitalia was used for detection of human papillomavirus (HPV) with the Hybrid Capture method. The incidence rates were 3.4% for C. trachomatis and 5.9% for HPV. No N. gonorrhoeae was detected. In sexually active men who had regular sexual intercourse, the detection rate of C. trachomatis was 4.7% and that of HPV was 8.0%. Our study showed that the incidence rates of asymptomatic STIs were not negligible, and that we should provide screening programs for young men with a high risk for such unrecognized infections and study their natural histories. © Japanese Society of Chemotherapy and The Japanese Association for Infectious Diseases 2005.

Systemic inflammatory response syndrome in open versus laparoscopic adrenalectomy

Objectives. To compare the surgical invasiveness of laparoscopic adrenalectomy with that of open surgery when the invasiveness is evaluated by the incidence of systemic inflammatory response syndrome (SIRS). Methods. Patients who underwent open adrenalectomy (35 patients) and those who underwent transperitoneal laparoscopic adrenalectomy (35 patients) were included in this study. We retrospectively investigated the incidence and duration of SIRS in the two groups. Of the 70 patients, 39 had primary aldosteronism and 31 had clinically nonfunctioning adrenal tumors. Results. The incidence of SIRS was 51.4% (18 patients) in the open group and 31.4% (1 1 patients) in the laparoscopic group (P = 0.14, Fisher's exact probability test). The mean duration of SIRS was 1.9 days in the open group and 1.2 days in the laparoscopic group (P = 0.04, unpaired t test). Surgical site infection, which occurred in 2 patients in the open group (5.7%) and 3 patients (8.5%) in the laparoscopic group, was the only infectious complication. No patient had any severe or life-threatening complications. Conclusions. Laparoscopic adrenalectomy seems less invasive than open surgery, when the magnitude of the invasiveness is evaluated by the incidence of SIRS.

Membrane-anchored CD14 is important for induction of interleukin-8 by lipopolysaccharide and peptidoglycan in uroepithelial cells

We investigated the induction of interleukin-8 (IL-8) by bacterial lipopolysaccharide (LPS) and peptidoglycan (PGN) in the bladder cancer cell lines T24, 5637, UM-UC-3, and HT1197. T24 and 5637 cells strongly induced IL-8 after stimulation with LPS or PGN in a dose- and time-dependent manner, whereas UM-UC-3 and HT1197 cells did so very weakly. The expression of CD14 at the mRNA, total cellular protein, and cell surface protein levels differed among these cell lines, but the expression levels of Toll-like receptors 2 and 4 (TLR2 and TLR4) were not significantly different. The CD14 expression levels were found to correlate with the inducibility of IL-8 by LPS or PGN. Treatment of T24 and 5637 cells with phosphatidylinositol-specific phospholipase C to eliminate CD14 from the cell surface dramatically suppressed the induction of IL-8. On the other hand, UM-UC-3 cells transfected with CD14 cDNA expressed membrane-anchored CD14 and showed more efficent induction of IL-8 by LPS stimulation than untransfected controls. These results suggest that the presence of the membrane-anchored, but not the soluble, form of CD14 is a strong factor in IL-8 induction in bladder epithelial cells in response to bacterial components. The presence of the membrane-anchored form of CD14 may thus be a determinant for the inflammatory response of uroepithelial cells.

Is seminal vesiculitis a discrete disease entity? Clinical and microbiological study of seminal vesiculitis in patients with acute epididymitis

Purpose: To our knowledge direct evidence of inflammatory involvement of the seminal vesicles has not previously been reported in patients with acute epididymitis. We verified the discrete disease entity of seminal vesiculitis associated with acute epididymitis. Materials and Methods: The study included 13 patients who were clinically diagnosed with acute epididymitis. We report imaging, cytological and bacteriological findings in the seminal vesicles of patients with acute epididymitis. Results: On transrectal ultrasonography 12 of the 13 patients (92.3%) had dilatation of the seminal vesicle on the side ipsilateral to epididymitis. Dilatation of the contralateral side was found in only 4 patients. Seminal vesicle fluid from the ipsilateral side showed inflammatory findings in all patients. In patients 40 years and younger Chlamydia trachomatis was detected in seminal vesicle fluid in 7 of the 8 patients with epididymitis with results positive for the microorganism on first voided urine. Conclusions: Inflammatory responses were found in the seminal vesicles of patients with acute epididymitis. Chlamydia trachomatis was the causative pathogen most frequently detected in seminal vesicle fluid. Seminal vesiculitis is clearly associated with acute epididymitis and it may be a discrete disease entity.

The objective of this study was to provide precise data on the incidence of sexually transmitted diseases (STDs) in Hokkaido. The goal of this prospective surveillance, study was to clarify the STD incidence between 1998 and 2001 in Hokkaido, Japan. The incidence of gonococcal infection in men was found to be 127-199 per 100000 people per year, which was three or four times higher than that for women. Female genital chlamydial infection had an incidence of 300-400 with a female to male ratio of two or three to one. Younger adults had higher incidences of gonococcal and chlamydial infections than older people. In conclusion, the current study of STDs revealed high incidences of gonococcal and chlamydial infections in the Hokkaido area, and there was no decreasing trend in STD incidence during these 4 years.

Prophylactic antimicrobial agents in urologic laparoscopic surgery: 1-Day versus 3-day treatments

Although the incidence of surgical site infection (SSI) is generally low in laparoscopic urologic surgery, the standard protocol for prophylactic use of antimicrobial agents remains to be established. We retrospectively compared the incidence and severity of SSI after laparoscopic surgery between two different protocols for prophylactic use of antimicrobial agents. This study included 114 patients who underwent urologic laparoscopic surgery categorized as "clean" or "clean-contaminated" in Sapporo Medical University School Hospital between January 1996 and October 2002. As a prophylactic antimicrobial agent, one of the cephalosporins or penicillins was administered intravenously to all patients. For 46 consecutive patients between January 1996 and July 2000, an antimicrobial agent was given 30min before operation and thereafter every 12h on the same day and the next 2 days after operation (the 3-day group). For 68 consecutive patients from August 2000 to October 2002, an antimicrobial agent was given once 30 min before operation and was additionally given only in the evening or night of the day of operation (the 1-day group). The incidence of SSI was retrospectively investigated. There were two patients who developed SSI in each group (4.3% in the 3-day group and 2.9% in the 1-day group). The incidence of SSI was not significantly different between the two groups. The 1-day protocol has efficacy equal to that of the 3-day protocol in prophylaxis of SSI. The 1-day use of a prophylactic antimicrobial agent may be recommended for the clean or clean-contaminated urologic laparoscopic surgery described above.

Detection of human papillomavirus DNA on the external genitalia of healthy men and male patients with urethritis

Background. Only a few studies have been done involving detection of human papillomavirus (HPV) DNA on the external genitalia of men without genital warts, although many have been done for women. We conducted HPV DNA detection among healthy male volunteers and men with urethritis, both having no visible lesions on their external genitalia. Goal. The goal of the study was to determine the detection rate of HPV DNA in volunteers and patients with urethritis and to determine risk factor(s) for positive DNA. Study Design: This was a prospective clinical study. Results: HPV DNA was found in 1.3% of 75 volunteers and in 18.5% of 130 patients with urethritis. DNA of a high-intermediate oncogenic risk was more predominant than the low-risk type. Among various risk factors, only a history of STD was a significant factor for the positive detection of HPV DNA in multiple regression analysis. Conclusion: HPV DNA was found in patients with urethritis more frequently than in volunteers, probably because the former had higher sexual activity.
