胸郭低形成は、骨系統疾患や羊水過少が原因となって生じることが多い。骨系統疾患では四肢短縮を認めるが、今回胸郭低形成以外に骨所見はなく、羊水過少もない染色体不均衡型転座の症例を経験したので報告する。症例の母体は30歳、7妊2産。妊娠28週で胎児発育不全(fetal growth restriction:FGR)のため近医産婦人科より紹介となった。胎児エコーでは四肢短縮は認めないものの、ベル型の胸郭のため胸郭低形成が疑われた。羊水インデックス(amniotic fluid index:AFI)は5～7cmであった。他、エコースクリーニングでは心房中隔欠損(Atrial septal defect:ASD)疑いとなったが、致死的疾患は認めなかった。TORCH検索では異常は認められなかった。妊娠38週に帝王切開で2,011gの男児が出生し、アプガースコア4/9点、臍帯動脈血ガスpH=7.039であった。出生後、胸郭低形成、ASD、PDA(patent ductus arteriosus)、埋没陰茎、両側難聴などの診断となり、染色体検査に提出したところ46,XY,add(6)(p25)と診断された。両親の染色体を検査に提出したところ、母親が46,XX,t(6;11)(p25;q21)であり、母親由来の染色体不均衡型転座と判明した。児はPDAに対し開胸手術を必要としたが、術後経過順調であり自宅退院となり、3歳の時点で発達の遅れは伴うものの致死的疾患は生じていない。本症例は、6p25欠失と11q部分重複からなる不均衡型転座である。四肢短縮など他の骨所見のない胸郭低形成の症例は、染色体不均衡型転座も考慮する必要がある。(著者抄録)

Recurrent Massive Uterine Bleeding from a Cesarean Scar Treated Successfully by Laparoscopic Surgery.

A cesarean scar can cause abnormal uterine bleeding including prolonged menstruation or postmenstrual spotting. Our patient showed massive uterine bleeding from a cesarean scar and needed blood transfusion for hemorrhagic shock. A cesarean section had only been performed once for delivery stop 9 years ago. Recurrent hemorrhage could not be controlled by conservative treatment, and we performed laparoscopic scar resection and repair. The abnormal uterine bleeding was successfully stopped, and the menstrual cycle was normalized after surgical treatment. We should be aware that even an uneventful cesarean section may have a risk of massive hemorrhage postoperatively as in the present case.

Preeclampsia (PE) is related to an imbalance of angiogenic factors. Placental growth factor (P1GF) is reduced in early onset PE. Recent evidence indicates that metformin and statins may suppress endoplasmic reticulum (ER) stress. We previously reported that the unfolded protein response activated by ER stress down-regulated P1GF protein expression and predicted these drugs could prevent placental ER stress and up-regulate P1GF protein expression in trophoblast-like cells. In this study, we aimed to establish the effects of these drugs on P1GF and activating transcription factor 4 (ATF4) protein expression. We confirmed that P1GF messenger RNA (mRNA) levels were decreased under ER stress induced by thapsigargin and that ATF4 mRNA was increased under the same conditions and then administered metformin to it. Transcript analysis showed increased P1GF mRNA compared with thapsigargin only treatment and that this was dependent on metformin levels. Under ER stress, western blot showed high levels of ATF4 and phosphorylated-eukaryotic initiation factor 2 subunit a (p-eIF2a) but low levels of P1GF protein. By contrast, compared with thapsigargin alone, ATF4 and p-eIF2a levels were low and P1GF levels were high when metformin and thapsigargin were given, but these were again dependent on metformin concentrations. Western blot also confirmed that pravastatin attenuated ER stress and increased P1GF protein expression. In conclusion, metformin and pravastatin suppressed ER stress and restored P1GF levels in trophoblastlike cells. However, although these results indicate that these drugs have a potential for preventing or treating PE, the lack of clarity on the mechanism requires further study.

Currently, Clostridium perfringens enterotoxin (CPE) is being investigated as an anti-cancer drug for tumors expressing the tight junction (TJ) transmembrane proteins claudin-3 and/or claudin-4. However, the optimal conditions for CPE cytotoxicity are still unclear. Our objectives were to determine the optimal conditions for CPE as an anti-cancer drug for treating ovarian cancer in vitro and in vivo. In our experiments, cells at low culture density showed higher sensitivity to CPE, suggesting that claudins at TJs were poorly accessible to CPE compared with those at the edge of cell colonies. Ovarian cancer cells cultured under calcium-depleted pretreatment conditions to disrupt TJs and to knock-down TJ proteins and E-cadherin production altered CPE cytotoxicity, which was mainly dependent on claudin-4 expression. These results suggest that the condition of claudin-4 at the cell surface is important for CPE cytotoxicity. Our in vivo experiments showed that a high dose of CPE is required for the effective treatment of peritoneal dissemination of ovarian cancer cells. Here, we suggest that the accessibility of CPE to claudins is important for its cytotoxicity and depends on the conditions of claudin-4 in vitro. In addition, E-cadherin expression in ovarian cancer cells affects the efficiency of CPE in vivo.

Cervical adenocarcinomas are believed to lose estrogen response on the basis of no expression of a nuclear estrogen receptor such as ERα in clinical pathology. Here, we demonstrated that cervical adenocarcinoma cells respond to a physiological concentration of estrogen to upregulate claudin-1, a cell surface molecule highly expressed in cervical adenocarcinomas. Knockout of claudin-1 induced apoptosis and significantly inhibited proliferation, migration, and invasion of cervical adenocarcinoma cells and tumorigenicity in vivo. Importantly, all of the cervical adenocarcinoma cell lines examined expressed a membrane-bound type estrogen receptor, G protein-coupled receptor 30 (GPR30/GPER1), but not ERα. Estrogen-dependent induction of claudin-1 expression was mediated by GPR30 via ERK and/or Akt signaling. In surgical specimens, there was a positive correlation between claudin-1 expression and GPR30 expression. Double high expression of claudin-1 and GPR30 predicts poor prognosis in patients with cervical adenocarcinomas. Mechanism-based targeting of estrogen/GPR30 signaling and claudin-1 may be effective for cervical adenocarcinoma therapy.

CASE: A 32 year old woman was referred because of secondary amenorrhea, hirsutism, and voice deepening. OUTCOME: The blood testosterone level was markedly high. A transvaginal ultrasound revealed a small region in the left ovary, but whether or not it was a tumor was unclear. Therefore, selective ovarian venous sampling was performed. Consequently, the testosterone level was selectively increased in a blood sample that was taken from the left ovarian vein, the tumor was successfully localized, and a laparoscopic left oophorectomy was performed. Although the left ovary appeared to be normal at laparoscopy, the androgen-secreting tumor was located within it. The tumor was diagnosed as a Leydig cell tumor by histopathological analyses. CONCLUSION: This report demonstrates that selective blood sampling from ovarian veins before an operation is effective in localizing an androgen-producing ovarian tumor that is difficult to diagnose by imaging studies.
